Medigap and Medicare Advantage are different ways to manage Medicare coverage, and you generally cannot use both for the same care. Medigap supplements Original Medicare by helping pay certain Part A and Part B costs. Medicare Advantage is a private-plan alternative for receiving Medicare benefits, with plan-specific costs and rules for accessing care.
The practical choice depends on the doctors and hospitals you want to use, your prescriptions, your expected care, local plan details, and what you can afford in premiums and out-of-pocket costs. Neither option is universally cheaper or better.

How the two choices work
Original Medicare consists of Part A hospital insurance and Part B medical insurance. With Original Medicare, you can add a Medigap policy to help pay certain cost sharing, and you can enroll in a separate Part D plan for prescription drugs. A Medigap policy supplements Original Medicare; it does not replace it.
Medicare Advantage, also called Part C, is a private-plan way to receive Medicare-covered services. The plan takes the place of Original Medicare for how you receive those benefits and sets plan-specific cost sharing and, for non-emergency care, may set network, service-area, and referral rules. Medicare Advantage plans may also include prescription drug coverage and some extra benefits.

You cannot use Medigap to pay Medicare Advantage premiums, deductibles, copayments, or coinsurance. To use Medigap, you generally need to be enrolled in Original Medicare Parts A and B. Each Medigap policy covers one person, so household members make their own coverage choices and need separate policies if they want Medigap. Medicare explains the structural differences in its Original Medicare and Medicare Advantage comparison.

Provider access: flexibility or a plan network?
Original Medicare with Medigap
Medicare.gov says, “You can use any doctor or hospital that takes Medicare, anywhere in the U.S.” That broad access can matter if you see specialists in different locations, travel within the country, or want to avoid choosing care solely from a plan directory. It is not a guarantee that every provider is accepting patients or that every provider participates in Medicare. Ask the clinician or facility whether it takes Medicare, and check how Medicare assignment affects your costs.

That can be useful when your care involves providers in more than one location. It still does not mean every clinician will accept you as a patient, or that every service has the same cost. Confirm participation and ask what you may owe for the specific care you expect.
Medicare Advantage
Medicare.gov says, “You may need to use doctors and other providers who are in the plan’s network and service area (for non-emergency care).” Whether a particular doctor is in-network, whether a referral is needed, and what out-of-network treatment costs depend on the plan. Emergency and urgent-care rules are not a substitute for checking routine-care access.
Before enrolling, verify each important doctor, specialist, hospital, and other facility against the current directory for the specific plan and service area. Then confirm directly with the provider: directories can change, and a provider’s general acceptance of Medicare does not establish participation in a particular Advantage plan. Ask the plan how it handles out-of-network care and whether referrals or prior authorization apply to the services you expect to use.

How to compare the real costs
A premium alone does not show what a year of coverage may cost. Original Medicare generally has 20% Part B coinsurance after the Part B deductible and no annual out-of-pocket limit unless you have supplemental or other qualifying coverage. Medigap can pay some covered cost sharing according to its benefit letter, but it has its own premium and does not cover every type of expense.

A plan has a yearly limit for covered Medicare services, but its premiums, deductibles, copayments, coinsurance, drug costs, and rules determine what you may pay on the way to that limit. The limit is not a ceiling on all possible spending: it applies to covered Medicare services, not necessarily to services the plan does not cover or other expenses outside its terms. Review the plan’s documents to understand which services count toward the limit.
A plan advertised with a $0 plan premium still requires payment of the Part B premium. Conversely, it is not accurate to assume Medigap always costs more: Medigap premiums vary by policy and location, and total costs depend on care use and the other coverage you select. Medicare describes this variation in its coverage comparison.
Think of the two cost patterns as different rather than as a simple low-premium versus high-premium choice. With Original Medicare and Medigap, the Medigap premium is a separate, recurring cost intended to help with certain covered cost sharing; you may also pay for Part D. With Medicare Advantage, the plan’s cost sharing applies when you receive care, and the annual limit provides a boundary for covered Medicare services. The balance between premiums and costs when care is used is plan- and policy-specific.
Build a likely-year estimate
For each option, estimate a year using your expected care rather than the lowest advertised premium. Include the same services and prescriptions in both estimates where possible.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →- List fixed premiums. For Original Medicare with Medigap, include the Part B premium, Medigap premium, and any separate Part D premium. For Medicare Advantage, include the Part B premium and any plan premium.
- Add likely medical cost sharing. Use the policy or plan documents to estimate deductibles, copayments, and coinsurance for the services you expect to use.
- Check prescription costs. Compare your medicines against the plan’s drug list and account for its pharmacy network and stated cost sharing. For Original Medicare, include the separate Part D plan you would use.
- Account for the protection limit or policy benefits. For an Advantage plan, note its annual limit for covered Medicare services. For Medigap, identify which cost-sharing benefits the lettered policy covers and any relevant limit.
- Include uncovered care and access trade-offs. Consider services not covered, out-of-network costs, referral or authorization requirements, and whether your preferred providers are accessible under the option.
Use plan documents and current quotes for the year you are comparing rather than relying on a prior year’s premium or benefit summary. If a cost is uncertain, note the uncertainty instead of treating the lowest possible charge as certain. This estimate is a planning comparison, not a promise of your final bill. Actual costs depend on services used, the plan or policy terms, and the providers and pharmacies involved.

