The one-time $90 Medicare Improvement Fund Premium Rebate is for people who live in the United States and have Original Medicare Part B, provided they do not receive Medicaid premium assistance and do not pay an Income-Related Monthly Adjustment Amount (IRMAA). Medicare Advantage enrollees are excluded. The Centers for Medicare & Medicaid Services (CMS) says most eligible people should receive the payment automatically in October 2026; no purchase is required.

Who is eligible for the $90 rebate?

CMS’s eligibility description sets out four conditions. You must:

  • Be enrolled in Original Medicare Part B.
  • Live in the United States.
  • Not receive premium assistance from Medicaid.
  • Not pay an Income-Related Monthly Adjustment Amount (IRMAA) on your Part B premium.

CMS published these criteria in its Medicare Improvement Fund Premium Rebate FAQ. The White House’s October 2, 2026 fact sheet also identifies Medicaid-paid premiums and IRMAA as exclusions.

Who does not qualify?

People enrolled in Medicare Advantage

CMS explicitly excludes people enrolled in Medicare Advantage. The announced eligibility category is Original Medicare Part B, not Medicare Advantage. Do not assume that changing plans will make you eligible for this one-time payment.

People receiving Medicaid premium assistance

If Medicaid helps pay your Medicare premiums, you do not qualify under the announced criteria. This exclusion concerns premium assistance, not simply whether you have ever used Medicaid.

People who pay IRMAA

IRMAA is an income-related surcharge added to certain Medicare premiums. The stated exclusion is whether you pay IRMAA; describing it only as an exclusion for “high-income people” could be misleading because the official criterion is the surcharge itself.

How much is the payment, and is it recurring?

The rebate is $90 once, not a monthly reduction in your Part B premium. CMS says it will be paid in October 2026. The standard 2026 Part B premium is $202.90 per month, according to CMS’s 2026 premium and deductible fact sheet; Medicare.gov also lists that standard amount and notes that premiums may be higher based on income at Medicare costs. The one-time rebate is less than half of one month of the standard premium.

When and how will eligible people receive it?

In its FAQ published October 3, 2026, CMS said most eligible beneficiaries should receive a direct deposit on or around October 8, 2026. CMS said most deposits would be made through the Social Security Administration. Some beneficiaries were expected to receive a paper check later in October, mailed to the address registered with Medicare. These are CMS’s announced delivery dates, not a guarantee that every eligible account will be credited on the same day.

CMS describes the payment as automatic. Its official materials do not require buying a product or paying for an application. If you have an eligibility question, call Medicare at 1-800-MEDICARE (1-800-633-4227). For payment-status questions, CMS says to call the Social Security Administration at 1-800-772-1213 beginning October 15, 2026.

Why is CMS issuing the rebate?

The White House says the payment comes from the Medicare Improvement Fund. CMS says Congress created that fund under section 7002 of the Supplemental Appropriations Act of 2008 to make improvements to Original Medicare fee-for-service coverage under Parts A and B. CMS estimates that 20.8 million Americans meet the announced criteria.

The Associated Press reported the administration’s announcement and quoted KFF executive vice president for health policy Larry Levitt: “The Medicare Improvement Fund language is quite broad and gives substantial discretion to the administration.” That is policy context reported by the AP, not an additional eligibility rule; CMS’s FAQ is the source for the announced criteria and payment details.
