What happened in the March 18 strike?
The Associated Press reported that social workers, psychologists and other Kaiser Permanente mental-health professionals walked out for one day in Northern California. The clinicians served an estimated 4.6 million patients across the San Francisco Bay Area, Central Valley and Sacramento regions. More than 23,000 Kaiser nurses also joined the action. The action took place during contract negotiations between Kaiser and the National Union of Healthcare Workers (NUHW). AI was one part of a wider labor dispute, alongside concerns about clinical work and the time and resources available to care for patients.
What did workers say AI was changing?
Workers’ objections were not limited to a claim that therapists had already lost their jobs to AI. Licensed clinical social worker Ilana Marcucci-Morris said triage work she had done was reassigned, and that screening was being handled by unlicensed lay operators using scripts or by e-visits. Clinicians also objected to management demands to spend less time on notes and patient messages. They feared that efficiency initiatives, potentially including AI, could mean more appointments with less time and fewer resources. Kaiser psychiatrist and union steward Dr. Emma Olsen characterized the concern to AP as management wanting clinicians “to be seeing more people for less time with less resources.” That is a worker’s description of the workload dispute, not an independently established finding about the employer’s motives. What did Kaiser say about AI and patient care?
Kaiser told AP that the union’s claim was false. The employer said its Northern California operation did not then use AI for therapy and that AI would not replace human assessment or make patient-care decisions. Kaiser described AI as a tool intended to support clinicians and access to care. These are the employer’s statements about its current use and intended role for the technology; they do not independently establish how AI may be used in the future or demonstrate its safety. In the same AP report, Kaiser human-resources leaders Lionel Sims and Priya Smith described technology and AI as tools to help clinicians manage their practices and improve access and connection with patients. The difference between that stated purpose and workers’ concerns about staffing, triage and time is central to the dispute.
How did the AI dispute fit into contract talks?
Kaiser’s employee update of June 11, 2026, said the parties had held 34 bargaining sessions and still lacked a contract. Kaiser said they had agreed to mediation after NUHW rejected its proposal for a joint AI committee. Its account of bargaining—including its explanation of the impasse—is the employer’s position. In an August 11, 2026, update, Kaiser said mediation was scheduled for August 11–14 and that a proposal for a joint AI committee remained on the table. That update does not give the mediation outcome or establish whether a later contract was reached. The action documents a labor dispute over how AI might affect mental-health care, who controls triage, and how clinical workloads are organized. Workers raised concerns about possible substitution and the transfer of triage tasks; Kaiser disputed that AI was replacing therapists and said it did not then use AI for therapy or let AI make care decisions. The available reporting does not establish that AI had already replaced the striking clinicians, nor does the strike count quantify AI-driven job losses. It is evidence of a disagreement about the direction and governance of care, not a measure of nationwide displacement.
