Como localizar e preencher o requerimento certo
- Identifique o município responsável. Use o município onde fica o imóvel ou estabelecimento e procure o site oficial da prefeitura ou o departamento municipal de licenciamento, cadastro ou atividades econômicas.
- Defina a atividade e o tipo de licença. Informe a atividade que será exercida e verifique qual autorização ou cadastro municipal se aplica. Não escolha um formulário apenas porque seu título contém “alvará”.
- Baixe ou preencha o formulário oficial. Siga o canal indicado pela prefeitura. Algumas administrações oferecem formulário para gerar documento, protocolo online ou ambos; essas opções não estão disponíveis em todas as cidades.
- Confira documentos e etapas antes de enviar. Verifique a lista vigente, o formato dos anexos, eventual taxa, inspeções e outras aprovações necessárias diretamente com o setor responsável. Guarde o comprovante de protocolo e acompanhe o pedido pelo canal informado.

A Prefeitura de Monte Azul Paulista oferece um requerimento inicial destinado a pessoas jurídicas ou MEIs. No caso de São Paulo, o artigo 2º, parágrafo 1º, do Decreto Municipal 49.969/2008 afirma que o simples protocolo de pedido de Auto de Licença de Funcionamento, Alvará de Funcionamento ou Alvará de Autorização não autoriza o funcionamento da atividade. Essa disposição se refere ao regime municipal de São Paulo; não estabelece, por si só, o efeito do protocolo em todos os municípios.
