UnitedHealth Group reported first-quarter 2026 revenue of $111.7 billion on April 21, up 2% from a year earlier, with reported earnings of $6.90 per share and adjusted, non-GAAP earnings of $7.23 per share. The insurer raised its full-year adjusted earnings outlook to more than $18.25 per share. Its medical care ratio improved year over year, although management said utilization and unit-cost trends remained elevated.
The headline claim that UnitedHealth “topped estimates” requires a dated, attributable analyst-consensus figure that is not established by the company’s release or SEC filing. The verified facts are the company’s reported results, its guidance increase and its explanation of medical-cost trends.

What UnitedHealth reported for the first quarter
| Measure | Q1 2026 | Comparison or qualification |
|---|---|---|
| Revenue | $111.7 billion | Up 2% year over year |
| Reported EPS | $6.90 | GAAP measure reported by the company |
| Adjusted EPS | $7.23 | Non-GAAP measure; not the same as reported EPS |
| Medical care ratio | 83.9% | 84.8% in Q1 2025 |
| Operating cost ratio | 13.8% | 12.4% a year earlier |
| Net margin | 5.6% | Separate from the medical care ratio |
Revenue measures the group’s total reported sales. Reported EPS reflects generally accepted accounting principles, while adjusted EPS excludes items defined in the company’s non-GAAP reconciliation. Those figures should not be presented as interchangeable.
Why the medical care ratio matters
The medical care ratio (MCR) measures medical costs as a share of premiums. UnitedHealth’s 83.9% Q1 ratio means that, on this measure, the company incurred 83.9 cents of medical costs for each dollar of premium revenue. It is not a net profit margin; the company separately reported a 5.6% net margin.

Year-over-year movement
The Q1 MCR fell from 84.8% to 83.9%. UnitedHealth attributed the improvement to medical-cost management and favorable reserve development. The company also said consistently elevated utilization and unit-cost trends partly offset those benefits, so the lower ratio does not establish that underlying medical demand or prices had eased broadly.
Effect of a prior reserve
The SEC-filed exhibit said the quarter’s MCR included a 20-basis-point positive impact from a previously disclosed Optum Health loss-contract reserve. That company-reported accounting contribution helped the ratio, but it is not evidence that current utilization or unit costs improved by the same amount.

What changed in the profit outlook
In the April 21 release, UnitedHealth forecast reported full-year 2026 EPS of greater than $17.35 and adjusted EPS of greater than $18.25. The adjusted figure is the company’s non-GAAP outlook and should be compared with adjusted results, not substituted for the reported measure.

The company’s operating cost ratio rose to 13.8% from 12.4% a year earlier.

July update: guidance moved higher again
The April outlook was not the final guidance change verified for 2026. On July 16, UnitedHealth reported second-quarter revenue of $112.0 billion, reported EPS of $6.04 and adjusted EPS of $6.38. It then raised its full-year adjusted EPS outlook to $19.50–$20.00.
| Guidance date | Full-year adjusted EPS outlook | Context |
|---|---|---|
| April 21, 2026 | Greater than $18.25 | Issued with Q1 results |
| July 16, 2026 | $19.50–$20.00 | Raised with Q2 results |
Second-quarter MCR was 86.7%. UnitedHealth said that ratio reflected cost and pricing discipline and changes in the mix of benefit offerings; its filing also noted favorable prior-period development. The available verified results run through Q2, so they should not be described as full-year results or as the latest quarter beyond that date without a newer primary release.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How to interpret the “tops estimates” wording
A results headline normally compares actual earnings with an analyst consensus published before the release. UnitedHealth’s April release and SEC exhibit substantiate the $6.90 reported EPS, $7.23 adjusted EPS and $111.7 billion revenue figures, but they do not state the consensus estimate or the size of any beat. Without a dated, attributable consensus source, the estimates comparison remains unverified.
For a sound comparison, identify whether analysts expected reported or adjusted EPS, specify the consensus date and provider, and compare the same revenue and EPS definitions. Mixing adjusted company earnings with a GAAP consensus, or vice versa, can create a misleading “beat.”
What investors should watch next
- Medical-cost trend: Track MCR over matching quarters and separate current-period utilization and unit costs from reserve development.
- Guidance durability: Compare each outlook by issue date; the April threshold above $18.25 and July range of $19.50–$20.00 are not equivalent formats.
- Operating expenses: The higher Q1 operating cost ratio reflects investments the company said were directed to people, processes and technology.
- Reported versus adjusted earnings: Use the company’s reconciliation when assessing how non-GAAP adjustments affect the headline profit number.
- Benefit mix and pricing: The Q2 explanation indicates that product mix and pricing discipline can move MCR even when the underlying health-care cost environment remains pressured.
Management’s stated strategy
Chief Executive Officer Stephen Hemsley said the company was “continuing to help simplify and modernize health care for the people and care providers we serve, bringing greater value, affordability, transparency and connectivity.” That is a statement of management’s aims, not independent evidence that care became more affordable or transparent.
