What caused the implosion?
The NTSB’s technical finding
The NTSB identified OceanGate’s inadequate engineering process as the probable cause. In the Board’s account, the company had not established the pressure vessel’s actual strength and durability. The vessel likely developed delaminations after dive 80, sustained further damage after dive 82, and ultimately failed through local buckling. Flawed analysis of real-time monitoring data meant OceanGate did not recognize the damage and remove the vessel from service, according to the NTSB summary.
How the Canadian investigation describes the failure
Canada’s Transportation Safety Board (TSB) describes the carbon-fibre cylinder as failing progressively as damage accumulated through dive cycles. Its findings summary also says OceanGate did not identify and mitigate key structural risks. The technical accounts complement one another: both describe accumulated vessel damage and shortcomings in recognizing or managing structural risk, rather than workplace culture as the physical failure mechanism. What did investigators mean by a “toxic workplace culture”?
The U.S. Coast Guard Marine Board of Investigation’s 2025 report examined OceanGate’s operational and safety practices, workplace culture, oversight and the casualty. The Board characterized the company’s practices as critically flawed and identified a toxic workplace culture among casualty factors. The Coast Guard report states: “OceanGate’s operational and safety practices were critically flawed, which contributed to the catastrophic implosion of the TITAN submersible.” This is the Marine Board’s finding, not a claim that workplace culture alone physically caused the pressure vessel to fail. The report is available from the Coast Guard’s Titan investigation materials.
The Coast Guard release also points to an ineffective whistleblower process under the Seaman’s Protection Act among the factors examined. Separately, the Canadian TSB says power dynamics and social and psychological factors hindered risk management. Together, those findings explain how organizational conditions can obstruct the identification and response to risk; they do not replace the NTSB’s engineering-based probable-cause conclusion.

These bodies addressed different parts of the disaster, so their accounts are complementary rather than competing explanations. The NTSB is clearest on the physical failure and probable cause; the Coast Guard Board on workplace culture and marine oversight; and the Canadian TSB on risk-management dynamics and progressive structural failure.

The Coast Guard’s 2025 release says the Marine Board report included 17 safety recommendations. They address strengthening oversight, improving coordination among federal agencies, and closing gaps in international maritime policy. The NTSB also noted that voluntary guidance and current U.S. small-passenger-vessel regulations were not sufficiently tailored to pressure vessels designed for human occupancy. These points frame the regulatory issue as one of whether oversight and rules adequately account for this type of vessel, not simply whether rules existed.

Marine Board chair Jason Neubauer said in the Coast Guard release, “This marine casualty and the loss of five lives was preventable.” That is the chair’s statement about preventability; it should not be confused with the NTSB’s separate technical determination of probable cause.
