Your hospital stay, recovery, and follow-up schedule depend on your treatment, the center’s protocol, and your health. Your care team can give you the plan and urgent-contact instructions that apply to you.
What happens during the CAR-T treatment journey?
CAR-T cells are made from a patient’s own T cells. The National Cancer Institute describes a process in which blood cells are collected, T cells are modified in a laboratory to add chimeric antigen receptors, and the finished cells are expanded and returned to the hospital for infusion. NCI estimates that cell production takes about 3 to 5 weeks from collection to infusion. That is the manufacturing interval—not a timetable for the full evaluation, any bridging treatment, hospital stay, or recovery.
The infusion is followed by close observation because important side effects can emerge after treatment. Monitoring may take place in hospital or through an outpatient pathway; the treatment center determines which approach is appropriate and explains what to expect if symptoms develop.
How long will I stay in the hospital after CAR-T?
There is no single hospital-stay length that applies to everyone. The product, the patient’s condition, the center’s protocol, and any complications can all affect the plan. Some patients are monitored as inpatients, while some centers use outpatient or ambulatory monitoring for suitable patients.

Published center examples illustrate the range, not a general average: University College London Hospitals describes one pathway with at least 14 inpatient days and a possible transition to ambulatory care or home; Memorial Sloan Kettering describes its adult inpatient example as 7 to 10 days or longer. A complication can extend a stay, and a clinical-trial pathway may differ from routine-care instructions.
When your team explains the proposed pathway, ask where monitoring will happen, how often you will need to return to the center, how close you must remain, what caregiver support is required, and what the team wants you to do if symptoms appear. These details—not a stay length from another hospital—are what make the schedule meaningful for you.

Why is monitoring important after the infusion?
Two acute risks the care team watches for are cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS). CRS can cause fever and flu-like symptoms and, in serious cases, low blood pressure or breathing difficulty. Neurologic effects can include confusion or difficulty speaking. The National Cancer Institute notes that side effects vary, and the team cannot know in advance exactly when they will occur or how severe they will be.
In a 2025 communication, the U.S. Food and Drug Administration described updated labeling for six listed autologous CAR-T therapies that calls for at least two weeks of monitoring, including daily monitoring for at least one week; staying within proximity of a healthcare facility for at least two weeks; and avoiding driving for two weeks after administration. These are U.S. labeling instructions for the named products, not a universal rule for every CAR-T therapy worldwide. Ask your team which current product-specific instructions apply to you.

Follow your treatment center’s urgent-contact instructions for a new fever, confusion, difficulty speaking, trouble breathing, or any other symptom it has identified. Do not wait to see whether a concerning symptom passes or substitute another center’s thresholds for your own instructions. The NIH Clinical Center’s patient education sheet also advises contacting the care team or seeking emergency care for CRS signs after discharge, but it was written for people in that center’s clinical research program and may not apply to other patients.
What should caregivers and patients plan for?
A caregiver may help with transportation to appointments, notice changes in symptoms or behavior, and stay with the patient after discharge. Dana-Farber’s guidance describes caregiver support during early recovery, which may extend up to 30 days depending on the patient’s situation. Whether a caregiver is required, and for how long, depends on the treating center’s instructions and the patient’s needs.

If you live far from the treatment center, ask early whether you will need temporary accommodation nearby. Dana-Farber advises patients to plan for short-term lodging when distance makes frequent visits difficult. The FDA’s 2025 U.S. labeling communication includes a proximity instruction for the six therapies it addresses; a center may set more specific requirements.
For practical and financial planning, ask the center about expected appointment frequency, transportation options, distance rules, available lodging resources, and financial or insurance support. These arrangements are center- and patient-specific, so confirm them with the team rather than assuming that another patient’s plan—or a general estimate—will fit yours.

What is recovery like after discharge?
Discharge does not necessarily mean treatment-related monitoring is over. Dana-Farber describes the typical recovery period as the 30 days after infusion, while noting that some patients need frequent visits or additional hospitalization. Fatigue and reduced appetite can continue after discharge, and caregiver needs may last longer depending on recovery.
There is no fixed recovery finish line. Your pace and any extra care depend on how you are doing and on your center’s assessment. Ask the team what activities to avoid, which symptoms to report, and how to reach someone outside scheduled appointments.
How does follow-up work in the short and long term?
Follow-up timing is individualized. Memorial Sloan Kettering gives one center’s example of visits around 30 days, 90 days, 6 months, and 1 year, followed by coordination with a local oncologist. Your schedule may differ; ask which visits will be at the CAR-T center and how care will be coordinated closer to home.
The FDA says required postmarketing observational safety studies include 15 years of follow-up to assess secondary malignancies and long-term safety. That study requirement is not the same as a promise that each patient will have the same clinic-visit schedule for 15 years.
