Who Can Help With a Medicare Premium or Social Security Payment Issue?

Medicare handles premium billing, SSA handles Social Security payments and IRMAA, and plans handle coverage appeals. Here’s where to start and what to do next.

Contact Medicare about a Medicare premium bill or payment processing, and contact the Social Security Administration (SSA) about a Social Security benefit, a premium deduction from your benefit, or an income-related Medicare premium (IRMAA). If the issue is whether a service or prescription is covered, use the appeal process for Original Medicare or your plan. For help understanding options or paying premiums, a free State Health Insurance Assistance Program (SHIP) counselor or your state may help.

Start with the issue, not the word “Medicare”

A Medicare premium problem can involve several different decisions. Medicare handles its premium billing and payment processing; SSA handles Social Security benefit payments and IRMAA determinations. A coverage or service-payment disagreement belongs in Medicare’s or your health plan’s appeal process. Choosing the right starting point can prevent a billing question from being treated as a benefits question, or vice versa.

Your issue Start with What that contact handles
Medicare premium bill, payment history, returned payment, or Medicare Easy Pay Medicare: 1-800-MEDICARE (1-800-633-4227) Medicare premium billing and payment processing
Social Security payment amount or a premium deduction from a Social Security check Social Security Administration (SSA): 1-800-772-1213; TTY: 1-800-325-0778 Social Security benefit and deduction questions requiring review of your account or notice
IRMAA increase, request to lower IRMAA after a qualifying event, or dispute about income information SSA IRMAA determinations, requests for a lower amount, and reconsideration questions
Disagreement about coverage or payment for a service, item, supply, or drug Original Medicare or the relevant health or drug plan’s appeal process Review of a coverage or payment decision
Difficulty affording premiums or understanding options Your state and a local SHIP Questions about Medicare Savings Programs, Medicaid, and free Medicare counseling

The agencies do not substitute for each other: SSA does not process Medicare premium bills, and Medicare does not calculate Social Security benefits. For IRMAA, however, SSA is the contact even though the amount concerns Medicare.

A quick way to distinguish the routes is to ask what decision or transaction is in question. If you want to know whether a premium payment arrived, start with Medicare. If a Social Security deposit changed or a premium deduction appears on your benefit, SSA must review the benefit account. If the amount is an income-related surcharge, contact SSA about IRMAA. If Medicare or a plan refused to cover a treatment, drug, or other service, follow the appeal instructions attached to that coverage decision.

For a Medicare bill or payment that may not have processed

If you receive a Medicare premium bill, or you are unsure whether a payment was received, sign in to your secure Medicare account and check the “My premiums” payment history. For a billing or processing question, call Medicare at 1-800-MEDICARE (1-800-633-4227); the TTY number is 1-877-486-2048. If the payment was set up through your bank’s online bill-pay service, contact the bank about that payment arrangement as well; Medicare is still the place to ask about the Medicare bill itself.

Checking the account history can help answer the basic question of whether a payment appears as processed before you make another payment. If it does not appear, or the account history does not resolve what happened, contact Medicare with the bill and payment information available. For bank bill pay, the bank can address how its payment was set up or sent; it does not decide Medicare’s billing status.

Use the due date printed on your bill

Medicare says bills are generally due on the 25th of the month and recommends submitting payment at least five business days before the due date. Use the due date shown on your own bill rather than relying on a general schedule. If the bill is marked delinquent, take it seriously: Medicare warns that not paying the total due risks loss of Medicare coverage.

Do not treat the general due-date guidance as an extension or as confirmation that a payment has been credited. Check the bill itself, and allow the recommended processing time when planning a payment. If you have a question about a past payment or a delinquent notice, contact Medicare rather than assuming that a bank transfer or scheduled bill-pay instruction completed the transaction.

