The FDA approved it on April 1, 2026, for long-term weight management in certain adults, alongside a reduced-calorie diet and increased physical activity. In the 72-week ATTAIN-1 trial, participants taking the highest studied dose lost an average of 11.2% of their body weight under the treatment-regimen analysis; that is a group average, not a prediction for any one person.
What is Eli Lilly’s oral GLP-1 drug?
Orforglipron is the drug’s generic name; Foundayo is its U.S. brand name. It is a prescription medicine, not an over-the-counter weight-loss product. The FDA indication is to reduce excess body weight and maintain weight reduction long term in adults with obesity, or adults with overweight and at least one weight-related comorbidity, in combination with a reduced-calorie diet and increased physical activity. It is not labeled for cosmetic weight loss or for everyone who wants to lose weight. See the FDA prescribing information and FDA approval announcement.
How much weight did people lose in the ATTAIN-1 trial?
ATTAIN-1 was a Phase 3, randomized, double-blind, placebo-controlled, 72-week study of 3,127 adults with obesity or overweight plus a weight-related condition, without diabetes. The peer-reviewed report presents two ways of analyzing results. The treatment-regimen estimand accounts for treatment discontinuation and rescue treatment; the efficacy estimand estimates outcomes as if participants stayed on assigned treatment and did not receive rescue treatment. These estimates answer different questions and should not be combined.

All figures are mean changes reported in the New England Journal of Medicine ATTAIN-1 report. The 36 mg group’s 11.2% result uses the treatment-regimen estimand; 12.4% is the separate efficacy-estimand result. Lilly also reported a mean absolute loss of 27.3 pounds at the highest dose under the efficacy estimand, based on the trial’s average starting weight. That pound figure is not a universal outcome or an individual forecast; percentages are more useful for interpreting results across different starting weights. How Foundayo is taken
The FDA label directs patients to take Foundayo once daily, with or without food, and swallow the tablet whole. Treatment starts at 0.8 mg daily; the dose can be increased at intervals of at least 30 days according to the label, up to a maximum of 17.2 mg daily. The label does not recommend using it with another GLP-1 receptor agonist. Follow the prescriber’s instructions and the current label rather than changing a dose independently.
What are Foundayo’s risks and warnings?
The FDA label carries a boxed warning about thyroid C-cell tumors observed with certain GLP-1 medicines in rodents; it says the relevance to humans has not been determined. Foundayo is contraindicated for people with a personal or family history of medullary thyroid carcinoma or with multiple endocrine neoplasia syndrome type 2 (MEN 2).

Other label warnings include pancreatitis; severe gastrointestinal reactions, with use not recommended in severe gastroparesis; kidney injury related to volume depletion; low blood sugar risk when used with insulin or insulin secretagogues; serious hypersensitivity reactions; diabetic retinopathy complications in people with type 2 diabetes; gallbladder disease; and aspiration risk during general anesthesia or deep sedation. Common adverse reactions listed on the label include nausea, constipation, diarrhea, vomiting, indigestion (dyspepsia), abdominal pain, headache, abdominal distension, fatigue, belching, reflux, flatulence and hair loss.

What the trial does—and does not—show
ATTAIN-1 provides evidence about average results over 72 weeks in adults who met its eligibility criteria and did not have diabetes. It does not establish what an individual will lose, how they will respond after longer treatment, or whether orforglipron is better than another weight-loss medicine. Results from separate trials cannot establish direct superiority or inferiority because populations, study duration, analysis methods and other design details may differ. A fair comparison with another oral GLP-1 would also need to account for its approved indication, dosing instructions, adverse events and availability in the reader’s country.
