How Medicare Part B Premiums Are Deducted From Social Security Benefits

Medicare Part B premiums are usually deducted from Social Security benefits. Learn when CMS bills instead, how 2026 IRMAA affects the amount, and how to check a notice or request a review.

If you receive monthly Social Security benefits, your Medicare Part B premium is usually deducted automatically from your benefit. If you do not receive Social Security, the Centers for Medicare & Medicaid Services (CMS) generally bills you quarterly. The amount can be higher than the standard premium if you owe an income-related monthly adjustment amount, or IRMAA.

How the deduction works

Social Security Administration (SSA) says that Medicare premiums are usually deducted automatically for people receiving monthly Social Security benefits. The deduction comes out of the monthly benefit payment; it is a payment method, not a separate charge added after the benefit reaches your bank account.

The automatic deduction does not mean every Medicare-related premium shown in an account is necessarily being taken from Social Security. A beneficiary’s SSA account may display Part B together with Part C or Part D premiums. But SSA’s account guidance says that the display does not include premiums paid by someone else or premiums the beneficiary pays by check or automatic bank-account deduction. The displayed total and the amount withheld from a particular benefit may therefore differ.

Payment arrangements also depend on whether Social Security is being paid. Medicare enrollment and Social Security benefit receipt are related for many people, but they are not the same thing: a person can owe a Medicare premium without receiving a monthly Social Security payment from which it can be withheld.

Which payment arrangement applies to you?

Situation How the premium is generally collected What to check
You receive monthly Social Security benefits SSA usually deducts Medicare premiums from the monthly benefit. Check your SSA account or benefit notice for the amount withheld and which premiums it includes.
You do not receive Social Security benefits CMS mails a quarterly bill with payment instructions, according to SSA’s FAQ dated April 16, 2025. Follow the instructions and due date on your own CMS bill; the FAQ does not give a universal due date or list every payment channel.
Your Social Security benefit is too small to cover the Part B deduction SSA policy says SSA withholds as much as possible and bills you for the remaining Part B balance. Use your current SSA notice or CMS bill to confirm the amount left to pay.
Your benefit is insufficient to cover IRMAA The federal regulation provides that CMS bills the full IRMAA amount owed if the benefit cannot cover it. Check the notice or bill for the amount due and payment instructions.

The last two rows describe related but distinct rules. SSA policy addresses an insufficient Part B deduction: SSA withholds what it can and bills for the remaining balance. The regulation specifically says that when a benefit is insufficient to pay the full IRMAA, CMS bills the full IRMAA amount owed. Do not assume that a partial withholding settles the rest of either amount; use the notice or bill to see what remains payable.

How much Part B costs in 2026

The standard Medicare Part B premium for 2026 is $202.90 per month, according to SSA. That is the standard premium, not a personalized estimate for every enrollee. Some beneficiaries pay more because they owe IRMAA in addition to the standard premium.

IRMAA is an income-related adjustment to Medicare premiums. SSA determines it using modified adjusted gross income (MAGI), which it defines for this purpose as adjusted gross income plus tax-exempt interest. The amount depends on both the applicable income band and filing status. The 2026 SSA table, for example, lists a Part B premium of $284.10 per month for an individual with MAGI above $109,000 and through $137,000. For an individual above $205,000 and below $500,000, it lists $649.20 per month. These are examples from the 2026 table, not predictions of any one person’s bill.

For 2026, SSA policy shows IRMAA thresholds of $109,000 for filing statuses other than married filing jointly and $218,000 for married filing jointly. These figures identify threshold levels; the premium owed depends on the full table, the person’s filing status and MAGI, and SSA’s determination. Use the applicable SSA table and your own notice rather than extrapolating from an example band.

Why a current premium can reflect income from two years ago

SSA generally uses the most recent tax information available from the Internal Revenue Service, usually a return from two years earlier. Under SSA’s policy for 2026 premiums, the usual tax-year basis is 2024. If 2024 information is unavailable, SSA may use 2023 information in the circumstances described in that policy.

This timing can make an IRMAA amount seem out of step with current finances. A recent retirement, loss of income, or other change does not by itself mean the premium will immediately be recalculated using this year’s income. The amount is based on the tax information SSA uses for its determination unless the person requests a review and qualifies for a change.

