Important version update: CS Form No. 212, Revised 2025 was the official Philippine Civil Service Commission Personal Data Sheet (PDS) under the 2025 Omnibus Rules on Appointments and Other Human Resource Actions. However, as of August 10, 2026, the CSC’s central download page lists CS Form No. 212, Revised 2026. If you are applying now, use Revised 2026 unless the vacancy notice or receiving HR office specifically requires Revised 2025.

This guide explains what the 2025 PDS is, where to download the authentic form and Work Experience Sheet (WES), how to complete every section, and which errors commonly cause an application to be returned or delayed.

What is CS Form No. 212, Revised 2025?

CS Form No. 212 is the Philippine government’s Personal Data Sheet, commonly called the PDS. The Revised 2025 version records an applicant’s identity, family background, education, eligibility, employment history, training, volunteer work, qualifications, legal and background information, references, photograph, and certification.

It is not simply a résumé. The PDS is a sworn government personnel record. By signing it, the applicant certifies that the information is personally supplied, true, correct, and complete, and authorizes the agency to verify it. The form warns that misrepresentation may lead to administrative or criminal cases. Read the official form and its guide before signing: official copy of the 2025 PDS.

Is Revised 2025 still the correct form in 2026?

Not automatically. The CSC’s current central download page now provides CS Form No. 212, Revised 2026, together with a Revised 2026 filling-out guide. For a new application submitted in 2026, Revised 2026 is the safer default unless the hiring agency’s notice says otherwise.

The 2025 version was officially adopted as Annex H-1 of the 2025 ORAOHRA. The related Work Experience Sheet was Annex H-2 and the filling-out guide was Annex H-3. The 2025 rules were promulgated through CSC Resolution No. 2500358 on April 30, 2025, published on July 18, 2025, and took effect on August 3, 2025. A transitory rule allowed agencies with old forms printed in bulk to use them until December 31, 2025. Appointments using the old forms beginning January 1, 2026 were to be returned for resubmission using the new prescribed form.

That transition does not mean every agency will reject a 2025 form in every circumstance. Government recruitment pages can lag behind the CSC’s central page. Some 2026 notices still refer to Revised 2025, while other current notices—such as a July 2026 DBM vacancy notice—request Revised 2026. The vacancy announcement and the receiving HR office therefore control the practical requirement for your application.

Situation What to do

Download the form from the CSC or from an official government agency hosting an exact copy. A Scribd upload may help you preview the layout, but it is not the issuing authority and should not be treated as the definitive version.

Revised 2025 files

Revised 2026 files

Always download the guide and WES that match the revision of the PDS you are completing. Do not combine the 2025 form with the 2026 guide.

Who uses the PDS?

The PDS is used by people applying for or holding Philippine government positions, including:

  • applicants for vacant government positions;
  • original appointees;
  • employees seeking promotion or transfer;
  • people seeking reemployment;
  • employees whose personnel records must be updated;
  • certain reappointment or renewal applicants, subject to the applicable exception; and
  • nominees or candidates for some government recognition programs.

Under the 2025 ORAOHRA, the PDS together with the WES is required for specified appointment actions, including original appointment, promotion, transfer, reemployment, and certain reappointments. The exact document package varies by transaction and agency. A vacancy notice may also require an application letter, résumé, proof of eligibility, diploma, transcript of records, training certificates, certificate of employment, performance rating, clearances, or a data-privacy consent form.

The rules also direct the HRMO to ensure that the PDS is updated annually for monitoring purposes. That does not necessarily mean every employee submits the same form through the same process every year; follow your agency’s HR instructions.

Prepare before filling out the PDS

Gather the information and records you will need before opening the spreadsheet:

  • government-issued ID and the required ID number;
  • civil service eligibility, board or bar rating, or professional-license records;
  • school records, including dates attended, graduation year, units earned, honors, and scholarships;
  • certificates of employment showing exact position titles and inclusive dates;
  • training certificates showing dates, hours, and the sponsoring institution;
  • volunteer organization details and hours rendered;
  • names and current contact details of three non-related references;
  • a recent photograph meeting the agency’s size and age requirements; and
  • the vacancy notice and its list of attachments, file-format rules, file-size limit, and submission channel.

Use one consistent date format throughout the 2025 form: dd/mm/yyyy. Keep supporting documents nearby so that you do not guess dates, license numbers, or employment details.

How to complete each section of Revised 2025

The Revised 2025 PDS has four pages and numbered items 1 through 42. Follow the labels and instructions on the official form rather than relying on an unofficial summary.

