Where to find the Tennessee application
The official program is the Surgical Assistant Registry, administered through the Tennessee Board of Medical Examiners. Tennessee says surgical assistants practicing in the state must be registered.
The registry page links the Application Instructions and Application for Registration as a Surgical Assistant. The PDF is marked PH-0235 Rev. 02/17, so check the registry page again before filing to make sure you are using the current form and fee.

Who may qualify for registration
Tennessee Code Annotated § 63-6-219 lists three qualification pathways. The statute says, “No person shall use or assume the title ‘registered surgical assistant’ unless such person is registered with the board as a registered surgical assistant.” Confirm your eligibility against current Board materials and the current code before applying.

The statutory qualification language is reproduced in a Tennessee General Assembly publication of Tennessee Code Annotated § 63-6-219. For legal reliance, verify the provision against the current codification.
How to apply
- Check the live registry page. Start at the Tennessee Surgical Assistant Registry, open the linked application, and confirm the form and fee before preparing a mailing.
- Complete application pages 1–6. The PDF instructs applicants not to leave prompts blank. Include the supporting materials for your qualification pathway.
- Arrange any required third-party verification. For military training, have the program’s administrative office send Attachment 1 directly to the Board. If you hold or have held a license in another medical profession, send Attachment 2 to each relevant jurisdiction.
- Include background and eligibility documents. The instructions list a criminal background check and proof of U.S. citizenship or legal entitlement to live or work in the United States. All applicants are directed to complete, sign, and notarize the Declaration of Citizenship and include the documents it requires.
- Verify payment instructions and mail the packet. The linked PDF lists a total fee of $60 ($50 application fee plus $10 state regulatory fee), payable by check or money order in U.S. funds. It also says fees are non-refundable. Because the form is marked revision 02/17, confirm the current amount and payment instructions on the registry page before sending payment.

Processing times and missing documents
The application instructions say to allow 14 working days for mailed information to reach and be placed in an applicant’s file. This is a handling instruction, not a promise that the full application will be processed within 14 days.

If the Board identifies missing documents, it may send a deficiency letter. The form instructs applicants to provide the requested supporting documents within 90 days of that letter; otherwise, the file may be closed. These form instructions do not establish a general registration deadline of October 8, 2026.
