Brian Thompson, UnitedHealthcare’s chief executive, was shot outside the Hilton Midtown in Manhattan on December 4, 2024. The gunman was unidentified and still at large in the first days after the shooting. Luigi Mangione was arrested in Altoona, Pennsylvania, on December 9—five days later. By October 2, 2026, the latest case update available here, Mangione had pleaded guilty to federal stalking charges, while his New York state murder case was postponed indefinitely.
What happened in the UnitedHealthcare CEO shooting?
Thompson was shot outside the Hilton Midtown in Manhattan on the morning of December 4, 2024, as he arrived for UnitedHealth Group’s investor conference. A contemporaneous Washington Post visual-forensics report published the next day examined police-released images and surveillance footage. It described a person whose clothing matched that of the sought suspect near the hotel before the shooting; it was a reconstruction based on footage and police statements, not an eyewitness account by the newspaper.
Why was the gunman described as still at large?
In the days immediately after the shooting, police were seeking an unidentified suspect. That description belongs to the initial manhunt, before authorities announced an arrest; it should not be read as the suspect’s current status.
On December 9, 2024, New York City Police Commissioner Jessica Tisch announced that Luigi Mangione had been arrested in Altoona, Pennsylvania. “Earlier this morning in Altoona, Pennsylvania, members of the Altoona Police Department arrested Luigi Mangione, a 26-year-old male on firearms charges,” Tisch said at the news conference that day. She said an employee at a McDonald’s recognized Mangione and contacted local police. Tisch also described a firearm, suppressor, clothing and identification that officials said they recovered. Those details were part of the officials’ arrest announcement, not a final court finding. Read the city’s transcript of the December 9 announcement.

What did federal prosecutors allege?
In a December 19, 2024 announcement, the Justice Department summarized allegations in a federal complaint about planning, travel to New York, reconnaissance, the shooting and the arrest. The complaint’s account was a set of allegations, not proof of guilt. The Justice Department said Mangione is presumed innocent unless and until proven guilty. An Associated Press report dated October 2, 2026, said Mangione had pleaded guilty in August to federal stalking charges and was due to be sentenced in federal court in December 2026. The report also said he still faced murder charges in New York state court, where proceedings had been postponed indefinitely while his lawyers sought to have the case dismissed in a double-jeopardy dispute. A separate AP report on August 17, 2026, described the postponement and dispute. Neither the scheduled federal sentencing nor the state case was a final outcome in the October 2 report.

This status is dated: developments after October 2, 2026, are not verified here. The early “still at large” description is therefore a historical account of the first days after the shooting, not a current fugitive-status update.