Section What to enter Common mistake
Personal Information Legal name, birth details, civil status, physical information, government identifiers, citizenship, addresses, and contact details. Guessing an ID number, using inconsistent names, or leaving an applicable field unexplained.
Family Background Spouse, children, father, and mother’s maiden name. Using the mother’s married name instead of her maiden name or omitting a child.
Educational Background Elementary, secondary, vocational or trade, college, and graduate studies. Using unexplained abbreviations or failing to state units earned when a program was not completed.
Civil Service Eligibility Eligibility or license, rating, date and place of examination or conferment, license number, and validity date. Listing a qualification without its exact title or omitting license details.
Work Experience Government and private employment, most recent first, inclusive dates, full position and employer names, status, and government-service indication. Omitting private employment or creating unexplained gaps in the timeline.
Voluntary Work Organization, address, dates, hours, and nature of volunteer work. Classifying paid work as voluntary work.
Learning and Development Training title, dates, hours, type of intervention, and full sponsoring or conducting institution. Listing only “CSC” instead of “Civil Service Commission,” or omitting hours.
Other Information Skills and hobbies, non-academic awards, and organization memberships. Listing unsupported skills or abbreviating organization names.
Legal and background questions Accurate answers to the questions in items 34–40, with details for every “Yes.” Leaving a “Yes” response unexplained or treating the section like an optional résumé disclosure.
References Three non-related people, with names, addresses, phone or mobile numbers, and email addresses where available. Using relatives or people whose contact details are outdated.
Certification and signature Certification under oath, signatures, dates, thumbmark where specified, ID information, and photograph. Signing only the final page or overlooking the oath and photograph rules.

Items 1–21: Personal Information

Enter your surname, first name, name extension, and middle name exactly and consistently with your official records. The section also asks for your date and place of birth, sex at birth, civil status, height, weight, blood type, and government identifiers.

The government identifiers listed in the 2025 form include UMID, Pag-IBIG, PhilHealth, PhilSys, TIN, and agency employee number. Do not invent or estimate a number. If a field does not apply, follow the guide and use N/A rather than leaving an ambiguous blank. Complete residential and permanent addresses, telephone and mobile numbers, and email address carefully because the agency may use them for verification or notification.

The citizenship portion also asks about dual citizenship. Answer it based on your actual legal status and provide the requested details. Do not select or describe dual citizenship merely because you were born abroad or hold a foreign passport without confirming what the form is asking.

Items 22–25: Family Background

Provide the spouse’s full name, occupation, employer or business name, business address, and telephone number where applicable. List all children and their birth dates in the prescribed name order.

For the parents section, provide the father’s full name and the mother’s maiden name—her name before marriage or while single. If a particular field does not apply, use N/A according to the guide rather than silently skipping it.

Item 26: Educational Background

Report elementary, secondary, vocational or trade courses, college, and graduate studies. For each applicable entry, provide the full school name, degree or course, period of attendance, year graduated, and highest level or units earned if you did not graduate. Include scholarships and academic honors.

Write full school and course names. For example, do not rely only on an acronym for a school, degree, or program if the official record identifies it more precisely. If you attended but did not complete a degree, state the highest level or number of units earned instead of implying graduation.

Item 27: Civil Service Eligibility

List every applicable civil service eligibility, license, or professional qualification. Depending on your circumstances, this may include career service eligibility, RA 1080 board or bar eligibility, special-law eligibility, CES or CSEE eligibility, or eligibility for uniformed personnel.

Enter the rating, examination or conferment date, place of examination or conferment, license number, and license validity date where required. Check the exact title against your certificate or licensing record. Do not list an eligibility that you have not earned or cannot support with official documentation.

Item 28: Work Experience

The 2025 form requires both government and private employment. Begin with your current or most recent work and move backward. For each position, provide inclusive dates, the full position title, the complete department, agency, office, or company name, employment status, and whether the work was in government service.

Use exact dates from certificates of employment or personnel records. If two jobs overlap, do not “correct” the dates to make the timeline look simpler; report the actual periods and be prepared to explain the arrangement if the agency asks.

A major version difference matters here: the official 2025 PDS copy does not show the Monthly Salary and Salary/Job/Pay Grade & Step Increment columns that the Revised 2026 guide instructs applicants to complete. This is one reason not to mix a 2025 template with 2026 instructions.

How to complete the Work Experience Sheet

The WES supplements—not replaces—the work-experience portion of the PDS. The PDS gives the employment timeline; the WES provides more detailed duties and responsibilities.

For a vacant-position application, the 2025 guide says the additional work-experience sheet should contain work experience relevant to the position being applied for. Describe the actual duties you performed, not just a list of job titles or generic résumé statements. Use the same employer names, position titles, and dates in the PDS and WES.

Attach the WES whenever the vacancy notice or applicable appointment rules require it. The WES is not automatically required for every government transaction, so check the specific notice. Use the official 2025 WES template for a Revised 2025 submission, or the agency’s current WES instruction when using Revised 2026.

Item 29: Voluntary Work

List the full name and address of the organization, inclusive dates, number of hours rendered, and your position or nature of the volunteer work.

Do not label an ordinary paid job as voluntary work simply because the employer was a nonprofit or community organization. Classify the activity according to the actual relationship.

Item 30: Learning and Development Interventions or Training Programs

List training from most recent to oldest. Include the full program title, inclusive attendance dates, number of hours, type of intervention, and full name of the conducting or sponsoring institution.

Use full institutional names. For example, the 2025 guide instructs applicants to write Civil Service Commission rather than only “CSC.” Verify the number of hours and dates against the training certificate.

