It is a transition into a connected operating system: mine development, mobile equipment, water controls, tailings storage, power, processing, workers and emergency systems must perform together. A weakness in one link can delay output, increase costs or create safety and environmental consequences.
The main risk groups are worker safety and ground control; commissioning and equipment reliability; geology, grade, recovery and ramp-up uncertainty; water and tailings containment; environmental and community effects; and project finance, costs and markets. Their likelihood and severity vary materially with the mine method, commodity, geology, climate, infrastructure and jurisdiction.

Why production start-up is a high-risk transition
A mine can be physically built yet not be operationally ready. During start-up, systems that may have been tested separately must work at the same time and at commercial scale. Ore must reach the plant at a predictable rate, the plant must recover saleable material, water must be supplied and managed, tailings must be contained, and workers must operate safely under changing conditions.
A project execution plan cited in an Australian Securities Exchange filing illustrates the dependencies. Its phased production build-up included mining-equipment procurement, orebody development, water-treatment and surface-water infrastructure, tailings commissioning and process-plant commissioning. That is an example from one project, not a universal checklist, but it shows why a late pump, incomplete tailings facility or underperforming circuit can hold back the entire ramp-up.

There is no reliable global percentage for the chance that a new mine will miss its planned production target, and the available regulator programs do not establish a cross-mine incident rate. A mine-specific technical study, operating history and regulator record are more informative than a generic probability.
1. Worker safety and ground-control hazards
Surface mines
Open-pit operations can expose workers and equipment to unstable benches, pit-wall movement, falling rock, vehicle interactions, blasting hazards and water entering the pit. Geological conditions encountered during excavation may differ from the assumptions used in the design. A wall that appears stable during development can become less predictable as excavation deepens, water pressures change or structures in the rock mass are exposed.
Underground mines
Underground operations add hazards associated with unsupported ground, rockbursts, seismic activity, ventilation, fires, explosives, confined access and inrushes of water or other materials. The relevant controls depend on the mining method, depth, rock properties and the mine’s ventilation and emergency systems. A risk disclosure for a mining project identifies geological uncertainty as a factor that can affect both mine design and operational safety; it also identifies pit-wall instability and ore dilution as project risks.
What readiness looks like
Before sustained production, operators normally need current ground models, support standards, inspection routines, exclusion zones, communications, refuge or evacuation arrangements and trained emergency teams. These are controls, not guarantees. Conditions can change faster than a design model, so monitoring and the authority to stop work are essential. The sources do not rank surface and underground hazards across all mines; the correct comparison is site-specific.
2. Commissioning failures and unreliable equipment
Commissioning is the period in which equipment and processing circuits are started, tested, adjusted and handed over for routine operation. A mine may achieve mechanical completion while still failing to meet throughput, availability or recovery expectations. Conveyors can misalign, crushers can choke, pumps can fail, electrical systems can trip and control software can behave differently under full loads than during a short test.

A disclosed project risk states that successful commissioning and trial production cannot be guaranteed. It specifically points to breakdowns, maintenance problems, stoppages, delays and additional costs. This is a disclosure of possible outcomes, not an incident frequency or a prediction that a particular mine will fail.
Why small failures become schedule problems
- Single points of failure: one unspared pump, transformer, crusher or thickener can stop a whole circuit.
- Interface gaps: mining, stockpiles, crushing, grinding, flotation or leaching and tailings may each pass an isolated test but fail at their connection points.
- Maintenance learning: a new fleet has no site-specific failure history, spare-parts consumption data or established preventive-maintenance intervals.
- Operator learning curves: crews must learn control-room procedures, lockout rules and abnormal-condition responses while production pressure is rising.
Commissioning plans should therefore include realistic load tests, spare capacity, critical-spares inventories, vendor support, maintenance systems and a controlled progression from first ore to trial production and then commercial production. The exact sequence varies by mine and plant design.