For example, someone whose income has fallen since the tax year SSA used should first compare the year and income figures in the SSA determination with their current circumstances. The important question is not simply whether income is lower now; it is whether a qualifying life-changing event and supporting information allow SSA to adjust the IRMAA determination.

Requesting an IRMAA review after a qualifying life-changing event

If the tax information used for IRMAA no longer reflects your circumstances because of a qualifying life-changing event, you may ask SSA to reduce the adjustment using Form SSA-44. A reduction is not automatic: SSA evaluates the request and supporting information.

  1. Review the IRMAA determination. Identify the tax year and income information SSA used, as well as the premium amount in your notice.
  2. Check whether your situation is a qualifying life-changing event. Form SSA-44 lists examples and explains the documentation requirements. Do not assume that every income decrease qualifies.
  3. Complete Form SSA-44 with the requested information. The form is the route SSA identifies for requesting a lower IRMAA based on a qualifying event.
  4. Provide the supporting documentation the form requires. SSA evaluates the evidence; submitting a request does not itself change the amount.
  5. Follow the determination you receive. Use the updated notice, if SSA grants a change, to verify the new amount and when it applies. Do not infer a start month without that notice.

Form SSA-44 and SSA’s instructions are the controlling sources for which events qualify and what evidence is needed. If your circumstances do not fit the form’s criteria, a current income drop alone is not enough to conclude that SSA will reduce the adjustment.

What to do if no Social Security payment is available

If you are not receiving Social Security benefits, SSA says CMS will mail you a quarterly bill with instructions for paying the premium. Use the bill’s own due date and payment instructions. SSA’s FAQ, dated April 16, 2025, confirms quarterly billing but does not establish a single due date or enumerate all payment methods on the page reviewed, so do not rely on a generic date or payment option in place of your bill.

The federal regulation also addresses people who are not receiving Social Security, Railroad Retirement Board, or Office of Personnel Management benefits: CMS bills them for IRMAA. This is consistent with the broader point that an amount that cannot be collected from a Social Security benefit may instead be billed. The relevant notice or CMS bill tells you what amount is due and how to pay it.

How to check why your Social Security payment changed

If a benefit payment is lower than you expected, check the current records before attributing the difference to Part B. The amount withheld can change when the premium changes, when IRMAA applies, or when the SSA collection arrangement for a balance affects the payment. The available general rules do not establish an individual’s deduction, benefit amount, or the month a change began.

  • Check the benefit notice or SSA account. Look for the monthly amount and the premiums identified as withheld. The account display may include Part C or Part D as well as Part B.
  • Distinguish a displayed premium from a deduction. SSA’s account help says the display excludes premiums paid by someone else and premiums paid by the beneficiary by check or automatic bank-account deduction.
  • Compare the determination with the 2026 figures only if they apply to your period. The standard Part B premium is $202.90 per month in 2026, but a person’s IRMAA, enrollment details, and current notice determine the individual amount.
  • Look for a separate bill or balance notice. If a benefit could not cover the full Part B deduction or IRMAA, an amount may remain to be paid through billing rather than withholding.
  • Use the agency named on the notice. SSA handles the benefit and IRMAA determination; CMS sends quarterly bills to people who are not receiving Social Security. Follow the contact and payment instructions on the document that concerns the amount in question.

Part B should also be kept distinct from Part C and Part D. A combined figure in an SSA account does not, by itself, establish that the entire figure is the Part B deduction from Social Security. Check the itemized entries and any separate payment records to identify which program premium is being paid through which channel.

Practical decision path

  1. Are you receiving monthly Social Security? If yes, premiums are usually withheld automatically. If no, look for the quarterly CMS bill.
  2. Is the amount the standard premium or does IRMAA apply? Compare the notice with SSA’s relevant-year table and the tax information cited in the determination. For 2026, the standard premium is $202.90 monthly; IRMAA can raise it.
  3. Does the tax year used still reflect your circumstances? SSA usually uses tax information from two years earlier. If a qualifying life-changing event makes that information unrepresentative, review Form SSA-44.
  4. Was the benefit too small to cover the amount? SSA policy says it withholds what it can for an insufficient Part B deduction and bills the balance; the regulation says CMS bills the full IRMAA owed if the benefit cannot cover IRMAA.
  5. What is the amount and deadline for you? Confirm those details in your own SSA notice, SSA account, or CMS bill. General premium rules do not reveal an individual’s balance or payment due date.
