In the United States, one IVF cycle can cost about $15,000–$25,000, according to the American Society for Reproductive Medicine (ASRM) in 2025. That is a broad estimate—not a guaranteed all-in bill. Your clinic quote depends on what services and medications it includes, your treatment needs, and how your insurance or employer benefit applies. The practical first steps are to get an itemized estimate in writing and verify coverage with your insurer or benefits administrator.

How much does one IVF cycle cost?

ASRM’s 2025 committee opinion puts the cost of a single U.S. IVF cycle at $15,000–$25,000. Treat that as a starting range for planning, not a price every clinic charges or a total that necessarily includes every medication and service. Ask the clinic what its estimate covers and which costs could be billed separately.

The amount you ultimately pay can also depend on insurance coverage, plan limits and eligibility rules. More than one cycle may be needed, which can raise the cumulative cost substantially. Neither the broad range nor a clinic’s initial estimate predicts how many cycles an individual will need.

What to check in a clinic estimate

Request a written, itemized estimate for your proposed treatment plan. Make sure you can distinguish the clinic’s charges from any items that are excluded or billed separately, then ask the clinic to explain anything unclear.

  • Which treatment services are included in the quoted cycle price?
  • Are medications included, or should you budget for them separately?
  • Are screening or other services excluded from the estimate?
  • What charges could change if your treatment plan or clinical circumstances change?
  • If you are considering a multi-cycle package, how many cycles does it cover and what are its eligibility and refund terms?

These questions help make clinic estimates more comparable. Do not assume that similarly named packages include the same services.

Does insurance cover IVF?

There is no single answer for every U.S. patient. ASRM describes state infertility mandates as varied: they can differ in the treatments covered, eligibility, exemptions and limits. A state requirement or an employer benefit therefore does not establish what a particular plan will pay for your care.

Contact the insurer or your employer’s benefits administrator and ask how your exact plan treats IVF and related services. Ask about covered services, annual or lifetime limits, eligibility requirements, required referrals or in-network providers, and your share of costs. Review the benefit documents and request confirmation of important details; do not rely on a general statement that the plan “covers fertility.”

What current federal policy does—and does not—mean

On March 26, 2026, ASRM reported that the HOPE Act had been reintroduced in the 119th Congress. Reintroduction is not enactment, so it does not guarantee coverage. ASRM’s announcement also says ERISA currently has no federal standard for fertility care. Read ASRM’s HOPE Act announcement.

ASRM’s analysis of the October 2025 federal initiative describes select IVF drug-cost measures and voluntary pathways for employers to offer standalone fertility benefits. It did not create a direct federal IVF coverage mandate or direct IVF subsidy. An initiative aimed at voluntary employer benefits does not automatically change an individual’s plan. How to pay for IVF if coverage is limited

Compare insurance, employer benefits and self-pay

Start by comparing your verified plan benefits with the clinic’s written estimate. Ask your employer benefits administrator whether fertility benefits are offered and what eligibility rules apply. If you will pay out of pocket, use the clinic’s itemized estimate to plan around the services it includes and excludes; the available information does not establish that every clinic offers a discount, payment plan or grant.

Evaluate multi-cycle risk-sharing or refund packages

Some programs charge a higher upfront amount for a package of IVF cycles and may refund some or all of the fee if a specified outcome is not achieved. ASRM says screening and medication are ordinarily excluded, and the terms—including the program’s definition of success—should be explained clearly before you enroll. These programs cannot guarantee pregnancy or delivery.

Compare the full package price with per-cycle pricing, not just the advertised refund. A patient who succeeds early may pay more through a package than by paying cycle by cycle. Before committing, check:

  • the total fee and number of cycles covered;
  • what treatment, screening and medication costs are excluded;
  • eligibility requirements and any clinic-specific outcome information provided;
  • the exact outcome that qualifies for a refund, the refund amount and conditions; and
  • how the total cost changes if treatment succeeds early.

ASRM’s Ethics Committee says: “The Committee finds that the financial ‘‘risk-sharing’’ form of payment for IVF treatment is an option that can ethically be offered to patients without health insurance coverage for IVF treatment when certain conditions that protect patient interests are met.” This is an ethical assessment of an option under specified conditions, not a recommendation that every patient enroll. A practical payment-comparison checklist

  1. Get the clinic estimate: Request an itemized written quote for your treatment plan, including clear exclusions.
  2. Verify benefits: Ask your insurer or benefits administrator how your exact plan covers IVF, related services and medications, and what limits or requirements apply.