3. Geological, grade, recovery and production-ramp uncertainty
The production plan is built on a geological model and assumptions about ore hardness, mineralogy, grade distribution, dilution, recovery and operating rates. The exposed ore can differ from the model. Lower grade, more waste, harder rock or minerals that respond poorly to the chosen process can reduce tonnes, recovery or saleable product at the same time.
Ore dilution and grade variability
Dilution adds waste or low-grade material to mined ore. It can reduce feed grade, consume haulage and processing capacity and increase the volume of tailings. Grade variability can also make a plant look unreliable when the underlying issue is feed composition rather than a mechanical fault. Reconciliation between the short-term model, actual mined material and plant assays is a critical early-production discipline.
Recovery and throughput
“Nameplate capacity” is a design reference, not an automatic first-year result. A circuit may reach its tonnes-per-hour target but recover less metal, or achieve recovery only at a lower feed rate. Water chemistry, reagent consumption, grind size, residence time and ore mineralogy can all affect results. Production guidance should be read with its assumptions and ramp-up period, rather than treated as a guaranteed run rate.

How to interpret a missed target
A shortfall does not by itself prove that the mine design is defective. Investigators should separate geological variance, planned commissioning losses, equipment availability, weather, labor constraints and market-driven operating decisions. Without the mine’s technical disclosures and operating data, outside readers should avoid assigning a single cause.

4. Water supply, inrush and tailings containment
Water is both a production input and a hazard. Mines need water for processing, dust control and sometimes underground services, while rainfall, groundwater and snowmelt must be kept away from workings and facilities or safely stored. Tailings facilities must contain finely ground waste and manage the water that remains with it.
Lifecycle tailings risk
The NSW Resources Regulator treats tailings-storage-facility risk as a lifecycle issue covering design, construction, operation and decommissioning. A failure can injure or kill workers or members of the public and contaminate rivers, land, groundwater and wildlife. These consequences explain why tailings risk remains material after a facility has passed construction inspections.
The regulator’s 2024–2025 compliance-priority program assessed 37 operations for hazards including inundation or inrush and uncontrolled movement involving tailings dams, water storages or waste tips. That number is the size of the NSW assessment program, not a global mine count, a failure rate or an estimate of incident probability.
Water-balance errors and inrush
Inrush can flood an underground working or overwhelm a surface facility. A water-balance model that omits an inflow pathway, storm event or surplus-water volume can leave too little storage or pumping capacity. The regulator’s recommendations included reviewing site hazard plans, emergency response arrangements and ongoing factor-of-safety calculations as facilities develop. Those are NSW regulator recommendations, not a universal legal checklist for every jurisdiction.

Mount Polley: a documented case
On August 4, 2014, the Mount Polley tailings facility in British Columbia breached. A 2015 British Columbia government investigation summary describes a structural failure combined with insufficient beaches and excess supernatant water, producing an erosional breach. It also identifies inadequate water management and incomplete characterization of surplus water among the shortcomings discussed by the investigation. The event demonstrates how design, water and operating conditions can interact; it does not establish that every new mine has the same conditions or cause.
Controls that matter during ramp-up
- Maintain a current water balance that includes ordinary operations, extreme-weather scenarios and credible failure modes.
- Track pond levels, seepage, pore pressures, deformation and freeboard against action thresholds.
- Recalculate stability factors as construction stages, loading and water conditions change.
- Test alarms, communications, evacuation routes and mutual-aid arrangements before they are needed.
- Define who can reduce throughput, stop deposition or declare an emergency without waiting for commercial approval.
5. Environmental incidents and community effects
Start-up can change a site’s environmental footprint quickly. Clearing, traffic, noise, dust, blasting, water withdrawals, process chemicals and waste streams may all increase as the operation moves from construction to routine output. Communities may experience changed road use, employment patterns, housing demand or concerns about water and cultural sites.
Potential impacts depend on the mine’s location and receptors. A spill near a seasonal stream is different from one in a contained industrial area; the relevant questions are where contaminants can travel, how quickly they can be detected and whether downstream users or ecosystems are exposed.
Evidence from a lithium project assessment
A comprehensive assessment report for the North American Lithium Spodumene Mine Project records two accidental spills after start-up. In discussing one water-related event, the report presents the proponent’s explanation involving water accumulation beneath a tailings-facility liner and hydraulic mechanisms, alongside site observations. That account should be attributed to the assessment record and the proponent; it is not an independent finding that the proposed mechanism is the sole cause.
Community trust as an operating issue
Permit conditions, monitoring results and incident notifications are often scrutinized more closely once production begins. Delayed or unclear communication can increase opposition even when physical impacts are limited. Operators need defined reporting thresholds, accessible complaint channels, independent review where required and a process for correcting monitoring failures. Requirements differ by jurisdiction and by the mine’s approvals.

