Medicare did change how it pays for specialized skin substitutes in 2026, but the final rate was not the roughly $809 per square inch described in a 2025 proposal-era news report. The Centers for Medicare & Medicaid Services (CMS) finalized an approximate rate of $127.28 per square centimeter for the physician fee schedule. The products are clinician-applied wound-care materials—not ordinary store-bought bandages—and a payment change is not the same as a decision about which treatments Medicare covers.
What the “weird bandages” are—and what they are not
Skin substitutes are specialized materials clinicians apply to wounds as part of wound care. They may be synthetic, derived from animal tissue (xenogeneic), or made from human tissue (allogeneic); their regulatory pathways vary. CMS aligned payment categories with FDA regulatory status, including 361 human cells, tissues, and cellular and tissue-based products (HCT/Ps), premarket approval (PMA) products, and 510(k) devices.
They are not the adhesive bandages people buy at a pharmacy. CMS describes ordinary surgical dressings under a separate benefit. That benefit covers primary and secondary dressings required for certain surgical wounds and qualifying care after debridement; examples of secondary dressings include adhesive tape, roll gauze, bandages, and disposable compression material.
What the $10 billion figure measures
The figure comes from Medicare Part B spending, not a tally of ordinary bandages. In a September 2025 report, the Department of Health and Human Services Office of Inspector General (HHS OIG) said annual Part B expenditures for skin substitutes had exceeded $10 billion by the end of 2024. OIG attributed the rapid growth to increased use and higher prices and warned of questionable billing and vulnerability to fraud. Those findings do not establish that every claim is fraudulent or that every product lacks clinical value.

A separate figure should not be mistaken for the OIG’s number: in a July 16, 2026 press release, the U.S. House Committee on Oversight and Government Reform reported more than $14.4 billion in Medicare Part B spending in 2025. That is the committee’s reported figure for a different year and source; it is not the OIG’s 2024 finding.
How the 2026 payment change works
CMS finalized a change in its October 31, 2025 Calendar Year (CY) 2026 Physician Fee Schedule final-rule fact sheet. When skin substitutes are used in a covered application procedure, CMS treats the products as incident-to supplies. The policy applies to covered procedures paid under the physician fee schedule in the non-facility setting or under the hospital outpatient prospective payment system.

For CY 2026, CMS set an approximate physician fee schedule payment rate of $127.28 per square centimeter. CMS chose one rate based on the highest average among the product categories, with differentiation by FDA category intended for later rulemaking. The amount is a Medicare payment rate for the specified context, not a retail price or a guarantee that a patient will owe that amount.

CMS said it expected the finalized change to reduce spending by nearly 90% without compromising access or quality. That was the agency’s expectation, not a measured post-implementation outcome. A June 2026 Health Affairs Scholar analysis discusses the reform and argues that additional policies may be needed for sustainable wound care; it is analysis, not a CMS finding.

Payment reform did not settle the coverage question
Payment rules determine how Medicare pays when a covered service is furnished; coverage rules determine whether a service is covered in a particular circumstance. CMS announced on December 24, 2025 that local coverage determinations (LCDs) for certain skin-substitute graft applications for diabetic foot and venous leg ulcers, which had been scheduled to take effect January 1, 2026, were withdrawn effective immediately. That withdrawal did not undo the separate CY 2026 payment change.
CMS’s WISeR (Wasteful and Inappropriate Service Reduction) model is another distinct policy. It runs from January 1, 2026 through December 31, 2031 in New Jersey, Ohio, Oklahoma, Texas, Arizona, and Washington, and includes skin and tissue substitutes. CMS says WISeR does not change Medicare coverage or payment policy; it is context for review and utilization controls, not a replacement for the national payment rule.

What this means for Medicare beneficiaries
The 2026 rule changes Medicare’s payment treatment for products used in covered application procedures. It does not make skin substitutes ordinary supplies, establish that a specific product is appropriate for a particular wound, or by itself answer whether an individual treatment is covered. Ask the clinician what product and procedure are planned, and ask the billing office or Medicare plan how coverage and any cost-sharing apply to the specific service. The cited policy announcements do not establish a post-implementation estimate of clinical outcomes or a complete evaluation of patient access.
