What Reports Say About Luigi Mangione’s Writings on Health Care

The viral Substack “manifesto” was not written by Luigi Mangione. Reports describe separate writings, but their sources and evidentiary status are different.

There was no single, publicly authenticated document that can safely be called Luigi Mangione’s “manifesto.” The viral Substack post often given that label was falsely attributed to him. Separate reporting described a handwritten document found when he was arrested, and later coverage reported excerpts from other writings in a Manhattan District Attorney filing. Those sources must be kept distinct.

Did Luigi Mangione write the viral Substack manifesto?

No. The widely shared Substack post titled “The Allopathic Complex and Its Consequences” was falsely attributed to Mangione. PolitiFact rated the claim false and reported that Substack removed the post for impersonation. Its Dec. 10, 2024 fact check said the related viral X post had more than 5 million views at that time; that was a dated count, not a current total. PolitiFact’s fact check addresses the attribution directly.

Lead Stories also reported Substack’s explanation: “We’ve removed this post for violating Substack’s Content Guidelines, which prohibit impersonation.” The outlet contrasted the post with phrases reported from a different handwritten note, while noting that journalist Ken Klippenstein published a transcription without providing proof of authenticity. That transcription is attributed reporting, not an independently authenticated official copy. Lead Stories’ fact check explains the distinction.

What did reports say the handwritten document contained?

Contemporaneous accounts described a handwritten document found when Mangione was arrested. Police characterized it as speaking to his motivation and mindset, but authorities had not publicly released the document as of PolitiFact’s Dec. 10, 2024 report. Accordingly, descriptions of its contents should be attributed to the outlets that reported them, rather than presented as a verified official transcript.

STAT reported that the document mentioned UnitedHealth Group once, criticized the rising cost of the broader health care system, and referred to health care critics Elisabeth Rosenthal and Michael Moore. STAT noted that the cited work by Rosenthal and Moore did not focus specifically on UnitedHealthcare. STAT’s account also included Rosenthal’s statement that she did not condone murder and that the killing was terrible and tragic. Her comments contextualize her reaction; they do not authenticate the document or prove its claims.

How are the later writings different?

Later coverage discussed excerpts from additional writings included in a Manhattan District Attorney filing. The Independent reported material concerning health care companies, possible targeting and hoped-for headlines. These were described as notebook or diary excerpts from a prosecution filing, not as a judicial finding about Mangione’s motive. They should not be collapsed into the separate handwritten note addressed to federal authorities. The Independent’s report describes the later material and its filing context.

Was Mangione’s mother denied insurance coverage?

The personal story that a denied insurance claim for Mangione’s mother motivated him appeared in the falsely attributed Substack post; the reporting reviewed here does not establish it as a statement by Mangione. The Associated Press reported that his public posts about back surgery were broadly positive and did not explicitly discuss corporate greed in health insurance. AP also said it was unclear whether he personally experienced insurance or billing problems. AP’s reporting therefore does not support claiming that a denied claim motivated him.

What the reporting does—and does not—say about health insurance

The reported writings and their circulation illustrate why attribution matters in a personal-finance context: a real or alleged critique of health care costs is not evidence that a specific insurer denied a specific claim, nor does it verify a viral personal story. STAT’s reporting says Rosenthal described KFF Health News and NPR’s Bill of the Month project as having received nearly 10,000 submissions over its six-plus years. That is a historical figure she gave in 2024 about the project, not a measure of denial rates or proof of Mangione’s personal experience.

STAT also reported that Rosenthal’s book described UnitedHealthcare payments to laboratories for vitamin D tests ranging from $17 to $618 in 2014. This is a book-reported historical range for those tests, not a current price or a general measure of insurer payments. The reporting does not provide a primary statistical study establishing broader claims about U.S. health care costs, life expectancy, denial rates or insurer behavior.

Why the label “manifesto” causes confusion

News coverage and social media have used “manifesto” for more than one writing. In this case, the term can blur three different things: a viral post falsely assigned to Mangione, a handwritten document described by early reporting, and later notebook excerpts reported from a court filing. Their provenance and evidentiary status differ. A reported excerpt can show what an outlet says appeared in a document; it does not, by itself, authenticate every circulating version or settle what motivated a crime.

As of PolitiFact’s Dec. 10, 2024 fact check, authorities had not publicly released the handwritten document, and PolitiFact said it had not obtained it. STAT said it requested a copy from the Manhattan District Attorney’s office and did not receive a response. This article therefore treats descriptions and excerpts according to their named reporting sources rather than implying that an official public copy has been independently verified.
