There is no single long-term care insurance benefit that fits everyone. Estimate the kind of care you would want, its current cost where you live, and how much you could pay from income or assets; then choose policy limits and features to cover the portion of risk you do not want to carry yourself. Long-term care insurance can help pay for personal and custodial support at home or in facilities, but the contract determines which services, providers, and circumstances qualify.
What long-term care insurance covers
Long-term care is ongoing assistance for someone with chronic illness, disability, or cognitive impairment who needs help with everyday activities or supervision. It is broader than nursing-home care: support may be provided at home, in an adult day health program, assisted living, or a nursing home. Depending on the contract, covered services may include personal care, home health care, respite or hospice care, adult day care, home-delivered meals, and transportation. The Administration for Community Living (ACL) describes the range of services and policy features in its long-term care insurance overview; the National Association of Insurance Commissioners (NAIC) also emphasizes that coverage varies by policy in its consumer guide.

Long-term care insurance is designed primarily for personal and custodial support, rather than medical insurance’s central role in diagnosing and treating illness. A policy can restrict coverage to particular services, care settings, or provider qualifications, and may contain exclusions or pre-existing-condition provisions. Read the policy’s benefit schedule and definitions rather than assuming that every service commonly associated with long-term care is included.

Medicare and Medicaid are different
Medicare generally does not pay for ongoing custodial long-term care, whether in a nursing home or community setting. Its separate, limited skilled nursing benefit is not the same as long-term custodial care. Medicaid may cover long-term services for people who meet their state’s eligibility rules, so eligibility and coverage depend on where the person lives. See the ACL long-term care information for an overview of these distinctions.

How to estimate how much coverage you need
Start with the gap between the care you might want and the resources you are willing and able to use—not with a generic national benefit figure. NAIC’s consumer page puts the central cost issue simply: “The cost depends on the amount and type of care you need and where you get it.” Use current local or state-specific information from providers or other reliable sources to estimate costs. The official materials cited here do not provide a current, nationally representative local cost table that can produce a personalized target; NAIC’s national averages are from 2017 and are too old to treat as current estimates.
- Choose a plausible care setting. Consider whether you would prefer care at home, adult day services, assisted living, or a nursing home. These settings have different costs, and prices vary by location.
- Estimate the amount you would need to fund. Compare expected costs with income and assets you would be prepared to spend on care. Suitability depends in part on age, health, retirement goals, income, and assets, according to NAIC’s Long-Term Care Insurance Shopper’s Guide.
- Set a daily or monthly benefit limit. This is the policy’s maximum payment for each covered day or month, subject to its rules. ACL notes that many policies pay a selected daily amount up to a limit. A limit below expected costs leaves a portion for you to fund.
- Choose the total benefit pool or duration. A useful shorthand is the daily maximum multiplied by the number of covered days, but actual contract accounting can differ. Some policies impose a time limit and some a lifetime maximum; check the contract rather than relying on the shorthand alone.
- Decide how much future cost growth to insure. A benefit that stays level may pay a smaller share of care costs later. Inflation protection can increase benefits over time, usually in exchange for a higher premium.
- Account for the elimination period. This is the time you must pay eligible care costs yourself after meeting the benefit trigger and before the insurer begins paying. The duration and counting method affect how much cash you need available at the start of a claim.
These choices work together. A higher daily limit does not necessarily mean a longer period of protection, and a longer benefit period does not ensure that the daily limit will keep pace with future costs. Compare both the maximum payment and the maximum pool, then test them against a realistic local cost estimate and the amount you expect to self-fund.
For example, if your estimate of care costs exceeds a policy’s daily maximum, the difference does not disappear: you would need to cover it from other resources, subject to the policy’s payment method and terms. Likewise, a large maximum pool may still run down faster than expected if care is expensive or lasts longer than the selected benefit duration. This is why the benefit limit, duration, inflation feature, and your intended out-of-pocket share should be considered together, rather than choosing a policy based on one headline number.
What the benefit limits and waiting period mean in practice
Policies commonly organize coverage around an eligibility trigger, an elimination period, and a payment method. A policyholder must first satisfy the contract’s trigger. The waiting period then determines when payment can begin, and the benefit limit and payment method determine how much the insurer pays once benefits are available.

Benefit trigger
ACL says most policies use an assessment that often considers whether the person needs help with two or more of six activities of daily living (ADLs), or has cognitive impairment. ADLs generally refer to basic personal activities such as bathing, dressing, eating, and using the bathroom. The exact trigger, assessment process, certification requirements, and any care-plan rules are contract matters.
Tax-qualified contract rules are related but should not be assumed to match every policy’s benefit trigger. IRS instructions for tax year 2025 describe qualified chronic illness criteria that include inability, due to functional loss, to perform at least two ADLs without substantial help for at least 90 days, or a need for substantial supervision because of severe cognitive impairment; a licensed health care practitioner must certify the condition. See the IRS Form 1099-LTC instructions for the tax definition.
Elimination period
The elimination period is the interval between qualifying for benefits and the start of policy payments. ACL describes 30-, 60-, and 90-day choices as common; NAIC’s 2022 shopper guide gives examples ranging from 20 to 100 days, including 20, 30, 60, 90, or 100 days. These are examples, not a promise that every insurer offers each choice. During the waiting period, you pay for care that would otherwise be eligible.

