What happened to UPS Flight 2976?
At about 5:14 p.m. EST on November 4, 2025, UPS Flight 2976, a Boeing (McDonnell Douglas) MD-11F registered as N259UP, crashed shortly after taking off from runway 17R at Louisville Muhammad Ali International Airport. It was a Part 121 domestic cargo flight from Louisville to Daniel K. Inouye International Airport in Honolulu. The NTSB records 14 deaths: the three crew members and 11 people on the ground. Another 23 people on the ground were injured. The NTSB’s case page is the official source for the accident record and investigation materials.
Why did UPS and FedEx ground their MD-11 fleets?
Boeing recommended that the three MD-11 Freighter operators suspend flights while it conducted additional engineering analysis. In its November 7, 2025 statement, Boeing said it supported the FAA’s airworthiness directive. UPS announced its temporary grounding that same day, saying it acted on the manufacturer’s recommendation and out of caution. UPS said MD-11s made up approximately 9% of its airline fleet at the time and that contingency plans were in place to maintain service. UPS’s statement said it grounded the aircraft before the FAA issued its directive.
The FAA issued its emergency action on November 8, making the grounding mandatory for operators covered by the order. This was a regulatory action, distinct from Boeing’s recommendation and UPS’s earlier company decision.

What did the FAA order?
The FAA’s November 8, 2025 emergency airworthiness directive prohibited MD-11 and MD-11F aircraft from flying until operators completed specified inspections and any necessary corrective actions. The agency said the initial directive applied to approximately 109 U.S.-registered aircraft. The FAA’s announcement describes the order’s scope.
On November 15, the FAA expanded its action to MD-10 and DC-10 aircraft after identifying shared design features. That later expansion was separate from the initial MD-11 directive.

Are UPS or FedEx still flying MD-11s?
As reported by the Associated Press on May 11, 2026, the FAA approved Boeing’s proposed corrective plan after review, and FedEx resumed cargo flights with some MD-11s that weekend. The report described replacement of a key spherical bearing and increased inspections of engine-to-wing attachment components. It does not establish that every FedEx MD-11 returned to service.
UPS, by contrast, retired its entire MD-11 fleet earlier in 2026, according to AP. The same report said FedEx had 46 aircraft of the type, with more than two dozen already in storage before the crash; that is a reported owned-aircraft count, not a count of aircraft then active. AP also reported that FedEx had planned to retire the MD-11 eventually even before the accident.

The carrier percentages come from different sources and dates: UPS supplied its approximate share in its November 7, 2025 statement, while AP reported FedEx’s approximate share on May 11, 2026. They should not be read as a direct comparison of active aircraft.
What is known about the corrective plan and maintenance history?
AP reported on July 2, 2026 that the crashed aircraft had accumulated 21,043 flight cycles. It also reported that the return-to-service plan calls for spherical bearings to be replaced every 4,000 takeoff-and-landing cycles. AP described earlier instances involving similar bearing or retaining-part failures. These maintenance details provide context for the corrective plan, but they do not establish the cause of Flight 2976’s crash.

Has the NTSB determined what caused the crash?
Not in the official NTSB material cited here. The NTSB held an investigative fact-gathering hearing on May 19–20, 2026, to help determine the facts and circumstances. Its case page lists ongoing investigation work and identifies the FAA, UPS, Boeing, the Independent Pilots Association, GE Aerospace, Teamsters Airline Division, and Collins Aerospace as participating parties. The reviewed official record did not announce a final probable-cause finding. The NTSB, not the FAA or the operators, is conducting the accident investigation; UPS has directed readers to the NTSB for official investigative updates.
