OSHA opened an inspection at a Rivian facility in Illinois on March 5, 2026, after a contractor died at a warehouse earlier that week, according to TechCrunch and the agency’s public establishment search. TechCrunch reported that local authorities said the worker was pinned between a tractor trailer and a loading dock. The available public record does not establish OSHA’s final findings or whether the agency will issue citations.
What was reported about the fatal incident?
TechCrunch reported on March 6, 2026, that 61-year-old contractor Kevin Lancaster died after being pinned between a tractor trailer and a loading dock at a Rivian warehouse in Illinois. The account attributed Lancaster’s identity and cause of death to local authorities; it reported the cause as “blunt traumatic compressional injuries.” Those are reported local-authority details, not an OSHA determination.

TechCrunch, citing local reporting, said emergency crews were called at 1:40 p.m. local time and Lancaster was pronounced dead at a medical center at 2:33 p.m. It reported that he had been trapped for around 20 minutes.
Rivian told TechCrunch: “Safety at our facilities is our top priority. Unfortunately yesterday afternoon, a contractor passed away after an incident at our warehouse. Our sympathy and thoughts are with their family and friends. We are working with the Normal Police Department on its investigation.”

What does OSHA’s public record show?
OSHA’s establishment search lists an Illinois inspection for Rivian opened March 5, 2026, under activity number 1879095.015. The record labels the inspection “Unprog Rel” and “Partial.” Its employer, state and opening date align with TechCrunch’s account, but the listing itself does not describe the fatality. Who is investigating, and how long could OSHA take?
At the time of TechCrunch’s March 6 report, the Normal Police Department and McLean County Coroner were also investigating. TechCrunch reported that OSHA told the outlet its investigation could take up to six months. That is a reported estimate, not a guaranteed deadline or a statement of the investigation’s outcome.

Have OSHA’s findings or any citations been published?
The sources available here do not establish a final OSHA finding or citation. OSHA says investigation summaries are prepared after inspections following fatalities, catastrophes or employer-reported referrals. Summaries generally describe events and causal factors, but the agency notes that summaries for investigations completed within the previous year may not yet be available while work is completed and records are screened. OSHA’s separate fatality-data page is updated daily, but displays only inspections with an opening conference date within the preceding six months. The agency also notes that a victim’s name may be absent for jurisdictional reasons. What remains unknown?
- Whether OSHA will identify violations or issue citations.
- What causal factors OSHA will determine, if any.
- The eventual findings or status of the investigations by local authorities.
Until official results are published, the reported mechanism and cause of death should not be treated as OSHA’s conclusions, and the case does not by itself establish fault, preventability or broader conditions at Rivian facilities.