It may count calendar days or only days on which covered paid services are received. Also ask whether the period must be satisfied again after a later episode of care. Two policies with the same stated number of days can expose you to different costs if their counting rules differ.

Reimbursement or cash benefit
A reimbursement policy pays eligible expenses up to its daily or monthly limit. A cash-benefit policy may instead pay a fixed amount for each day the benefit trigger is met, regardless of which services are purchased. Cash benefits can offer more flexibility, but may cost more. Compare how each structure fits the care you expect to use and the provider requirements in the contract.
Inflation protection and future buying power
Inflation protection is an option that increases the policy benefit over time so it may better match rising care costs. It generally raises premiums, so compare the added cost with the risk of a level benefit covering less in the future. A useful illustration—not a current price or forecast—is in NAIC’s 2022 guide: a nursing home costing $150 per day in 2018 would cost $398 per day in 20 years under an assumed 5% annual inflation rate. The example shows how compounding can affect a fixed daily limit; it does not establish what care will cost in a particular location.
What determines premiums and whether a policy stays affordable
Premiums vary with age at purchase, health underwriting, policy type, selected benefit amount and duration, and optional features such as inflation increases. Most individual policies require medical underwriting; people already receiving care or in poor health may not qualify, or may face limited or higher-priced coverage. A quote is specific to the applicant, insurer, coverage choices, and applicable rules, so historical averages cannot substitute for an actual offer.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minutePremiums are not guaranteed to stay unchanged under every policy. ACL notes that insurers may raise premiums when pricing assumptions prove wrong. Before buying, ask about the insurer’s rate-increase history and what changes, if any, could be available if the premium becomes unaffordable. NAIC discusses reduced-benefit options in connection with rate increases, but available choices depend on the contract and state.

For historical context only, ACL reported that in 2007 an average policy cost about $2,207 per year, provided a $160 daily benefit, and covered 4.8 years for buyers excluding the 20% who elected lifetime coverage. The reported average policy covered facilities and home care and included some form of automatic inflation protection. Those 2007 figures are not current premiums, costs, or recommended benefit levels.

Compare multiple companies and agents using the same questions. Policies with similar benefits can have different prices, and NAIC advises reviewing both what a policy does and does not cover.
- What are the daily or monthly benefit and total benefit maximum?
- How long can benefits last, and is a lifetime option available?
- Which settings and services are covered: home care, adult day care, assisted living, nursing home, respite, or hospice?
- Does the policy reimburse eligible expenses or pay a cash benefit?
- What exactly triggers benefits, who makes the assessment, and are there plan-of-care requirements?
- How long is the elimination period, how are days counted, and does it apply again after a later care episode?
- What type of inflation protection is offered, how does it change the benefit, and what does it add to the premium?
- Which provider qualifications, exclusions, and pre-existing-condition rules apply?
- What is the current premium and payment structure? What is the insurer’s rate-increase history, and what options apply if rates rise?
- What underwriting is required, what is the insurer’s financial standing, and which state-specific rules apply?
Be cautious of advertising that falsely suggests Medicare sells or endorses long-term care policies, a warning included in the NAIC shopper guide. Verify the insurer, policy form, and rules that apply in your state before making a decision.
Frequently Asked Questions
How much long-term care insurance do I need?
There is no universal benefit amount. Estimate the setting and current local cost of care you would want, subtract the income and assets you are willing and able to spend, then compare the remaining gap with a policy’s daily or monthly maximum, total benefit pool or duration, elimination period, and inflation protection. Use current local information rather than outdated national averages.

What does long-term care insurance cover?
Depending on the contract, it may cover personal and custodial support at home, adult day health care, assisted living, or a nursing home. Services may include help with daily activities, home health and personal care, respite or hospice care, meals, and transportation. Check covered settings, services, providers, and exclusions in the policy.
Does Medicare cover long-term care?
Medicare generally does not pay for ongoing custodial long-term care. Its separate short-term skilled nursing benefit is not the same as long-term custodial care. Medicaid may cover long-term services for people who meet eligibility rules in their state.
How do long-term care insurance benefits start?
You must first meet the policy’s benefit trigger, which often involves needing help with two or more activities of daily living or having cognitive impairment. After that, the elimination period must be satisfied before payments begin. The contract sets the assessment, certification, counting rules, and payment terms.

Some premiums may rise if an insurer’s pricing assumptions prove wrong. Ask about the company’s rate-increase history and any reduced-benefit or other options available if a premium becomes unaffordable; choices depend on the policy and state.
The Bottom Line
Choose coverage by matching your preferred care setting and current local costs with the resources you are prepared to spend, then compare the policy’s benefit maximum, duration, trigger, elimination period, payment method, and inflation protection. Review the contract and premium-increase history carefully: a headline benefit amount alone does not show how much protection you will have or what you may need to pay yourself.
