If your Social Security check went down after Medicare enrollment, first identify what is being deducted and why. Most people receiving Social Security have their Medicare Part B premium withheld automatically. A higher-than-standard deduction may also include an income-related monthly adjustment amount (IRMAA) for Part B, Part D, or both. The right next step depends on whether the deduction is expected, the income information is wrong, your income fell after a qualifying life event, or you need help affording coverage.
Find out what changed your check
Compare your current benefit statement with the notice from Social Security or Medicare. Look for the Part B premium and, if applicable, an income-related monthly adjustment amount (IRMAA). Check the amount, the month it applies to, and the reason stated in the notice. A smaller deposit does not by itself show that Social Security made a mistake: the deduction may reflect a new Medicare enrollment, a premium adjustment, or a separate income-based charge.

For 2026, the Social Security Administration (SSA) lists a standard Part B premium of $202.90 per month. Some beneficiaries pay more because of IRMAA. The $202.90 figure is the 2026 standard premium, not a prediction of how much any one person’s check will fall; an individual deduction depends on enrollment, income, filing status, and any applicable adjustment. Check the current SSA notice for your own amount.

Medicare says most people who receive Social Security or Railroad Retirement Board benefits have the Part B premium taken directly from their benefit. If you do not receive those benefits, you generally receive a Medicare premium bill instead. So a Part B deduction appearing on a Social Security statement can be ordinary premium collection rather than a penalty or an error.

If Medicare bills you and you have requested automatic deductions, do not stop paying the bill just because you expect withholding to begin. Medicare says deductions can take six to eight weeks to start; continue paying another way until the deduction begins. That delay affects how you pay, not the premium you owe.
IRMAA: an additional income-based amount
IRMAA is added to the standard premium for certain beneficiaries with higher income. SSA bases its determination on modified adjusted gross income (MAGI) and filing status reported through the latest federal tax return supplied by the Internal Revenue Service (IRS). The 2026 Medicare handbook describes use of a tax return from two years earlier. That means the income used for a current premium may not match what you earn now.
Read the determination notice rather than assuming every Medicare deduction is IRMAA. The notice should help identify whether the amount relates to Part B, Part D, or both, and the income and filing information used. Those details determine whether to correct tax data, appeal SSA’s determination, or request a new decision after a qualifying life change.

Choose the right route for your situation
These routes address different problems. A factual error in tax information is not the same as a later drop in income, and neither is the same as being unable to afford a correctly assessed premium.
| What the notice or circumstances show | What to do |
|---|---|
| Part B premium newly appears as a deduction | Confirm Medicare enrollment and the amount and period in your SSA or Medicare notice. Automatic withholding is typical for people receiving Social Security or Railroad Retirement Board benefits. |
| SSA used tax or filing information that is factually wrong | If the MAGI sent by IRS is wrong, correct the underlying information with IRS. Ask SSA about appealing or requesting reconsideration of its determination. |
| Your income fell after an SSA-recognized life-changing event | Consider requesting a new IRMAA determination with Form SSA-44 and supporting evidence. |
| The premium is difficult to afford because your income or resources are limited | Contact your state about a Medicare Savings Program, and check Extra Help if you need assistance with prescription drug costs. |
| You do not receive Social Security or Railroad Retirement Board benefits | Expect a direct Medicare premium bill and use Medicare’s payment instructions; automatic deduction guidance may not apply to you. |
If the tax information or SSA decision is wrong
Start by separating an error in the underlying IRS-reported information from disagreement with SSA’s decision. SSA says that if the MAGI it received from IRS is wrong, you should correct the tax information with IRS. Then follow the instructions on the SSA notice about the determination. Correcting information and appealing a decision are related but distinct actions; do not assume one automatically completes the other.
If you disagree with SSA’s IRMAA decision, SSA says you have the right to appeal. Its guidance describes an online appeal as the fastest and easiest route, and also permits a written Request for Reconsideration on Form SSA-561-U2 or contact with a local SSA office. Use the notice to identify what decision is being challenged and provide the explanation and supporting information requested. SSA’s process does not guarantee a particular outcome.

