How to Choose Where to Retire Early: Taxes, Housing, Healthcare, and Cost of Living

A practical framework for comparing U.S. towns and metro areas before Medicare: model taxes, Marketplace coverage, housing, local prices, healthcare access, and one-time moving costs.

The best place to retire early is the one that works for your actual cash flow, health coverage, housing needs, and support network—not the state with the lowest advertised tax rate or the cheapest cost-of-living ranking. Build a shortlist of specific towns or metro areas, price each one using the same assumptions, and reject any destination whose pre-65 insurance or medical network cannot meet your needs.

That process matters because early retirees face expenses that disappear or change only at certain ages. Before Medicare eligibility, you must arrange health coverage yourself if employer insurance ends. Retirement-account withdrawals can affect both taxes and Marketplace subsidies. A move that looks attractive on paper can also create large selling, moving, setup, and relationship-rebuilding costs.

Start with your household, not a list of “best places”

State rankings are screening tools, not retirement plans. Begin by writing down the facts that determine what a destination must provide:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Your target retirement date and the age when each income source begins.
  • Expected spending for housing, food, transportation, insurance, healthcare, taxes, travel, activities, and gifts.
  • Whether you will rent, buy with cash, finance a purchase, or keep another home.
  • Prescription drugs, specialists, hospitals, mobility requirements, and likely future care needs.
  • How close you need to be to family, friends, airports, cultural activities, outdoor recreation, and everyday services.
  • Your tolerance for climate risks, property-insurance volatility, traffic, isolation, and car dependence.

Use towns or metro areas rather than state names alone. A state can contain expensive coastal neighborhoods, lower-cost inland communities, and radically different hospital networks. For every candidate, identify the ZIP code, dwelling type, neighborhood, and health-plan service area you are actually considering.

Set a comparable annual budget

Make one worksheet for every candidate. Put recurring costs in annual dollars and identify whether each figure is an estimate, a quote, or a national benchmark. Separate fixed expenses from costs that vary with use.

Budget area What to include How to verify it
Taxes Federal income tax, state and local income taxes, property tax, sales tax, and taxes on withdrawals or investment income Apply the rules to your actual income mix and confirm residency or domicile requirements
Housing Rent or mortgage, opportunity cost of cash, property tax, insurance, utilities, maintenance, association fees, and hazard-related costs Use current listings, insurance quotes, tax records, utility estimates, and realistic repair reserves
Healthcare Marketplace premium, deductible, copayments, coinsurance, prescriptions, out-of-pocket maximum, dental and vision costs, and travel to providers Review plans for the candidate ZIP code and check provider and drug directories
Transportation Vehicle ownership, fuel, insurance, maintenance, parking, public transit, and rides to medical care Price the trips you expect to make, not a generic national average
Daily life Food, communications, household services, recreation, memberships, travel, and support services Price your normal basket and activities locally
Contingency Repairs, insurance deductibles, family assistance, uncovered care, and inflation Use a separate reserve rather than hiding surprises inside another category

Run at least two versions of the budget: a low-use healthcare year and a high-use year. Also test a year with a major home repair, a vehicle replacement, or a large taxable withdrawal. A destination that works only in the optimistic case is not financially resilient.

Taxes: compare your tax bill, not a slogan

“No income tax” is not the same as low total tax

A state income-tax headline says little until you apply it to your income. List each source separately: wages or consulting income, pension payments, Social Security, traditional IRA and 401(k) distributions, Roth distributions, dividends, interest, capital gains, rental income, and annuity payments. Then add property and sales taxes. A place with no state income tax can still impose high housing-related or consumption taxes, while a state with an income tax may exempt some retirement income or offer credits. The relevant comparison is your after-tax cash available for spending.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The sources available for this guide do not establish a state-by-state tax calculation or determine anyone’s legal residency. Before changing domicile, read the destination’s residency rules and obtain advice based on your facts. Keep records showing where you live, vote, register vehicles, maintain a mailing address, and spend time if those facts affect a tax determination.

Plan withdrawals around federal rules

Federal tax can dominate a state comparison. IRS Publication 554 (2025) explains that most taxable qualified-plan and nonqualified-annuity distributions taken before age 59½ generally face an additional 10% tax, subject to exceptions. Review the publication and your plan documents before scheduling withdrawals; do not assume that moving states changes this federal rule. Your withdrawal strategy should also account for ordinary income, capital-gain treatment, required distributions later in life, and the tax effect of converting accounts.

Estimate taxes by calendar year, because the amount and timing of a withdrawal can change both your marginal tax bill and your eligibility for health-insurance assistance. Treat a one-time distribution differently from a recurring withdrawal and record whether an amount is taxable, tax-free, or subject to an exception.

IRS Publication 554 (2025) is the primary reference for the additional-tax rule and related retirement-tax guidance. Tax law and thresholds can change, so verify the rules for the year in which you retire.

