The Best Toothpaste for Toddlers: What Parents Should Choose

Use fluoride toothpaste from the first tooth, in a rice-sized smear until age 3. Professional guidance does not identify a brand proven best in a kids-and-parents test.

For toddlers, the best-supported choice is fluoride toothpaste used in the right amount—not a particular brand. Start when the first tooth appears. Until age 3, use no more than a smear about the size of a grain of rice, and have an adult dispense the toothpaste and supervise brushing. The available evidence does not document a kids-and-parents comparison test, so it cannot support naming a tested winner.

What toothpaste should you choose for a toddler?

Choose fluoride toothpaste, follow the product’s current label, and use the age-appropriate amount. The American Dental Association’s toothpaste guidance and parent-facing guidance from the American Academy of Pediatrics support using fluoride toothpaste from the first tooth. The American Academy of Pediatric Dentistry’s fluoride guidance also provides age-based amounts.

Those sources give preventive-care guidance; they do not compare brands, flavors, or current product labels. No documented test by children and parents is established here. A claim that one toothpaste tastes best, was preferred by families, or won a test needs actual product-specific test records.

When should you start, and how much should you use?

The ADA describes the under-3 recommendation this way: “The ADA recommends that children use a smear of toothpaste (the size of a grain of rice) from the time the first tooth erupts until age 3 years.” The ADA’s age-specific guidance is available on its toothpaste page; AAPD guidance also supports the age-based amounts.

Caregivers should dispense the toothpaste and supervise brushing. A rice-sized smear is not a full brush-length ribbon or a pea-sized blob. The quantities are maximums, not targets to exceed.

Why does the amount matter?

The ADA offers illustrative calculations showing how the amount can affect fluoride swallowed if a child swallows all the toothpaste. For a hypothetical 15 kg child, swallowing an approximately 0.1 g rice-sized smear containing 0.1 mg fluoride twice a day would mean 0.2 mg fluoride per day, or 0.013 mg/kg. Under the same assumptions, swallowing an approximately 0.25 g pea-sized amount containing 0.25 mg fluoride twice a day would mean 0.5 mg per day, or 0.033 mg/kg. These are ADA estimates based on stated assumptions, not measured outcomes in children.

Which toothbrush should you use, and when should a dentist see your child?

The AAP recommends a soft-bristled, child-size toothbrush. Adults should help with brushing until a child can manage it independently, commonly around age 8, according to its parent guidance.

The same AAP resource says the AAP and AAPD recommend establishing a dental home by a child’s first birthday. A dental professional can advise on the child’s oral care and individual needs.

What a product test would need to show

Fluoride guidance does not establish which toothpaste toddlers like best. A credible kids-and-parents comparison would identify the exact products and test date, the number and ages of child testers, how parents participated, and the criteria used. Taste and child acceptance should be reported as measured preferences, separately from clinical effectiveness. Without those details, a “tested best” ranking is not substantiated.
