Re:

Medicare Premium Refund vs. Social Security Benefit Increase: What’s the Difference?

A higher Social Security deposit can reflect a benefit recalculation, a Medicare premium refund, or a changed deduction. Compare gross benefit, premiums, and covered months to find out which.

A Social Security benefit increase changes the benefit amount calculated for you. A Medicare premium refund returns money paid or withheld for eligible past months. Either can raise the amount that reaches your bank account, and a change in Medicare deductions can also alter your deposit without changing your underlying Social Security benefit. To identify what happened, compare the gross benefit, Medicare deductions, and any separate refund or retroactive credit on your notices.

Benefit increase, premium refund, and deposit: three different amounts

It helps to separate the figures that can appear on a Social Security notice or payment record:

As an Amazon Associate I earn from qualifying purchases.

  • Gross Social Security benefit: the monthly benefit calculated before Medicare premiums or other deductions are taken out.
  • Medicare premium: an amount for coverage that may be withheld from a Social Security payment or billed directly. A premium deduction reduces what reaches you; it does not, by itself, reduce the gross Social Security benefit.
  • Net payment or deposit: what remains after applicable deductions, plus or minus any separate adjustment included in the payment.

A benefit increase is a change to the benefit calculation. A premium refund is a return or correction of money paid for Medicare coverage in specified circumstances. A deposit can increase because of either one, because a deduction changed, or because more than one adjustment happened at once. The size of a deposit alone does not identify the reason.

These terms answer different questions. The gross benefit tells you the Social Security amount before deductions. The premium tells you what is charged for Medicare coverage and how it is collected. A refund tells you that some earlier payment or deduction is being returned or corrected. The net deposit is the amount that actually arrives after the applicable items are combined. A notice may discuss one of these figures without showing the full story for the others, so compare the dates and line items rather than relying on the bank deposit alone.

What counts as a Social Security benefit increase?

A cost-of-living adjustment (COLA) is a familiar reason a Social Security benefit may rise. The Social Security Administration (SSA) says the COLA is intended to keep the purchasing power of Social Security and Supplemental Security Income benefits from being eroded by inflation. Other benefit recalculations or changes can also affect an individual benefit, but they are distinct from a Medicare premium being returned.

A COLA does not necessarily mean that the increase in your monthly payment will equal the announced percentage applied directly to your old net deposit. SSA describes the calculation as an adjustment to the primary insurance amount (PIA), followed by the applicable early- or delayed-retirement factor, subtraction of offsets such as the Medicare Supplementary Medical Insurance (SMI) premium, and truncation of the final amount. As a result, SSA notes that the increase in the new monthly benefit amount over the previous amount may differ somewhat from the COLA.

The 2026 COLA figure

In its 2025 announcement, SSA set the 2026 COLA at 2.8% for Social Security benefits payable in January 2026. SSA said updated individual amounts would be available in December through the mailed COLA notice and the Message Center in a my Social Security account. The 2.8% figure is the announced adjustment for that year, not a permanent annual rate or a promise that every person’s net deposit will rise by exactly 2.8%.

The distinction matters when you compare an old and new payment. A COLA changes the benefit calculation, but it is not simply a percentage added to the amount that landed in your bank account last month. That old deposit may already have reflected Medicare premiums or other deductions. The amount SSA calculates also passes through the steps described above. For the specific new benefit amount that applies to you, use the individual notice or Message Center rather than estimating from the headline percentage.

What is a Medicare premium refund?

A Medicare premium refund is money returned or credited because premiums were paid or deducted for months that qualify for a correction or payment arrangement. It is not an increase to the Social Security benefit formula. Depending on the circumstances and how the adjustment is handled, a refund may arrive as a separate payment or appear as a correction to a payment or deduction. Check the notice for the amount, the months it covers, and the stated reason.

A refund is often about a past period, while the premium deduction shown on a current benefit notice concerns the current payment arrangement. Those can move in different directions: you might receive a refund for eligible earlier months while a premium continues to be charged for later months. Or, a premium deduction may stop for covered months and an amount already collected for those months may also be returned. Do not treat the one-time refund as proof that the current premium or future benefit has permanently changed.

State buy-in is one documented reason

SSA’s operating manual describes State buy-in as one situation in which eligible Part B premiums may be handled differently. When a State buy-in covers eligible Part B months, the Part B SMI premium no longer needs to be deducted from the recipient’s Social Security check for those covered months. If premiums were already deducted or paid for months covered by the buy-in, the recipient can also receive a refund for those months. The manual further says an excess amount paid for a closed period of buy-in coverage is refunded, and that an SMI premium refund is not counted as income for assistance eligibility.

This is a specific example, not a rule that every Medicare refund has the same cause or treatment. A notice should identify the arrangement or correction that applies to your case. Do not assume you have State buy-in, or that a payment is a buy-in refund, solely because a Medicare deduction stopped or a deposit increased.

The covered months are important. In the buy-in example, the premium deduction stops for months covered by the State arrangement, and any refund relates to premiums paid or deducted for those same eligible months. A notice about a closed period may describe a past coverage window rather than a new ongoing benefit amount. If the notice mentions buy-in, check its effective dates and the months for which premiums were returned. If the notice identifies a different arrangement, follow the explanation on that notice rather than applying the buy-in example to it.

How a deduction change can raise your deposit without raising your benefit

The deposit is the amount left after deductions. If a Medicare premium deduction falls, stops, or is corrected, the net amount may rise even though the gross Social Security benefit is unchanged. Conversely, a higher premium deduction can reduce the deposit while the gross benefit stays the same.

