Can You Use an FSA, HRA, or HSA to Pay for a Fitness Tracker?

IRS guidance makes wearable eligibility depend on the device, its specific disease-related purpose, and the circumstances of its purchase and use.

Sometimes, but not for general fitness tracking alone. Under current IRS guidance, a fitness tracker or other wearable may qualify for FSA, HRA, or HSA payment or reimbursement when you purchase and use it primarily to assist with the diagnosis, cure, mitigation, treatment, or prevention of a specific disease. Eligibility depends on the device’s functions and all the circumstances surrounding its purchase and use; no wearable is automatically eligible.

When can a fitness tracker be a qualified medical expense?

The IRS applies the medical-expense framework under Internal Revenue Code section 213 to wearables such as bands, watches, rings, patches, and sensors. The key question is whether you bought and use the device primarily for a medical purpose related to a specific disease—not simply whether it measures something about your health.

In its wearable-specific FAQ, added September 23, 2026, the IRS says: “A generalized desire to monitor or improve health is not sufficient.” A tracker used for routine step goals, general fitness, sleep insights, or an overall interest in wellness therefore is not established as eligible merely because it records health-related information. The IRS’s broader explanation of medical expenses likewise distinguishes care for disease from expenses that are merely beneficial to general health; see the IRS FAQ on nutrition, wellness, and general health.

What circumstances may support eligibility?

The IRS says the answer depends on all facts and circumstances, including what the device does and why it was purchased and used. Factors that may be relevant include:

  • Whether a medical practitioner recommended the wearable.
  • Whether you have symptoms, a diagnosis, abnormal test results, or risk factors relevant to a specific disease.
  • Whether the measurements the device tracks relate to that disease or to its evaluation or management.
  • How you or your practitioner review and use the information—for example, whether it informs diagnosis, treatment, or disease prevention.

These are relevant circumstances, not a checklist that guarantees approval. A practitioner’s recommendation alone does not automatically make a device eligible, and a particular measurement or feature does not establish eligibility by itself.

Do accessories, apps, or subscriptions qualify too?

Not automatically. Accessories, add-ons, and software subscriptions qualify only when they are purchased primarily to serve the same medical purpose. A general-purpose accessory or subscription does not become a medical expense just because it is paired with a wearable that may have a qualifying use.

What to check before paying or filing a claim

  1. Identify the specific medical purpose. Be prepared to explain the disease-related purpose for buying and using the device, rather than relying on a general goal to be healthier.
  2. Connect the device’s functions to that purpose. Note which measurements or features are relevant and how you or your practitioner will use the information.
  3. Ask your plan administrator about its process. Confirm how the plan handles the item, what documentation it requests, and whether the purchase can be made with a benefit card or must be submitted for reimbursement.
  4. Keep supporting records. Retain relevant documentation of the medical purpose, recommendation, purchase, and use. The IRS guidance does not certify a particular model or promise that a claim will be approved.

The IRS explains that qualified expenses are governed by the rules and arrangements applicable to each account. Its Publication 502 provides general background on medical expenses but expressly distinguishes itemized-deduction rules from the rules for determining FSA reimbursement; consult IRS Publication 502 and IRS Publication 969 for that background.
