PFRDA’s Form A2, version 1.0 dated March 2025, is the UPS migration form for eligible Central Government employees currently subscribed to NPS. It collects subscriber and service details, records the employee’s UPS option, and describes the DDO and PAO processing steps. Before signing or submitting it, verify your eligibility, the applicable form version, the route and any current deadline with your DDO: PFRDA says an exercised option is final and cannot be changed.
Get the applicable Form A2
The official document is titled “UNIFIED PENSION SCHEME (UPS) – SUBSCRIBER MIGRATION FORM – Government Sector.” The PFRDA Form A2, version 1.0 (March 2025) is intended for an eligible Central Government employee presently subscribed to NPS. The form refers to the Government’s UPS notification of 24 January 2025 and PFRDA’s UPS operationalisation regulations.
Check PFRDA and your employing office for any later applicable version before using a saved or printed copy. Form A2 is not, by itself, proof that a migration window is open now.
What information does Form A2 require?
Subscriber declaration
The subscriber section asks for the employee’s name and parentage, PRAN, and signature. The employee confirms being an NPS subscriber as at 1 April 2025 and declares the UPS option. The form’s wording is: “I do hereby exercise the option to be covered under Unified Pension Scheme (UPS).” It also includes authorization to collect and share necessary personal details for scheme purposes.

DDO-certified service details
The DDO section is completed and certified using service records. It includes the employee code or ID, commencement date of qualifying service, current month’s basic pay, NPA if applicable, and scheduled next increment date. The form also provides for DDO and PAO names, signatures and registration details.
How submission and processing are described
PFRDA’s UPS presentation dated 31 July 2025 describes Form A2 as submit-able online or to the DDO. The form itself sets out duties for the office-side process:

- For a physical submission, the DDO enters data verified by the Head of Office in the Central Record Keeping System and uploads the signed form.
- The signed form is retained in the employee’s service record, and a copy is provided to the employee.
- The PAO authorizes and approves the option through the CRA system.
Follow the route and document-handling instructions currently given by your DDO; the form’s description does not establish that every office uses an identical operational process.
Employees on deputation or foreign service
A PFRDA circular dated 24 September 2025 addresses physical Form A2 submission by Central Government employees on deputation or foreign service. It describes a specific circumstance, not a general physical-submission route for all employees, and it does not establish that a general migration window remains open in October 2026.

Check before exercising the option
PFRDA’s 31 July 2025 presentation says that once exercised, the UPS option is final and cannot be changed. The form also asks the employee to attest that they have read and understood the applicable provisions and are eligible. Confirm eligibility and the live procedure with your DDO or PFRDA before signing or submitting.
If you need to understand the legal or benefits effect of the choice, consult the current governing material rather than relying on the form alone. The NPS Trust Acts and Regulations index lists the 2025 UPS operationalisation regulations and a later amendment; Form A2 refers to the regulations as amended from time to time.

What happens to the existing NPS corpus?
PFRDA’s UPS FAQ listing dated 25 March 2026 includes the question, “What will happen to my existing corpus on migration from NPS to UPS?” Its search-result snippet says the corpus is transferred, but does not expose the complete answer or detailed conditions. See the PFRDA UPS FAQs and confirm the current treatment with your DDO or PFRDA.
An earlier PFRDA form dated 27 January 2025 described shifting the employee’s and employer’s NPS corpus contributions through March 2025 and said the completed migration form should be certified by the government nodal office or employer. That dated wording is not a substitute for current guidance on corpus treatment. The earlier Option to Shift form should not be confused with the March 2025 Form A2.
