The claim, exclusion, objection and W-9 deadlines have passed; the administrator says it no longer accepts W-9s and payment amounts cannot be disputed through the settlement process.
What the settlement covers
Jane Doe v. Lehigh Valley Health Network, Inc. was filed as a class action in 2023 in the Court of Common Pleas of Lackawanna County, Pennsylvania, docket 23-cv-1149. The complaint alleged that LVHN failed to adequately protect personal information exposed in a cyber incident announced on or about February 22, 2023. LVHN denied wrongdoing and said the class did not have a viable legal claim. The settlement resolves the case; it is not a court finding that the allegations were true or an admission of liability. LVHN’s June 23, 2023 notice said the organization was targeted by BlackCat ransomware, a group the notice associated with Russia. The notice said potentially affected information varied by person and could include names, addresses, phone numbers, medical record numbers, treatment and diagnosis information, including CPT codes, and health insurance information. The administrator’s FAQ says BlackCat published some class members’ information on the dark web. Lehigh Valley Public Media reported that the settlement covered 134,000 patients; that figure is attributed to the outlet and is not independently confirmed in the administrator’s visible summary. How the $65 million fund was allocated
The $65 million is the total settlement fund, not a promise that each person receives a set share. The administrator says the court approved the settlement and payment methodology on November 15, 2024. The agreement divided relief among four tiers based on the nature of the information or images published. The available administrator FAQ describes three allocations; it does not provide a complete Tier One description or a reliable per-person calculation. Settlement FAQ

The tier allocations are portions of the overall fund, not individual award amounts. Some payments were automatic for eligible class members after the settlement’s effective date, subject to court approval; out-of-pocket-loss reimbursement required a claim. The administrator says payments were determined under the approved methodology, and it does not provide a reliable individual payment formula in the FAQ.

What if you did not receive a payment?
Check the official administrator site for current notices and contact details, and direct questions about an individual payment or class membership to the administrator. The administrator says payment amounts cannot be disputed through the settlement process because the objection deadline passed and the court approved the payment methodology. It also says W-9s are no longer accepted. The site does not reopen those deadlines or offer a way to change a prior election.

The settlement FAQ says a class member who did not opt out and whose settlement became approved releases the defined claims in exchange for settlement benefits. It says someone who timely opted out receives no settlement money and may bring an individual suit concerning that person’s individual damages. Those are descriptions of settlement terms, not advice about a particular person’s legal rights.

Acceptance of a settlement is not proof that the defendant committed the conduct alleged. LVHN denied wrongdoing, and the case ended through a settlement rather than a court judgment finding the allegations true. Nor does the $65 million total establish a standard payment for every class member: relief depended on the agreement’s tiers and claim rules, and the administrator’s published information does not supply a dependable per-person calculation.
