Not exactly. A 2025 analysis modeled what might happen if community water fluoridation were eliminated across the United States: it estimated 25.4 million additional decayed teeth and $9.8 billion in added costs over five years. Those are uncertain projections—not 25 million children with ruined teeth, an observed bill, or proof that a nationwide federal ban has taken effect.
What the 25.4 million and $9.8 billion figures mean
The figures come from a 2025 study that used data on 8,484 children in the 2013–2016 National Health and Nutrition Examination Survey to model a five-year scenario without community water fluoridation. The analysis estimated a 7.5 percentage-point increase in caries prevalence and 25.4 million additional decayed teeth. Its 95% uncertainty interval for additional decayed teeth was 23.3 million to 27.6 million.
The $9.8 billion figure is the study authors’ modeled estimate of costs over that same five-year period. The reported 95% uncertainty interval was $8.7 billion to $10.8 billion. It is not a measured national loss or a guaranteed forecast; it depends on the study’s scenario and modeling. See the study abstract.
“25 million” refers to teeth, not children. The estimate does not mean every child would develop cavities, nor that all affected teeth would be severely damaged. The study also estimated a change in caries prevalence, but that is a percentage-point change—not a statement that 7.5% of all children would have ruined teeth.

Does this mean RFK Jr. or the FDA has banned fluoride in tap water?
No nationwide federal ban on community water fluoridation is established by the federal actions described here. The FDA’s actions concern certain ingestible prescription fluoride products for children, which are a different exposure and policy question from adding fluoride to public water.
FDA guidance recommends limiting ingestible fluoride drug products to children aged three or older who are at high risk for tooth decay. The agency says it intends compliance action against certain unapproved products labeled for children under three or for older children without a high-risk-use restriction. HHS announced on October 31, 2025, that FDA had sent notices to four companies. These actions do not themselves prohibit fluoridation of public water. FDA’s announcement and guidance and HHS’s announcement address the drug-product actions.

Water-fluoridation decisions also involve state and local policy. A Congressional Research Service report describes the distinct roles of federal rules and recommendations and notes state and local actions; its publication-date snapshot should not be treated as a real-time inventory of every later decision. How the estimate fits the evidence on preventing tooth decay
CDC defines community water fluoridation as controlled adjustment of fluoride in public water for dental-caries prevention. Its scientific statement says frequent, low-level fluoride contact reduces decay. CDC cites about a 25% reduction in tooth decay in children and adults and reports an average of 2.25 fewer decayed teeth among schoolchildren in fluoridated communities than among similar children in non-fluoridated communities. These are evidence summaries, not the same thing as the 2025 study’s modeled five-year national counterfactual. CDC’s scientific statement.

Fluoridated water is a population-wide measure: CDC says it reaches people through community water systems regardless of age, education, or income. Its 2024 fact sheet reports that 72.3% of the U.S. population served by community water systems—more than 209 million people—had fluoridated water in 2022. That figure describes people served by those systems, not all U.S. residents. CDC fluoridation facts and statistics.
Toothpaste and other topical fluoride products, swallowed prescription products, and fluoride in drinking water are not interchangeable interventions. A topical product can help prevent cavities, but that does not establish that it fully replaces the population-level effect attributed to fluoridated water. Prescription products involve swallowing fluoride and are addressed by FDA’s separate guidance.

What is known about risks—and what should not be conflated
CDC identifies dental fluorosis as the documented risk associated with community water fluoridation and says most fluorosis in the United States is mild. Risk and exposure depend on context: recommended community concentrations, fluoride from other sources, and excessive or swallowed exposures are not the same situation.
Evidence about dental outcomes should not be treated as interchangeable with evidence about other proposed health effects. The Congressional Research Service describes evidence on dental effects as generally accepted while noting that evidence for some other outcomes, including neurodevelopment, is less established and needs further study. That distinction does not settle every policy debate, but it prevents claims about one exposure or outcome from being applied automatically to another.

What the cost figure means for household and public budgets
The study’s $9.8 billion estimate describes modeled costs under a hypothetical nationwide removal scenario over five years. It is not a household bill, a government appropriation, or the price of implementing an enacted ban. Actual costs to families and public programs would depend on factors not expressed by that single national estimate, including how many people developed decay and what treatment they received.
There is a separate economic frame: CDC’s 2024 fact sheet summarizes a cited cost analysis estimating an average return of $20 saved per $1 invested in fluoridation for communities of 1,000 or more. That is an average estimate for the stated community-size group, not a guarantee for every locality. It should not be directly subtracted from the study’s $9.8 billion projection: the figures come from different analyses and measure different scenarios. CDC’s fact sheet.
