Best Acne Serums and Leave-On Treatments: Choose by Active Ingredient

The active ingredient matters more than the word “serum.” Learn which acne treatments target different concerns, how to introduce one, and when to seek dermatology care.

There is no substantiated, dermatologist-reviewed ranking of 16 acne serums to recommend here. More importantly, “serum” does not tell you whether a product suits your acne: the active ingredient and the type of breakout matter more. For blackheads and whiteheads, salicylic acid or adapalene may be relevant; for mild pimples, benzoyl peroxide is an option; azelaic acid can help address acne and lingering dark spots. Choose one appropriate product, use it as directed, and give it 6 to 8 weeks before adding another.

Choose an acne product by its active ingredient

The American Academy of Dermatology (AAD) advises choosing an acne treatment based on its active ingredient, not the marketing label on the bottle. A serum may be one way to deliver an ingredient, but it is not inherently more effective than a gel, liquid, or other treatment form.

These are treatment options, not a head-to-head ranking. Which one fits depends on the kind of acne you have, how your skin tolerates a product, and whether a clinician should guide treatment. What the available product evidence can—and cannot—support

A verifiable 2% salicylic acid serum example

Thayers Double Action Acne Serum is a product example, not a “best” pick: its manufacturer lists salicylic acid 2% as the active ingredient and identifies acne-prone skin as its target. Those manufacturer statements establish the product’s stated formulation and audience, not comparative effectiveness or dermatologist endorsement. The evidence available does not establish a vetted list of 16 products, a dermatologist-review method for such a list, current prices or inventory, or comparative product-trial results. How to introduce an acne treatment

  1. Match the ingredient to the concern. Use the ingredient table as a starting point; for treatment recommendations tailored to your acne, ask a dermatologist.
  2. Choose one product. AAD advises trying one product with an appropriate active ingredient rather than starting several treatments together.
  3. Follow the product directions. Use the selected treatment as directed on its label or by your clinician; do not assume different products have interchangeable instructions.
  4. Give it 6 to 8 weeks. AAD recommends using the product as directed for this period before adding another, since fewer breakouts may take that long to appear. AAD treatment guidance.

When to see a dermatologist

  • Deep, painful pimples or acne that scars: seek a dermatologist’s care rather than relying on a cosmetic serum alone.
  • Pregnancy or trying to become pregnant: consult a dermatologist before self-treating with acne products, because treatment choice needs professional guidance.
  • Uncertainty about what you have or what to use: a dermatologist can help identify an appropriate treatment; prescription combinations may be suitable depending on the acne. AAD guidance on when to see a dermatologist for acne.

Skip toothpaste as an acne treatment

Toothpaste is not a substitute for an acne treatment. Harper advises relying on established acne-fighting ingredients instead: “Absolutely not!