If Medicare Easy Pay is rejected or returned

Medicare says it will send a letter explaining why a Medicare Easy Pay payment was rejected or returned and issue a bill. Pay the amount due another way while the issue is being addressed; continue that alternative payment method until the amount due is paid and Easy Pay resumes. Do not assume that a failed automatic payment has cleared the bill.

Read the letter and bill together so you know what amount is due and why the automatic payment did not go through. If the letter does not answer a billing or processing question, Medicare is the contact for the premium account. The key practical point is to keep the bill current using another payment method rather than waiting for Easy Pay to restart.

For a Social Security payment or a premium deduction

Medicare premiums are usually deducted automatically from Social Security benefits. If you receive Social Security and your benefit amount or deduction looks wrong, contact SSA so it can review your account and the notice behind the amount. The SSA numbers given on its IRMAA guidance are 1-800-772-1213 and, for TTY, 1-800-325-0778.

If you are not receiving Social Security benefits, CMS generally sends your Medicare premium bill instead. The Social Security Administration explains that people not receiving Social Security benefits receive a quarterly Medicare premium bill from CMS with payment instructions. In that situation, a question about whether a Medicare premium payment was received belongs with Medicare, not SSA. SSA remains the contact for a Social Security benefit issue or an IRMAA determination.

This distinction matters if you are trying to trace a deduction or bill. A premium withheld from a Social Security benefit is part of a benefit-payment question for SSA to review. A separately mailed Medicare premium bill is handled through Medicare’s billing and payment channels. Calling the wrong agency may not resolve the underlying account issue, because each agency handles a different part of the process.

For an IRMAA increase or a change in income

IRMAA is the income-related monthly adjustment amount added to Medicare premiums for some people. SSA makes the IRMAA determination using modified adjusted gross income and tax filing-status information it receives from the IRS. SSA sends a notice explaining the decision and appeal rights, so use that notice to identify the reason for the amount and any applicable deadline.

Because SSA makes this determination from IRS information, a question about the surcharge is not the same as a question about whether a Medicare bill payment was posted. Contact SSA about the IRMAA amount and the basis for it. The notice is especially important: it explains the decision and the appeal rights that apply to your case.

If your income fell after a life-changing event

If your income has dropped after a qualifying life-changing event, ask SSA about requesting a lower IRMAA. Events SSA lists include marriage, divorce, the death of a spouse, loss of income, and an employer settlement payment. You can contact SSA directly or ask about submitting form SSA-44. The request is for SSA to consider a changed financial situation; a premium bill or a call to Medicare does not itself change SSA’s IRMAA determination.

When contacting SSA, explain which qualifying event occurred and that it reduced your income. Ask what information and supporting documentation are needed for your request. SSA can consider the changed circumstances through the SSA-44 process or direct contact; the request is not an automatic adjustment, so follow the agency’s instructions and retain the notice and records related to your case.

If the tax information used for the decision is wrong

A request based on lower income after a qualifying event is different from a claim that the tax information SSA used is incorrect. If the underlying IRS information is wrong, ask SSA about reconsideration and obtain IRS records supporting the correction. Follow the appeal instructions and deadlines in your SSA notice; do not assume that a life-changing-event request and a tax-data dispute use the same route.

In practical terms, separate these two questions before you call: “Has my income changed because of a qualifying event?” or “Is the IRS tax information on which this decision was based inaccurate?” The first may call for a request to lower IRMAA; the second calls for asking SSA about reconsideration and bringing IRS documentation that supports the correction. Use the route stated in the SSA notice and observe its deadline.

For a coverage or service-payment denial

A dispute over whether Medicare or a plan will cover or pay for a service, supply, item, or drug is not simply a premium-billing problem. For Original Medicare, follow Medicare’s appeal instructions. If you have Medicare Advantage or a drug or other health plan, use the appeal instructions in that plan’s materials. The appropriate appeal channel depends on the coverage involved, so start with the decision notice and the instructions that came with it.

