A checklist is a visible sequence of prompts that helps someone carry out a task or confirm that important actions are complete. It can reduce reliance on memory and make omissions less likely, especially when work is repetitive, distracting, or shared with others. It supports reliable action; it does not replace knowledge, judgment, training, or a sound process.
What is a checklist?
A checklist is a written or displayed set of items used either to guide a task or verify that key steps have been completed. Instead of remembering every detail unaided, a person can consult the list, act, and mark or otherwise confirm each item.
Some checklists remind an experienced person of essential steps. Others help a team pause together and verify that agreed safeguards are in place. A checklist is not necessarily a full set of instructions, and it is not automatically a decision-making tool. Its purpose and intended users should be clear.
Why are checklists important?
People can forget or skip steps because of distraction, fatigue, interruptions, information overload, or an unusual departure from routine. A checklist makes important prompts visible, can encourage a consistent sequence, and gives people working together a shared reference point.

That is useful in ordinary tasks such as packing for a trip or keeping up with recurring household responsibilities. It also matters in safety-critical settings, including healthcare and aviation, where missing a step can have serious consequences. A checklist is most useful when the task has clear, agreed steps and a meaningful risk that one may be overlooked.
How can checklists help prevent mistakes?
Checklists externalize memory: they let a person see a prompt rather than rely entirely on recall. The Agency for Healthcare Research and Quality’s Patient Safety Network describes checklists as a way to standardize steps and help reduce errors associated with slips, distractions, or lapses. They can also give a team a common moment to confirm that important actions have been addressed.

A well-known example is the World Health Organization’s Safe Surgery Saves Lives pilot. Across eight hospitals in eight cities, major inpatient complications declined from 11% before implementation to 7% afterward, while inpatient deaths following major operations declined from 1.5% to 0.8%. The data were collected from October 2007 through September 2008, and the results were published in the New England Journal of Medicine in January 2009. These are outcomes from a particular clinical team intervention that included a checklist, pauses, teamwork, communication, and implementation activity—not proof that every checklist produces the same results.
WHO also describes studies with differing results. A mandate by itself is not enough: implementation takes time and effort, and how the checklist is completed and the context in which it is used matter.

When should I use a checklist?
Consider one when a task recurs, contains steps that are easy to forget, or requires several people to coordinate. It is less suitable as a substitute for flexible reasoning when there is no agreed sequence or when the work depends primarily on expert judgment.
- Use a checklist to prepare for a trip, complete a recurring home task, or make sure routine preparations are finished.
- Use a team checklist when people need to verify agreed safeguards or coordinate a pause before a consequential action.
- Do not treat a checklist as a complete procedure if users also need detailed instructions, training, or the authority to adapt to changing circumstances.
If there is no consensus about which actions are correct, a checklist may have no sound basis. Select the task and steps deliberately rather than creating a list simply because a checklist seems reassuring.

How to design and implement a useful checklist
- Define the purpose and user. Identify the task, who will use the list, and which omissions or errors it is meant to address.
- Agree on the steps. Base prompts on evidence-based or otherwise agreed actions. Involve the people who will use the checklist so it fits their work.
- Make prompts clear and actionable. Keep the list concise enough to complete reliably, without removing important safety steps just to shorten it.
- Fit it to the setting. Make sure the list works with the actual workflow, equipment, and environment where the task happens.
- Test and review it. Try it in context, then check whether people use it and whether it helps meet the intended goal. Planning, education, leadership support, and ongoing review can all matter.
Publishing or requiring a checklist does not ensure it will work. The AHRQ Patient Safety Network notes the importance of leadership engagement, a supportive safety culture, and monitoring both checklist use and related outcomes. WHO likewise emphasizes staff involvement, education, time for implementation, and thoughtful adaptation.

These are practical trade-offs, not evidence that one format is more effective in every situation. Choose the format that makes the checklist usable in the real workflow.
What are the limits of checklists?
A checklist can become a box-ticking exercise: a user may mark items without carrying them out carefully. High completion rates do not guarantee good performance. As Robyn Clay-Williams and Lacey Colligan put it, “Checklists are excellent ‘aides memoire’ and directives to correct procedures, but they are not a panacea.”
Checklists can also take time or be a poor fit for work that requires flexible reasoning. AHRQ’s review of clinical reasoning checklists notes that evidence often comes from experimental settings and may overstate effects in routine practice; it also identifies possible time costs and increased ordering of tests or imaging. These cautions concern that clinical context, not every checklist in every field.
The practical test is whether the list helps the intended users carry out agreed actions in context. If it adds burden without improving the task, encourages mechanical compliance, or obscures the need for judgment, it needs to be revised or reconsidered.