SSA distinguishes an appeal from a request for a new determination after an event that reduced income. Its guidance says an appeal is not needed for a request based on one of the listed income-reducing life events, or if the information SSA used has been shown to be wrong. That distinction matters: choose the process that matches the notice and your facts rather than filing an appeal simply because your current income is lower.

When a qualifying life-changing event may have reduced your income, Form SSA-44 is the route to ask SSA to use a new, lower income estimate for IRMAA. The form is intended for a beneficiary who received an IRMAA notice and experienced a listed life-changing event that may reduce income and the adjustment amount.
Events that may qualify
SSA identifies examples including marriage, divorce, the death of a spouse, loss of income, and an employer settlement payment. A lower-income year by itself does not establish that you qualify for this process. The event, its effect on income, and the evidence you provide must fit SSA’s criteria.

How to request a new determination
- Read the IRMAA notice. Confirm which year and income information SSA used, and identify the event that changed your circumstances.
- Get the current SSA-44. The version reviewed for 2026 determinations is dated December 2025. Verify that the current form and instructions still apply before filing, since forms and tax-year references can change.
- Complete the form and gather evidence. Explain the qualifying event and provide the supporting documentation required by the current form. SSA must be able to assess both the event and the resulting income change.
- Submit the request through an available SSA channel. SSA says requests can be made online, by completing SSA-44 and submitting evidence, or by arranging an appointment. The form also describes submission by fax or mail. Follow the current form or notice for the applicable method and destination.
- Keep copies and monitor the result. Retain the form, evidence, and submission record, then follow any instructions SSA sends. Approval and any change to premiums depend on SSA’s determination.
SSA’s IRMAA service page lists 1-800-772-1213 and TTY 1-800-325-0778 for assistance. Confirm current contact details and service options with SSA before calling.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. If the premium is unaffordable
If the deduction is accurate but hard to manage, check assistance programs rather than treating affordability as an IRMAA appeal. Medicare Savings Programs (MSPs), administered through states, can help pay Part B premiums and, depending on the program, other Medicare costs. Eligibility depends on income and resources, and state rules can differ from published federal limits. Medicare’s current MSP information provides 2026 limits; for example, the Qualified Individual (QI) and Specified Low-Income Medicare Beneficiary (SLMB) programs have different income and resource limits. Alaska, Hawaii, and state-specific rules can also affect eligibility. Do not rely on a limit from another state or assume that meeting a published limit is enough to establish eligibility.
Contact your state Medicaid office to ask about applying for an MSP and the rules that apply where you live. State assistance eligibility is a separate determination from SSA’s federal IRMAA decision: applying for an MSP does not itself change an IRMAA notice.

Extra Help is different. It reduces Medicare Part D prescription drug costs, while an MSP may help with Part B premiums and other Medicare costs depending on the program. If the problem is the cost of your prescription coverage, check Extra Help through the appropriate official channel. Confirm current eligibility requirements and whether you need to apply or reapply; state-specific rules and program administration can matter.
Documents and details to have ready
Before contacting SSA, Medicare, IRS, or your state office, gather the notice and the records relevant to your particular route. The right evidence varies, so follow the instructions for the form or appeal rather than sending unrelated documents.
- Your current Social Security benefit statement and Medicare premium or IRMAA notice, including the period and amounts in question.
- The tax return or filing information relevant to the income SSA says it used, if you believe the IRS data is incorrect.
- For an SSA-44 request, the current form’s instructions and evidence of both the qualifying life event and its income impact.
- For an MSP or Extra Help inquiry, the state or program’s current application requirements and information about your income and resources.
Keep the decision specific to your case
No general article can determine whether a particular deduction is correct or whether SSA will lower it. The outcome depends on the notice, tax and filing facts, benefit record, life event, and—in the case of assistance programs—your state’s eligibility rules. Use the notice as the starting point, ask SSA about an SSA determination, IRS about incorrect tax information, and your state Medicaid office about MSP eligibility.