6. Costs, funding and market pressure
A technically ready mine can still struggle if its capital budget, financing or revenue assumptions no longer hold. A project risk disclosure identifies funding constraints, cost overruns and market-related conditions as possible threats. These are project-specific business risks, not universal explanations for every delayed mine.
Cost escalation
Construction inflation, exchange rates, fuel, power, contractor rates and imported equipment can raise the cost of completing or operating a mine. A delay can compound the effect by extending interest, temporary facilities, staff and contractor costs while postponing revenue. Early production can also require more sustaining capital than the original schedule assumed.
Funding and liquidity
Some projects need additional debt or equity before they reach positive cash flow. If lenders impose conditions, a sponsor withdraws support or a required financing does not close, management may defer waste stripping, expansion or maintenance even when ore is available. Readers assessing a company should examine liquidity, committed facilities, maturity dates, covenants and the assumptions behind the production schedule.
Commodity and offtake exposure
Prices, treatment charges, exchange rates and customer specifications affect revenue. A mine that produces a saleable concentrate may still face penalties, transport bottlenecks or delayed offtake. Management may deliberately reduce throughput when prices are weak, so production below the original plan is not always a mechanical failure.
How the risks interact during a ramp-up
Risk categories are not independent. A harder-than-expected orebody can lower throughput, increasing stockpiles and changing water and tailings volumes. A pump failure can interrupt processing, alter pond levels and create an environmental-control problem. A wall movement can stop a mining area, force a new sequence and raise costs while financing is already tight.
For that reason, a useful readiness review tests interfaces rather than checking only whether each department has completed its own tasks. Management should be able to show which assumptions are most sensitive, what early-warning indicators will be watched, and which decisions are triggered when an indicator moves outside its range.
A practical start-up readiness checklist
- Confirm the mine plan against current geology. Reconcile mined tonnes, grade, dilution, hardness and groundwater observations with the short-term model.
- Prove critical equipment under realistic loads. Test power, conveyors, crushers, pumps, control systems and process circuits with operating crews and planned redundancy.
- Verify water and tailings capacity. Update the water balance, freeboard, seepage controls, stability calculations and deposition plan for the next operating stage.
- Exercise emergency arrangements. Include ground failure, inrush, fire, loss of power, extreme rainfall, spill and tailings scenarios; check communications beyond the site boundary.
- Set ramp-up decision gates. Define what qualifies as mechanical completion, first ore, trial production and commercial production, with measurable criteria for throughput, recovery, availability and environmental performance.
- Stress-test the financial plan. Model lower grades, slower ramp-up, higher capital costs, downtime and weaker prices, then identify the funding required under each case.
- Publish and review monitoring results. Regulators, workers and communities need timely information about incidents, water quality, complaints and corrective actions.
What investors, workers and nearby residents should ask
- What mine method, commodity, climate and jurisdiction shape the risk profile?
- Which assumptions have the widest effect on production: grade, dilution, hardness, recovery, water or equipment availability?
- What independent engineering, regulator inspections or technical reports support the tailings and water-control claims?
- How much cash and committed funding remain if commercial production is delayed?
- What conditions trigger a production reduction, evacuation, spill notification or redesign?
- Are reported incidents and near misses distinguished from planned commissioning losses?
The answers should come from the operator’s technical reports, permits, regulator records and financing disclosures. A generic industry ranking cannot substitute for those documents.
Bottom line
The main risks when a mine starts production are the failure of connected systems: people working in changing ground conditions, equipment and plants that have not yet demonstrated reliable commercial performance, ore that differs from the model, water and tailings that exceed design assumptions, environmental incidents that affect communities, and financial or market conditions that remove the margin for delay. The severity and likelihood are mine-specific, so the most credible assessment combines the mine method and geology with its commissioning evidence, water and tailings controls, emergency readiness and funding plan.