What Medigap’s standardized letters mean
In most states, Medigap policies are standardized by letter: policies with the same letter provide the same basic benefits, even when sold by different companies. Price is the difference to compare among same-letter policies, but the price and availability of policies vary by location. Not every letter is available in every state, and Plan C and Plan F are not available to everyone; eligibility depends on when you first became eligible for Medicare.
Standardization makes it easier to compare benefit designs, but it does not make every policy equally priced or guarantee that a specific letter is offered where you live. When comparing two companies’ versions of the same letter, focus on their quoted premiums and confirm the quote and policy terms with the company. Compare like with like: a quote for one benefit letter is not a direct price comparison with a different letter that covers a different set of benefits.
Medicare’s benefit comparison lists what each standardized policy covers. To shop, first decide which benefit letter fits your needs, compare policies offered in your area, and request an official quote from the company. Check the quote and policy terms directly because prices can change. Medicare’s Medigap coverage guide also describes what Medigap does not cover.

2026 Medigap amounts to know
The following amounts are specific to 2026, not permanent thresholds or a forecast of your total yearly spending. CMS lists them in its Medigap benefit comparison:
- $283 Part B deductible: the 2026 amount relevant to the Plan K and Plan L annual-limit provisions.
- $2,950 high-deductible threshold: in 2026, a high-deductible Plan F or Plan G beneficiary pays up to this amount for Medicare-covered costs before the policy pays.
- $8,000 Plan K out-of-pocket limit: the 2026 limit.
- $4,000 Plan L out-of-pocket limit: the 2026 limit.
After the applicable annual limit and the Part B deductible are met, Plan K and Plan L pay 100% of covered services for the rest of the calendar year. These are benefit provisions, not a prediction that a person will spend exactly the limit. The high-deductible threshold likewise describes what the beneficiary pays for Medicare-covered costs before the policy pays; it is not the policy premium. Confirm current-year figures and policy details when shopping, because these amounts can change over time.
Prescription drugs and extra benefits
Most Medicare Advantage plans include Part D, but “included” does not mean every medicine or pharmacy is treated the same way. Check the plan’s drug list for each prescription and review its pharmacy network and cost sharing before choosing. A plan’s medical network and drug coverage are separate checks. A doctor’s participation in a plan does not establish that your medicines or preferred pharmacy have favorable coverage.

With Original Medicare and Medigap, a separate Part D plan may be used for prescriptions. Medigap policies sold after 2005 do not include prescription drug coverage. Medigap generally also does not cover routine dental or vision care, hearing aids, glasses, or long-term care. Some Medicare Advantage plans offer some dental, vision, or hearing benefits, but the exact services, coverage limits, and conditions are plan-specific. Read the plan documents rather than treating an extra-benefit label as comprehensive coverage.
Do these 3 things before closing this tab:

