The Individual Evaluation Sheet (IES) records a DepEd applicant’s qualifications and individual assessment results. Under DepEd’s 2025 recruitment guidance, signing acknowledges that the assessment process followed applicable guidelines and that the results were discussed with the applicant. It does not prevent the applicant from filing a protest.
What the DepEd Individual Evaluation Sheet is for
The IES is an applicant-specific record used in recruitment and open ranking. It captures qualifications, supporting documents, assessment computations and the applicant’s actual score. DepEd’s 2025 guidance says the signed sheet acknowledges discussion of individual results; it is not a waiver of the right to protest.
In the process described by the 2025 guidance, the HRMO collates qualified applicants’ documents and assigns a unique code to each set. Applicants receive their codes before open ranking, and scores of applicants present are publicly disclosed during the actual evaluation. Open ranking may be conducted remotely when the office considers that practicable and applicable under its internal guidance.
What information and scores can appear on the IES
The 2024 Annex G-2 teacher-position template includes fields for the applicant’s name, application code, position applied for, schools division office, contact number, and job group or Salary Grade level. Its score table provides space to describe qualifications and supporting documents, show the computation, and record the actual score.

2024 Annex G-2 teacher-position scoring
This particular template assigns 100 points across six criteria:

The rubric depends on the position and hiring cycle
Do not assume that the 2024 teacher-position weights apply to every DepEd vacancy. The official 2023 IES template uses a different set of criterion groupings: education, training, experience, performance, outstanding accomplishments, application of education, application of learning and development, and potential. The potential grouping includes a written test, BEI and work sample test. The indexed excerpt shows a 100-point total but does not state individual weights for those categories.

Because the templates differ, use the IES and rubric issued for the specific position and hiring cycle. The 2023 and 2024 forms should not be combined into one universal scoring formula.
What signing the IES means—and what it does not mean
DepEd’s 2025 guidance states: “The duly signed IES shall serve as acknowledgement that the applicant has undergone the application and assessment process based on applicable guidelines and that the results of the individual assessment have been discussed with the concerned applicant.” This is an acknowledgement of process and discussion, not proof that the applicant agrees with every score.

The same guidance says: “The IES shall not, in any case, preclude the applicant from filing a protest relative to their application.” In practical terms, signing does not by itself close off the protest route. The guidance excerpt does not set out the details or deadlines for filing a protest, so consult the applicable recruitment notice or the responsible DepEd office for those requirements.
How the IES is confirmed during open ranking
- Document collation and coding: The HRMO collates the qualified applicants’ documents and assigns a unique code to each set. Applicants are informed of their code before open ranking.
- Score disclosure: During the actual evaluation, scores are publicly disclosed for applicants who are present.
- Discussion and acknowledgement: The individual assessment results are discussed with the applicant, who acknowledges receipt by signing the IES.
- Alternative records: The 2025 guidance says a scanned signed copy may be accepted. If an applicant cannot provide a signed IES, the HRMO’s open-ranking meeting minutes may serve as evidence that the individual results were confirmed.
Personal information and disclosure
The IES may include personal information and any special needs the applicant reports. Under the 2025 guidance, applicants who meet the minimum requirements must proceed to the next recruitment stage regardless of whether they disclose religion, ethnicity or disability, except where disclosure is required by the nature of the position or by law.

Which official form should you use?
The available official materials span a 2023 IES annex, a 2024 Annex G-2 teacher-position form and revised 2025 recruitment guidance. Their criteria and process details are not interchangeable. Follow the latest applicable notice and the position-specific form supplied by the relevant DepEd office; do not rely on a different position’s template to predict your score.
The 2025 guidance also allows remote open ranking when practicable and applicable, subject to internal guidance. The cited materials do not establish one implementation method for every office or hiring cycle.
