- Date and version: when the review occurred and whether the deployed code has changed since.
- Finding details: what the major finding was, what “mitigated” means in the report, and what evidence supports that status.
- Additional safeguards: whether formal verification, ongoing monitoring, or a bug bounty supplements the code review.