Do not use a premium-payment inquiry as a substitute for an appeal. A question about what you owe for premiums and a challenge to a coverage decision are different matters with different processes. The notice from Medicare or your plan identifies the decision being challenged and the steps to take. Follow those instructions rather than assuming that a general customer-service call will count as an appeal.

A SHIP counselor can explain the process and help you understand your options. SHIPs are federally funded state programs that provide free local health insurance counseling, including assistance understanding appeals. They offer guidance; the appeal itself still follows the applicable Medicare or plan process.

If the problem is affording premiums

Ask your state about Medicare Savings Programs and Medicaid if premiums are difficult to pay. Eligibility and the assistance available are handled through state programs, so contact your state to ask what may apply to you. A SHIP can provide free Medicare counseling and help you understand where to direct questions about coverage and costs.

These are assistance routes, not replacements for resolving an incorrect bill, deduction, or appeal. If you need help with affordability, ask the state about available programs; if you also have an immediate Medicare billing question, contact Medicare about the bill. A SHIP can help explain Medicare options and where to take questions, while the state determines what program assistance may be available.

Prepare for the contact that fits your case

Before calling, identify whether you are asking about a Medicare bill, a Social Security benefit or deduction, an IRMAA decision, or a coverage appeal. Keep the relevant bill, payment record, decision notice, or plan materials available so you can describe the specific amount or decision. If a notice gives an appeal deadline, use that notice as your guide while contacting the responsible agency or plan.

For a payment question, check the “My premiums” history first and have the bill and payment details at hand. For a Social Security amount or deduction, be ready to identify the benefit or notice you are asking SSA to review. For IRMAA, distinguish a qualifying income change from incorrect tax information, and have the SSA notice available. For an appeal, use the decision notice and plan materials to follow the right channel. These steps help keep the conversation focused on the office that can act on the specific issue.

Official Medicare and SSA guidance accessed October 9, 2026, supports the contact routes and procedures described here. Contact details and procedures can change, so verify current instructions with the agency or plan and follow the notice tied to your case.

Frequently Asked Questions

Why was my Medicare premium deducted from my Social Security check?

Medicare premiums are usually deducted automatically from Social Security benefits. If the amount or deduction is unexpected, contact SSA at 1-800-772-1213 so it can review your benefit account and the notice behind the amount. If you do not receive Social Security benefits, CMS generally mails a quarterly premium bill instead.

Who do I call about a Medicare bill I already paid?

Check the “My premiums” payment history in your secure Medicare account. For a billing or processing question, call Medicare at 1-800-MEDICARE (1-800-633-4227), TTY 1-877-486-2048. If you arranged payment through your bank’s bill-pay service, contact the bank about that setup as well.

How can I lower my Medicare premium after my income dropped?

Contact SSA and ask about requesting a lower IRMAA after a qualifying life-changing event. SSA lists events such as marriage, divorce, death of a spouse, loss of income, or an employer settlement payment, and accepts requests through form SSA-44 or direct contact. Follow the instructions in your SSA notice.

What if the income information used for my IRMAA is wrong?

Ask SSA about reconsideration and obtain IRS documentation supporting the correction. This is different from requesting a lower IRMAA because income fell after a qualifying event. Follow the appeal instructions and deadline in your SSA notice.

Who can help if I cannot afford my Medicare premiums?

Contact your state to ask about Medicare Savings Programs and Medicaid. A local SHIP offers free Medicare counseling and can help you understand options, costs, and appeal processes.

Who handles an appeal if Medicare or my plan will not pay for a service or drug?

Follow the appeal instructions for Original Medicare or the relevant Medicare Advantage, health, or drug plan. A SHIP counselor can explain the process for free, but the appeal must go through the applicable Medicare or plan channel.

The Bottom Line

Bottom line: Call Medicare for a Medicare premium bill or payment-processing issue; call SSA for a Social Security benefit, premium deduction, or IRMAA decision. Use Medicare’s or your plan’s appeal instructions for a coverage denial, and ask your state about financial assistance. A SHIP can provide free help understanding your options.
