NPS Swasthya Explained: Up to ₹30 Lakh Cover, With a ₹3 Lakh Deductible

NPS Swasthya combines a pension investment account with a separate, mandatory health-insurance policy. Under PFRDA’s operational guidelines issued on 18 September 2026, the standard policy’s largest option offers a family floater sum insured of ₹30 lakh—but it has a ₹3 lakh annual aggregate deductible. That deductible is counted across admissible expenses for all covered family members during the policy year, not afresh for every claim.

The insurance is not ₹30 lakh in cash added to your pension account. It is a policy limit, and whether a bill is paid depends on the insurer’s policy wording, exclusions, limits and claim assessment.

What does NPS Swasthya include?

NPS Swasthya has two distinct parts: an NPS investment account intended to build a dedicated healthcare corpus, and a separate health-insurance policy arranged by the pension fund through an IRDAI-registered insurer. The policy is subject to insurance law and its final wording; the pension account and insurance policy are legally and operationally distinct.

PFRDA’s standard policy table lists four family-floater cover options. The ₹30 lakh option is the largest listed, but the guidelines do not establish that every pension fund has launched every option or that every subscriber automatically receives that level of cover.

Family-floater sum insured Annual aggregate deductible
₹1 lakh ₹10,000
₹5 lakh ₹50,000
₹10 lakh ₹1 lakh
₹30 lakh ₹3 lakh

These cover and deductible amounts are from PFRDA’s 18 September 2026 standard policy table. They are not premium quotes. The insurer sets the premium, which can depend on factors such as age; the actual quote and policy documents should be checked before enrollment.

How does the ₹3 lakh deductible work?

For the ₹30 lakh option, the family must first incur ₹3 lakh in cumulative insurance-admissible expenses during the policy year before the super top-up cover begins to apply. PFRDA specifies that the annual aggregate deductible applies to the cumulative admissible expenses of all covered family members; it is not applied separately to each claim.

For example, suppose one covered family member has ₹2 lakh of admissible expenses in one claim and another has ₹2 lakh in a later claim in the same policy year. If all ₹4 lakh is accepted as admissible, the cumulative expenses exceed the ₹3 lakh deductible by ₹1 lakh. The insurer’s payment would still depend on the policy’s terms, limits and claim assessment; this arithmetic does not guarantee a payout.

ASURION 3 Year Personal Care Protection Plan ($40 - $49.99)
  • NO ADDITIONAL COST: You pay no fees for repairs, replacements or reimbursements. Shipping included.
  • IMMEDIATE PROTECTION: Drops, spills, and cracked screens covered for portable products, plus power surge protection from day one. Malfunctions covered after the manufacturer’s warranty ends.
  • EASY CLAIMS: File a claim in minutes at Asurion.com/amazon. We'll repair, replace, or reimburse you up to the purchase price (excluding tax).

  • TERMS AND DETAILS: Terms & Conditions are emailed within 24 hours and available in Your Orders on Amazon. More information about this protection plan is available within the “Product guides and documents” section. Simply click “User Guide” for more info.
  • The deductible is an annual family aggregate, not a fresh threshold for every claim.
  • Only expenses the insurer treats as admissible count toward crossing it.
  • Crossing the deductible does not make every remaining bill payable: exclusions, policy limits and final wording continue to apply.

Who is covered, and what are the age and waiting-period rules?

The standard family floater covers the subscriber, their spouse and up to two dependent children. Parents are not included in this coverage unit. Entry age is 18 to 70. Renewal may continue up to and including age 85, subject to the premium, policy terms and applicable law.

PFRDA’s standard policy guidelines list these waiting periods, subject to the final policy and insurer disclosures:

  • Initial waiting period: 30 days, except for accidents as specified in the final wording.
  • Pre-existing diseases: 12 months.
  • Specified diseases or procedures: 12 months, subject to the final policy and law.

The guidelines describe controlled Type 2 diabetes, hypertension, hyperlipidaemia and asthma as covered after the 12-month initial waiting period when they do not trigger enhanced underwriting under the prescribed declaration. Read the insurer’s underwriting requirements and final policy wording to determine how they apply to an individual applicant.

What does the standard policy say it covers?

PFRDA’s standard policy guidance includes inpatient and domiciliary hospitalisation, medically necessary day-care procedures, AYUSH treatment, prescribed modern treatments and organ-donor medical expenses, subject to the final policy wording. It specifies a single private room for normal hospitalisation and ICU charges at actuals, subject to the sum insured and policy terms.

The guidance also lists 30 days of pre-hospitalisation cover, 60 days of post-hospitalisation cover and road ambulance cover up to ₹2,500 per emergency hospitalisation, unless the final policy sets a higher uniform limit. It says the standard policy has no co-payment or disease-specific sub-limit; consumables and non-payables remain governed by the applicable top-up policy. Confirm all of these details in the actual policy issued by the insurer.

How are insurance claims different from withdrawals from the NPS corpus?

Insurance claims and healthcare withdrawals use different rules. The policy pays only for expenses that meet its terms and, for a super top-up, the deductible threshold. Separately, the NPS Swasthya account permits partial withdrawals for eligible outpatient and inpatient healthcare expenses up to 25% of the subscriber’s contributions to that account.

PFRDA’s guidelines set no minimum waiting period or limit on the number of these partial withdrawals. Payment goes to the hospital, healthcare provider or other eligible entity, not directly to the subscriber. Eligible expenses that insurance does not pay may still be considered under the separate corpus-withdrawal rules; eligibility is not automatic.

If eligible inpatient expenditure in a single instance exceeds the amount available through a permitted partial withdrawal, the subscriber may use the premature-exit process. Any remaining balance is transferred or merged into an All Citizen Model NPS scheme under the guidelines.

What does it cost to join and maintain the account?

Under the 18 September 2026 operational guidelines, minimum initial funding consists of:

  • At least the first-year insurance premium, including applicable taxes.
  • ₹200 plus applicable taxes for annual HBA maintenance.
  • ₹1,000 invested in the Swasthya account.

The minimum subsequent contribution is ₹10. All Citizen Model charges also apply. In addition, the pension fund may charge up to 0.08% a year of the Swasthya corpus under management, plus applicable taxes. The actual insurance premium and charges must be disclosed before enrollment; the guidelines do not provide a universal premium.

What should you check before enrolling?

  • Which cover option is actually available: confirm the sum insured and its corresponding annual aggregate deductible with the pension fund.
  • The insurer’s quote and policy wording: review the premium, taxes, underwriting, exclusions, waiting periods, claim process and grievance route.
  • Who the floater covers: the standard family unit includes the subscriber, spouse and up to two dependent children, but not parents.
  • How the deductible fits your finances: consider whether your household could meet the deductible from available funds if admissible expenses arise.
  • How insurance and corpus rules interact: policy coverage is not the same as access to pension-corpus withdrawals, which have their own eligibility and payment rules.
  • Your broader plans: weigh the arrangement against existing health cover, affordability and your retirement objectives rather than treating the headline sum insured as a guaranteed benefit.

Why older launch details may not describe the current framework

NPS Swasthya began as a limited proof of concept in January 2026. A February 2026 launch announcement described healthcare services through Apollo 24|7 and selected Apollo network hospitals and pharmacies; those were launch-specific details, not universal terms for every later arrangement.

PFRDA’s 7 April 2026 circular set PoC 2 terms, including a ₹25,000 minimum initial contribution. Those terms differ from the minimum funding requirements in the operational guidelines issued on 18 September 2026, which govern the standard structure described here. Check the current offer and disclosures from the pension fund rather than relying on an earlier sandbox announcement.