Pre-65 healthcare is a location-and-budget constraint

Use the Marketplace when employer coverage ends

If you retire before 65 and lose job-based insurance, HealthCare.gov says you can use the Health Insurance Marketplace. Losing employer coverage qualifies for a Special Enrollment Period. Potential premium-tax-credit savings depend on estimated household income and household size. IRA and 401(k) withdrawals generally count as income for the application, so a withdrawal plan that looks tax-efficient can still raise your premiums or reduce assistance.

Retiree coverage and COBRA have different eligibility, duration, and cost rules. Confirm the exact end date of employer coverage, the enrollment window, any retiree plan available to you, and whether COBRA is worth its premium for your expected period. Do not cancel existing coverage until replacement coverage has an effective date.

Use the official HealthCare.gov retiree guidance for enrollment rules and update the income estimate when your withdrawal plan changes. Marketplace prices and plan availability are specific to the plan year and ZIP code.

Compare total exposure, not just the premium

For each candidate ZIP code, collect the premium, deductible, copayments, coinsurance, and annual out-of-pocket maximum for plausible plans. Add prescription costs and routine dental and vision spending. Model what you would pay in both a low-use year and a year involving imaging, surgery, specialist care, or an expensive drug.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Then verify the network. Check that the hospitals, primary-care clinicians, specialists, laboratories, pharmacies, and behavioral-health providers you would use are in-network for the exact plan—not merely located nearby. Confirm referral and prior-authorization rules. Marketplace premiums depend in part on location, age, plan category, tobacco use, and dependents, so another person’s quoted premium is not a reliable proxy.

A low premium is a poor bargain if the nearest suitable hospital is hours away, a needed specialist is out of network, or a prescription is placed on an unaffordable tier. The destination should pass a provider-access test before it passes a tax test.

Price housing as a complete cost, not a listing price

Owners need a full carrying-cost model

For a purchase, include the mortgage payment or the opportunity cost of cash, property tax, homeowners insurance, utilities, routine maintenance, association dues, security, and a reserve for major repairs. Add local hazard exposure—such as wildfire, flood, hurricane, tornado, or severe winter conditions—because deductibles and insurance availability can materially change the budget. If you will retain your current home, include both properties.

For context, the U.S. Census Bureau reported a 2024 median monthly owner cost with a mortgage of $2,035. That measure combines selected expenses rather than representing a destination quote. The Census economist Jacob Fabina describes the affordability measure this way: “One way we measure housing affordability is based on how much households spend on selected costs such as mortgage payments, insurance, taxes, utilities, and various fees.” See the Census Bureau’s 2024 American Community Survey release for the definition and methodology.

Renters should test lease and relocation risk

Renters should compare the full monthly payment, utilities, parking, renter’s insurance, pet or amenity fees, and likely renewal increases. Check lease length, accessibility, elevator reliability, maintenance response, and whether a move to a different building would disrupt medical access. The same Census release reports a 2024 U.S. median gross rent of $1,487, another national benchmark rather than a quote for any town.

Whether you rent or own, price the actual neighborhood and dwelling you would choose. A low countywide median may reflect housing that does not meet your accessibility, safety, transit, or healthcare requirements.

Use regional price indexes for screening, then replace them with local prices

The Bureau of Economic Analysis Regional Price Parities (RPPs) express regional price levels relative to the U.S. level of 100. They cover goods and services, including housing rents, and are useful for spotting broad differences among states and metro areas. They are not a personal consumer basket, a forecast of future inflation, or a substitute for quotes.

2024 all-items RPP State Interpretation
110.7 California About 10.7% above the U.S. price-level benchmark in the BEA measure
110.0 Hawaii About 10.0% above the benchmark
108.8 New Jersey About 8.8% above the benchmark
87.8 Iowa About 12.2% below the benchmark
87.8 Oklahoma About 12.2% below the benchmark
87.0 Mississippi About 13.0% below the benchmark
86.9 Arkansas About 13.1% below the benchmark

Housing can drive the spread. California’s 2024 housing-rent RPP was 154.3, while West Virginia’s was 54.2. Those figures describe aggregate regional price levels, not what your preferred apartment, home, or neighborhood will cost. The BEA’s state and metro RPP page reports 2024 estimates and lists a next release for December 10, 2026; check for an update after that date.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use the BEA Regional Price Parities data to narrow the field, then obtain local rent listings, home prices, utility estimates, insurance quotes, grocery prices, and transportation costs for the remaining ZIP codes.

