If your institution has lost access to a crypto wallet, first identify exactly what controls it and whether the problem is lost access or a possible compromise. Recovery depends on the wallet provider, account type, chain, signer setup, approval threshold, and recovery materials still available. Do not assume a provider can recover another provider’s keys, reverse an on-chain transaction, or bypass the wallet’s authorization rules.
What to do first
- Identify the wallet and account model. Establish whether it is a custodial exchange account, a provider-managed non-custodial wallet, a self-custodied smart-contract account, or another arrangement. Record the chain, wallet address, account type, signers, approval threshold, and which devices, credentials, key shards, or recovery materials remain accessible. The recovery authority depends on these details.
- Decide whether this is lost access or a suspected compromise. If a key, device, or account may have been stolen or exposed, treat the situation as a security incident and follow the institution’s incident-response process. Do not sign an unverified transaction or move funds to an address supplied through an unverified channel.
- Preserve the incident record. Record affected addresses, timestamps, relevant approvals, access changes, and recovery steps already attempted. Keep the record in an approved internal location and limit access to people who need it.
- Contact the provider through a verified support channel. Coinbase Token Manager’s guidance asks organizations reporting an inaccessible admin wallet to provide the organization name, admin wallet address, account type, and steps already tried. Follow the information requirements for your own provider.
- Never send support a seed phrase, private key, or recovery phrase. The reviewed Coinbase guidance does not ask institutions to disclose these secrets. Use only the provider’s documented recovery process and confirm any request through a trusted channel.
- Involve the right internal teams. Bring security, treasury, legal, finance, and compliance into the response to establish who may authorize recovery, assess operational and stakeholder impact, and determine whether notices are required under the rules applicable to your organization. Requirements and deadlines depend on jurisdiction; there is no universal notification rule established here.
Work out which kind of access was lost
“Admin wallet” can refer to different things: a person’s ability to use a signer device, the institution’s access to key material, or the owner key that controls a smart-contract account. Those are not interchangeable. A recovery process may restore a signer, replace a backup, or—in a separately configured system—move assets without restoring the original key.
Custodial account
For a custodial account, the provider controls the assets or signing process under its service model. Contact that provider using its verified incident channel and follow the account’s authentication and escalation requirements. A provider’s ability to restore account access is not evidence that it can recover a self-custodied wallet or override a blockchain authorization.
Provider-managed non-custodial wallet
A provider may supply software, signer coordination, or recovery tooling while the institution retains control of the wallet. Coinbase describes Prime Onchain Wallet as non-custodial. Its recovery instructions are specific to that product and should not be assumed to apply to other MPC or custody systems.

Self-custodied smart-contract account
In a smart-contract account, an owner or admin wallet may control account-level actions rather than simply provide access to a user interface. Coinbase Token Manager says its admin wallet can control configuration changes, escrow withdrawals, and ownership transfers. Coinbase also says it cannot recover the key, sign for the organization, or replace the on-chain owner without a valid signature from the current owner.
Coinbase Prime Onchain Wallet: documented recovery cases
Coinbase’s guidance distinguishes cases by which signer devices, key shards, and recovery passphrase remain available. These steps apply to Prime Onchain Wallet only; confirm the current procedure with Coinbase before acting.

Coinbase recommends considering at least three signers if possible. That is provider guidance for its product, not a universal institutional standard. Hardware requirements are also product-specific: the documented Prime recovery flow uses a YubiKey for administrator authentication, but the compatible model should be confirmed with Coinbase and the institution’s configuration.
If the admin key controls a smart-contract account
Loss of the owner or admin key can affect more than routine transaction signing. For Coinbase Token Manager accounts, the documented consequences can include losing the ability to change account configuration, withdraw escrow, or transfer ownership. If no valid signature or other recovery route remains, the institution’s options may be very limited; Coinbase says it cannot substitute its own signature for the missing owner key.

Token consequences can vary by account type and network support. Coinbase says some vested and unlocked stakeholder tokens may remain claimable, while unvested tokens or future vesting may no longer be administratively adjustable. Ask the provider to clarify the status of each affected allocation, and have legal and finance teams determine how to handle the consequences and communicate with affected stakeholders.
Key restoration is not the same as asset recovery
Restoring a key or signer restores the ability to authorize transactions with the existing wallet. A different recovery design can use transactions approved in advance to move assets to a backup destination without restoring the lost key. These mechanisms must be configured and authorized before an incident; they are not general remedies that can be added after access is lost.

Circuit Security describes its Automatic Asset Extraction product as using pre-signed transactions to sweep assets to a backup vault if keys are lost, systems fail, or a wallet is compromised. That is a vendor-described product capability, not a universal wallet feature or independent validation of its performance. Ripple describes an open-source recovery CLI for its wallet product and says its recovery path does not depend on Ripple or a single third party. Verify the current implementation and your institution’s setup before relying on either product-specific approach.

When reviewing an existing setup or choosing a future one, compare the actual recovery dependencies rather than relying on the label “institutional wallet.” The right questions are who can act, what must already be configured, and whether recovery still works when a key person, provider, or platform is unavailable.

Prepare before the next access incident
- Do not rely on one administrator. Coinbase recommends multisig or an institutional custody solution for admin access. Design the signer set and threshold for the institution’s risks rather than treating a provider’s suggested signer count as a universal rule.
- Maintain a controlled wallet inventory. Document wallets, chains, addresses, account types, signers, roles, approval thresholds, recovery materials, and escalation contacts. Store sensitive recovery materials under the approved security policy, separated from day-to-day signing where appropriate.
- Write down lifecycle procedures. Define key backup, signer recovery, employee role changes, offboarding, ownership transfers, and signer rotation. Coinbase recommends documenting ownership transfer and signer rotation procedures.
- Check access periodically. Coinbase recommends regular access checks. Confirm that more than one authorized person can use the required signing path, and that the documented recovery path still matches the institution’s configuration.
- Rehearse recovery safely. Test approved procedures and record the outcome without exposing production secrets or making an unapproved transaction. CoinCover describes regular tests and recovery drills as features of its own service; this is a planning consideration, not independent evidence of any provider’s performance.
- Map single points of failure. Identify dependencies on a provider, a particular administrator, a quorum, a backup location, hardware, or a platform. Establish who can act if the primary custodian or system is unavailable.
When recovery is no longer possible
If the provider confirms that no signer, key shard, passphrase, backup, or pre-approved asset-recovery mechanism can authorize access, do not try unverified recovery services or give anyone wallet secrets. Preserve the provider’s written response and the incident record, then have legal, finance, treasury, security, and compliance assess the account-specific effects, remaining claim paths, stakeholder communications, and applicable obligations. A lost credential does not by itself establish that every related token is unrecoverable; the outcome depends on the account, network, and remaining administrative or claim mechanisms.
