U.S. family caregivers may find help through state Medicaid programs, paid family leave, long-term care insurance, tax benefits, local aging services, or programs for eligible veterans. None is automatic: the right path depends on the care recipient’s coverage and needs, the caregiver’s state and work situation, tax eligibility, and—in veteran programs—the person’s military and VA eligibility.
These supports are not all payments to caregivers. Some may pay a family caregiver directly; others provide protected time off, replace part of a worker’s wages, reduce a tax bill, or offer respite and practical services. Start by identifying which kind of help you need, then contact the agency or organization that administers that program.

Which caregiver benefits are available, and who receives them?
There is no single national caregiver benefit that applies to every family. The options below have different eligibility rules and administrators. Some support the caregiver directly, while others benefit the person receiving care or make care easier to arrange.

Compare options by asking four questions: Who receives the support? Is it cash, wage replacement, protected time off, a tax reduction, respite, a referral, or another service? Which organization decides eligibility? And is the help ongoing, tied to a defined leave or service, or claimed for a particular tax year? For example, Medicaid self-direction may pay a caregiver if the state permits it; FMLA protects eligible time away from work but does not pay wages; and local aging services may offer practical help rather than cash.
Can Medicaid pay you to care for a family member?
Sometimes. USAGov says a state may let a family member or friend become a paid caregiver when the person receiving care already has Medicaid. These arrangements are often called consumer-directed or self-directed personal assistance. They are state-administered, and payment is not automatic simply because someone provides care.
How to check a state program
- Confirm whether the person receiving care has Medicaid in the state where they live.
- Contact the state Medicaid office and ask specifically about self-directed or consumer-directed home and community-based services.
- Ask whether relatives can be paid caregivers under the particular program, and whether any relationship-based limits apply.
- Request details on the recipient’s assessment, service plan, enrollment, caregiver qualifications, authorized hours, and payment process.
- Get the rules and next steps in writing before relying on caregiver income.
There is no national payment amount established for these programs. The state Medicaid office is the source for current availability, rates, and eligibility.

Can a caregiver take paid leave from work?
Possibly, depending on the state. Some states require employers to offer family leave programs, and some provide pay to workers taking leave to care for a family member. The state program’s coverage, pay, duration, and eligibility rules vary; check with the state labor office using USAGov’s paid family leave information.
Federal FMLA is job-protected but unpaid
The federal Family and Medical Leave Act (FMLA) is separate from state paid leave. For eligible employees of covered employers, it allows up to 12 weeks of unpaid leave in a 12-month period for qualifying family or medical reasons. It provides job protection and continued access to group health benefits while leave is taken, subject to the law’s conditions.

Eligibility includes working for a covered employer, having at least 12 months of employment, having worked at least 1,250 hours in the preceding 12 months, and working at a location where the employer has at least 50 employees within 75 miles. These are among the requirements, so check the details with your employer or the Department of Labor. USAGov’s FMLA overview was last updated March 28, 2025, and describes the leave as unpaid: FMLA eligibility and leave.
State paid leave may coexist with FMLA or follow its own rules. Do not assume that qualifying for one means qualifying for the other, or that FMLA itself replaces wages.

Can long-term care insurance pay a relative caregiver?
Some long-term care insurance policies may cover care provided by a family member, but coverage depends on the contract. A policy may define eligible caregivers, covered services, documentation, waiting periods, or benefit limits in ways that affect whether a relative can be paid.
Ask the insurer or agent to confirm the relevant policy language and requirements in writing before arranging care on the assumption that the policy will reimburse it. USAGov includes long-term care insurance among possible caregiver support routes in its caregiver information.
Which tax benefits might apply to a family caregiver?
Tax benefits depend on the taxpayer, the person receiving care, the expenses, and the tax year. They are not caregiver wages, and not every caregiver or dependent qualifies. Check the IRS rules for the return year at issue before claiming a credit.

Child and dependent care credit
A caregiver who paid qualifying care expenses for a child, spouse, or other dependent while working or looking for work may be eligible for the child and dependent care credit. Whether expenses and a person qualify depends on the applicable IRS requirements. Review the IRS’s current instructions and eligibility criteria rather than relying on a general description of the credit.

The Credit for Other Dependents may apply to someone responsible for an eligible dependent who does not qualify for the Child Tax Credit. USAGov lists a maximum of $500 per qualifying dependent; that is a credit limit, not a guaranteed payment to every caregiver. Confirm the current tax-year rules with the IRS. What local help can an Area Agency on Aging provide?
An Area Agency on Aging may connect caregivers and older adults with counseling, care referrals, adult day services, and meal delivery. Availability depends on the local area, and these supports are services or referrals—not guaranteed cash benefits. They can still help reduce care-related costs or give a caregiver time away from hands-on responsibilities.
Find a local agency by ZIP code or by city and state through USAGov’s caregiver benefits page. When you call, describe the care recipient’s needs and ask about both caregiver support and services for the person receiving care.
What help is available if you are caring for a veteran?
When the person receiving care is a veteran, several VA-related programs may be relevant. Each has its own eligibility requirements; contact the VA or the relevant pension management center to determine whether a particular veteran or survivor qualifies.

Veteran-Directed Home and Community-Based Services
This program may give an eligible veteran a flexible budget and counselor support to arrange services, which can include hiring a family member. Ask the VA about availability and eligibility for the veteran’s location and circumstances.
VA Respite Care
Respite care may provide a caregiver with a temporary break while the veteran’s care needs continue to be addressed. Ask the VA what services are available and what eligibility or scheduling rules apply.
Aid and Attendance
Aid and Attendance may add monthly payments to a VA pension for qualified veterans and survivors. The additional support can help cover caregiver costs, including care from a family member. It is a pension-related benefit for eligible recipients, not a universal caregiver stipend.

Start with the support that matches the problem you need to solve. If you need ongoing payment for care, ask the state Medicaid office about self-directed services, then review any long-term care insurance policy. If you need time away from work, check state paid leave and FMLA separately. If care costs affect your taxes, review the applicable IRS rules. If you need practical assistance or respite, contact the local aging agency or, for veteran care, the VA.
- Write down the state, coverage, and care arrangement: the care recipient’s state, Medicaid or VA status, and the caregiver’s relationship to them can determine which office to contact.
- Ask who receives the benefit: distinguish direct caregiver pay from wage replacement, a tax credit, a pension supplement, or services provided to the care recipient.
- Verify requirements before making financial plans: ask for written eligibility criteria, application steps, and any limits on hours, expenses, or covered services.
- Track dates and records: employment history, care expenses, policy documents, and program correspondence may matter for an application or tax return.
Because these programs are administered by different organizations, one denial or lack of availability does not establish that every other route is closed. Check each pathway with its responsible state office, employer, insurer, IRS guidance, aging agency, or VA program.