Compare candidate places on six decision axes

A worksheet that shows only tax rates and home prices hides the trade-offs. Score each candidate on the following axes while keeping the underlying dollar figures visible:

Axis Questions to answer Evidence to keep
After-tax recurring budget What remains after federal, state, local, property, and sales taxes? Year-by-year cash-flow model using your income sources
Pre-65 healthcare What are premium, expected out-of-pocket costs, drug costs, and network distances? Plan documents, provider directories, formularies, and enrollment dates
Housing and tenure risk Can you afford the dwelling after insurance, maintenance, taxes, and renewal or resale risks? Listings, quotes, tax records, lease terms, and repair reserve
Other local prices How much will food, utilities, transport, services, and activities cost? Local quotes and your household’s actual spending basket
One-time transition How long will it take to recover selling, buying, moving, and setup costs? Written estimates and a multi-year break-even calculation
Daily life and support Will the place provide family access, social ties, transportation, climate fit, and needed services? Travel times, service availability, and a trial stay if practical

You can assign weights—for example, giving healthcare and housing more importance than entertainment—but do not let a single composite score conceal a deal-breaking failure. A candidate with no practical specialist access should be eliminated even if its tax score is excellent.

Count the one-time economics of moving

Include the costs incurred before the new budget becomes normal: real-estate commissions, purchase closing costs, loan fees, inspections, repairs, cleaning, movers, storage, travel, temporary lodging, furnishings, utility deposits, and duplicate housing payments. Add the value of time spent managing the move and rebuilding local professional relationships.

Healthcare relationships can have transition costs too. You may need new primary-care and specialist appointments, new records transfers, a different pharmacy, or travel back to your former doctors. Financial, legal, tax, and home-maintenance relationships may also need to be recreated.

Kiplinger’s discussions of low-tax-state moves emphasize including these up-front expenses rather than comparing tax rates alone. See its analysis of whether retiring to a low-tax state is worthwhile and its guide to choosing where to retire. Divide total transition costs by the expected annual savings to estimate a simple break-even period, then test whether you would still stay if the savings were smaller than expected.

A practical shortlisting procedure

  1. Define non-negotiables. Set maximum acceptable travel time to a hospital, minimum provider availability, housing type, climate limits, and a minimum cash reserve.
  2. Select three to six specific areas. Use metro areas or towns and identify the ZIP code, neighborhood, and likely housing arrangement.
  3. Build the income calendar. Record when pensions, Social Security, investment income, and each planned retirement-account withdrawal begin.
  4. Price healthcare first. Check Marketplace plans, Special Enrollment Period timing, deductibles, out-of-pocket maximums, prescriptions, doctors, hospitals, and referral rules.
  5. Collect housing evidence. Save several current listings or rent quotes and add taxes, insurance, utilities, maintenance, fees, and hazard exposure.
  6. Calculate taxes. Apply federal rules and the destination’s state and local rules to your actual income mix; verify domicile issues before moving.
  7. Replace indexes with local prices. Use RPPs only as a screen, then price groceries, transportation, services, and activities you actually use.
  8. Add transition costs. Obtain estimates for sale, purchase, moving, repairs, temporary lodging, furnishing, and relationship rebuilding.
  9. Run stress cases. Test high healthcare use, a large withdrawal, a major repair, insurance increases, and a year with less investment income.
  10. Visit and validate. If feasible, stay in the neighborhood during an ordinary season, test the commute to care, and verify that daily errands work without relying on ideal traffic or weather.

Common mistakes that produce a false bargain

  • Choosing by state income-tax rate alone: it ignores federal tax, property tax, sales tax, and the way your withdrawals are taxed.
  • Using a national cost-of-living rank as a personal budget: averages may not represent your housing standard, transportation needs, or healthcare use.
  • Comparing premiums without networks: a cheap plan can be unusable if your doctors, hospital, or drugs are not covered on acceptable terms.
  • Ignoring withdrawal effects on subsidies: a large IRA or 401(k) distribution can change Marketplace income and premium assistance.
  • Budgeting only the purchase price: insurance, taxes, utilities, maintenance, fees, and hazard exposure can dominate the monthly cost.
  • Assuming a move is immediately profitable: commissions, closing costs, movers, repairs, temporary housing, and new professional relationships may take years to recover.
  • Waiting to research enrollment: losing employer coverage creates a time-limited Special Enrollment Period; coordinate the end and start dates.
  • Failing to refresh volatile data: tax rules, Marketplace premiums, plan networks, rents, home prices, insurance, and moving quotes change. Recheck them for the relevant plan year and location.

Make the decision conditional and revisitable

Choose the destination that meets your medical-access and housing requirements while leaving a durable after-tax surplus under conservative assumptions. Document the assumptions behind every major figure, the date each quote was obtained, and which costs are one-time or recurring. Re-run the worksheet when your retirement date, withdrawal amount, health needs, household size, or insurance year changes.

There is no universal best state or town for early retirement. A lower-price location may win for one household, while proximity to family, a particular hospital system, or a manageable rental market may be worth more than a tax saving for another. The defensible choice is the place whose complete, verified budget still works when healthcare use, taxes, housing costs, and transition expenses are less favorable than hoped.