Original Medicare’s nationwide provider access can be useful when receiving care away from home if a provider takes Medicare. Separate from that domestic access, some Medigap policies cover emergency care abroad under specific terms and limits; do not assume every policy does.
Medicare Advantage generally does not cover routine care outside the plan’s service area, though a plan may provide emergency or urgent travel coverage. Check the plan or policy’s travel provisions for the destinations and care you expect. Neither arrangement should be treated as blanket international health coverage.
A practical checklist before you choose
- Write down the doctors, specialists, hospitals, and facilities you want to use; confirm access under the exact plan or Medicare participation.
- List every prescription and check the applicable plan’s drug list, pharmacy network, and costs.
- Compare total likely-year spending, not just the monthly plan premium: premiums, deductibles, copayments or coinsurance, drug costs, and any relevant out-of-pocket limit.
- Review the plan’s service area, network rules, referral requirements, and prior-authorization rules in its current documents.
- Check which dental, vision, hearing, or travel benefits are actually covered and their limits.
- If considering Medigap, choose a benefit letter first, then request quotes for policies with that letter in your area.
- Use Medicare’s official comparison and relevant plan or policy documents for the current year. Recheck details for the year you will have coverage.
When reviewing the documents, make sure you are looking at the exact plan name and coverage year, not a similar plan or a summary for a different year. Keep notes on the answers you receive about provider participation, prescriptions, referrals, authorization, and cost sharing. Those details are more useful for your decision than a broad claim about what Medicare Advantage or Medigap usually costs.
Which option may fit your priorities?
Medigap with Original Medicare may fit someone who values broad access to providers that take Medicare and wants help paying certain Original Medicare cost sharing, and who can budget for the separate policy premium and any Part D coverage. Its standardized benefits make same-letter policies comparable, but local prices and availability still matter.
Medicare Advantage may fit someone whose preferred providers and medicines work well with a local plan and who values plan-based cost sharing or available extra benefits. That choice requires accepting the plan’s network, service-area, and other rules for non-emergency care, and checking how its costs apply to the care you expect.

Frequently Asked Questions
Can I have Medigap and Medicare Advantage at the same time?
Medigap is designed to supplement Original Medicare, not Medicare Advantage. You cannot use a Medigap policy to pay Medicare Advantage premiums, deductibles, copayments, or coinsurance. To use Medigap, you generally need Original Medicare Parts A and B.
Which option is cheaper: Medigap or Medicare Advantage?
There is no universal cost winner. Medigap has a separate premium, and you may also need a Part D premium; Medicare Advantage costs depend on the plan premium and its cost sharing. A $0 Medicare Advantage plan premium does not remove the Part B premium. Compare premiums, expected care costs, prescription costs, and the applicable coverage limit for your area and year.
Can I keep my doctor with Medicare Advantage?
Possibly, but check the exact plan’s current provider directory and confirm directly with the doctor or facility. For non-emergency care, Medicare Advantage may require providers in its network and service area; out-of-network costs and referral rules vary by plan. A provider’s general acceptance of Medicare does not confirm participation in a particular Advantage plan.
Does Medigap cover prescription drugs?
Medigap policies sold after 2005 do not include prescription coverage. With Original Medicare and Medigap, you can choose a separate Part D plan. Most Medicare Advantage plans include Part D, but check the plan’s drug list, pharmacy network, and costs for your prescriptions.
Does Medicare Advantage have an out-of-pocket maximum?
Medicare Advantage has a yearly limit for covered Medicare services. The limit and the cost sharing that applies before you reach it vary by plan, and it is not necessarily a ceiling on every expense, including services the plan does not cover. Original Medicare generally has no annual out-of-pocket limit unless you have supplemental or other qualifying coverage.
Does Medigap cover dental, vision, hearing, or care abroad?
Medigap generally does not cover routine dental or vision care, hearing aids, glasses, or long-term care. Some policies cover emergency care abroad subject to their terms and limits. Medicare Advantage plans may offer certain dental, vision, hearing, or emergency and urgent travel benefits, but coverage and limits depend on the specific plan.

The Bottom Line
Choose based on the coverage arrangement that fits your providers, prescriptions, expected care, travel, and budget—not on a premium headline. Medigap supplements Original Medicare and may offer broad access to providers who take Medicare, with a separate policy premium. Medicare Advantage uses plan-specific networks and cost sharing, with a yearly limit for covered Medicare services and possible extra benefits. Check current-year documents, provider and pharmacy access, and actual quotes before deciding.