Items 31–33: Other Information

These items cover special skills and hobbies, non-academic distinctions or recognitions, and memberships in associations or organizations. Use the full name of each association or organization. List skills that you can honestly support or explain; the PDS should not become a collection of unverified résumé keywords.

Items 34–40: Legal and background questions

These questions address matters such as:

  • relationships by consanguinity or affinity to appointing or recommending authorities;
  • administrative offenses;
  • criminal charges and convictions;
  • election candidacy or campaign-related resignation;
  • immigrant or permanent-resident status in another country;
  • separation from government or private employment;
  • indigenous-group membership;
  • disability status; and
  • solo-parent status.

Answer every question accurately. A “Yes” response is not a reason to omit the form or conceal the circumstance. The form requires details or specifications for every applicable “Yes.” Copy the exact question wording and numbering from the official form rather than relying on a third-party article that may have outdated language.

Item 41: References

Provide references who are not related to you by consanguinity or affinity. Include each person’s full name, office or residential address, telephone or mobile number, and email address where available.

Ask permission before listing someone as a reference, confirm that the contact details are current, and choose people who can reasonably verify your work, education, or character. Do not list a person who is unlikely to recognize your name or answer the agency’s call.

Item 42: Certification, signature, thumbmark, ID, and photo

The certification states under oath that the PDS was personally accomplished and that its contents are true, correct, and complete. It also authorizes verification and warns of administrative or criminal consequences for misrepresentation.

For the Revised 2025 form and guide:

  • sign and date the PDS at the bottom of every page;
  • provide the signature or right thumbmark in the specified box;
  • enter the required government-issued ID information;
  • attach a passport-sized or unfiltered digital photograph;
  • use a photograph measuring 4.5 cm × 3.5 cm; and
  • use a photograph taken within the last six months, unless the vacancy notice imposes a stricter period.

The appropriate signature method may depend on the agency’s process, such as a wet signature, accepted electronic signature, or digital certificate. Similarly, whether the certification must be notarized or sworn before an authorized agency officer depends on the vacancy notice and applicable instructions. Do not assume that notarization is universally required—or universally unnecessary.

This comparison is a practical guide, not a substitute for the specific vacancy announcement. The 2025 and 2026 templates and guides should be treated as separate documents.

Common reasons a PDS may be returned or questioned

1. Using the wrong revision

An outdated form may be rejected, returned for correction, or accepted only after HR confirms that it is appropriate. Check the revision printed on the form itself and compare it with the vacancy notice.

2. Mixing the 2025 and 2026 guides

Do not add 2026 salary or pay-grade information to a 2025 form because you found it in the 2026 guide, and do not omit fields required by the 2026 template. Download the matching files as a set.

3. Leaving unexplained blanks

Use N/A where a field truly does not apply, following the official guide. Do not use N/A for information that exists and can be verified, and do not leave an applicable field blank.

4. Omitting private employment

The Revised 2025 form says to include private employment. Leaving out a private-sector job can create unexplained gaps and inconsistencies with your résumé, employment certificates, or background verification.

5. Treating the PDS as a résumé

A résumé can be selective. The PDS is intended to be complete and verifiable. Do not omit responsive employment, eligibility, or legal information because it seems unfavorable.

6. Submitting an incomplete WES

A WES that repeats only position titles does not explain your relevant experience. Include actual duties and responsibilities, use the correct scope, and keep all dates and titles consistent with the PDS.

7. Missing signatures or dates

A signature on only the last page may not satisfy the 2025 guide or the agency’s instructions. Check every page, the certification section, and the WES if it requires a signature.

8. Incorrect photo

Check the photo’s dimensions, recency, digital or printed format, and any agency-specific requirements. The 2025 guide specifies 4.5 cm × 3.5 cm and a photograph taken within six months, but a vacancy notice may require a three-month-old or otherwise specially formatted image.

9. Inconsistent names or dates

Compare the PDS with your birth record, government ID, eligibility certificate, school records, licenses, and certificates of employment. A different spelling, birth date, license number, or employment period can delay verification.

10. Overlooking the submission rules

Some agencies require a single PDF, a specific file name, a maximum file size, or online-only submission. Others require printed and signed copies. The PDS itself does not replace the vacancy notice.

Recommended completion workflow

  1. Read the vacancy notice. Identify the required revision, deadline, attachments, signature method, oath or notarization instruction, photo rule, file format, and submission channel.
  2. Confirm the form year. Use Revised 2026 by default for a current application unless the agency expressly requires Revised 2025.
  3. Download the official files. Obtain the PDS, matching guide, and WES from CSC or the hiring agency’s official website.
  4. Build an evidence file. Gather identity, education, eligibility, employment, training, reference, and photo records.
  5. Complete the form consistently. Use legal names, full institutional names, dd/mm/yyyy dates for Revised 2025, and N/A for genuinely non-applicable fields.
  6. Complete the work history. Include government and private work, beginning with the most recent or current position.
  7. Complete the WES if required. Describe relevant duties and responsibilities and reconcile it with the PDS.