For 2026, SSA’s Office of the Chief Actuary lists the standard Medicare Part B premium as $202.90 per month, before any applicable income-related adjustment or other premium amount. That is a standard premium figure, not necessarily the amount withheld from every beneficiary. The same 2026 Trustees Summary states that individuals with modified adjusted gross income above $109,000 and joint filers above $218,000 pay the standard Part B premium plus an income-related adjustment amount. These are 2026 thresholds and amounts; they should not be treated as permanent figures.

Depending on the person’s circumstances, withholding can include an income-related monthly adjustment amount (IRMAA), and SSA may collect applicable premiums through benefit withholding or direct billing. Medicare Part D premiums may also be relevant to a person’s deductions. Compare the actual premium lines and periods on your own notices rather than assuming that the standard Part B figure is your full deduction.

The $202.90 figure is therefore a useful reference point, not a complete estimate of every beneficiary’s monthly Medicare cost. If your income-related adjustment applies, the Part B amount can be higher than the standard premium. If premiums are billed directly, the payment record may not show that charge as a Social Security withholding. A Part D amount or another listed adjustment can also make the total deduction different from Part B alone. Read each premium line separately and check whether the notice describes a monthly charge, an adjustment, or a separate billing method.

Compare the notices in this order

Use notices covering the same months whenever possible. A notice may describe a current monthly amount while another explains a past-period refund, so comparing unrelated dates can make a normal adjustment look like a benefit change.

  1. Find the gross benefit on each notice. Compare the current benefit amount before deductions with the prior amount. If the gross figure changed, look for the COLA or other benefit recalculation described in the notice. SSA says a COLA is applied through the benefit formula, so use the stated amounts rather than trying to infer the result from a percentage applied to your deposit.
  2. Compare Medicare deductions for matching periods. Check the Part B premium and any other listed premium or income-related adjustment. A difference here can explain a changed deposit even when the gross benefit is unchanged.
  3. Look for a separate refund, credit, or retroactive adjustment. Note whether it is a one-time payment, a correction to a prior month, or a continuing change to the monthly deduction. Record the covered months and the reason given.
  4. Check the effective date and months covered. Distinguish the month a change begins from earlier months for which a refund or correction is being made. A payment reflecting past months does not, by itself, establish a lasting increase in your monthly benefit.
  5. Read the adjustment explanation. Look for terms such as COLA, IRMAA, State buy-in, premium refund, or retroactive adjustment. If the explanation names another agency or program, use the contact information on that notice as well as any relevant SSA contact details.

For the 2026 COLA amount, SSA pointed beneficiaries to the December notice mailed to them or the Message Center in their my Social Security account. If the figures still do not reconcile, ask SSA or the agency named on the notice to explain the gross benefit, each deduction, and any payment for past months. Have both notices available and ask which months each amount covers.

A simple comparison worksheet can help keep the figures straight. For each period, write down: the gross benefit before deductions; the Part B premium; any IRMAA or other premium line; whether the premium is withheld or billed directly; any separate refund or credit; and the effective month or months identified. Then compare like with like. A current monthly amount should not be compared with a payment that includes several months of retroactive corrections as if both were ordinary monthly deposits.

If one notice gives a new monthly amount and another lists a one-time payment, keep those entries separate. Ask whether the new amount is ongoing, which month it starts, and whether the one-time figure covers earlier months. That distinction helps identify whether the ongoing benefit changed, whether the Medicare deduction changed, or whether the payment simply includes money returned for a previous period.

Illustration: the same deposit increase can have different causes

The following numbers are illustrative only; they are not an SSA estimate or a prediction of anyone’s benefit. Suppose a person’s old gross benefit is $2,000 and the total deduction is $200, leaving a $1,800 deposit. If the gross benefit later rises to $2,056 while the deduction remains $200, the deposit becomes $1,856: the increase comes from the benefit calculation in this simplified example.

Now suppose instead that the gross benefit remains $2,000, but a premium deduction that had been $200 is removed for a covered period. The resulting monthly amount before any other deductions would be $2,000. That larger payment reflects a deduction change, not a higher gross Social Security benefit. If the person also receives a separate refund for eligible past premiums, that one-time amount is a third item, separate from both the current benefit and the current premium deduction.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Actual COLA calculations and Medicare deductions can be more involved. SSA’s calculation steps, other deductions, premium adjustments, and the months included in a refund can all affect the amount that appears in a payment. Use this example to identify categories, not to calculate an individual award.

Consider how the records would differ in the two simplified cases. In the benefit-increase case, the gross figure itself rises from $2,000 to $2,056, while the deduction remains $200. In the deduction-change case, the gross figure remains $2,000, but the deduction changes from $200 to zero for the covered period. The deposits can look higher in both cases, but the gross-benefit line and premium line identify the different causes. If a refund is added for past premiums, look for the covered months and treat that amount as separate from the new recurring monthly calculation.

Common misreadings to avoid

  • “My deposit went up, so my Social Security benefit permanently increased.” Not necessarily. A refund, premium reduction, or corrected withholding can raise the net payment without changing the gross benefit.
  • “The COLA percentage should match the percentage increase in my deposit.” The COLA is applied through the benefit calculation, and the net deposit also reflects deductions. SSA says the change in the monthly benefit amount may differ somewhat from the COLA.
  • “A refund means my premium will stay lower every month.” A refund may address past months. A continuing change to a premium deduction is a separate issue; check its effective date and whether it applies going forward.
  • “The standard Part B premium is the amount everyone has withheld.” The 2026 standard premium is before any applicable income-related adjustment or other premium amount, and some premiums may be collected by direct billing.
  • “Every Medicare premium refund is a State buy-in refund.” SSA documents buy-in as one refund situation. The reason for an individual payment must come from that person’s notice or the agency handling the adjustment.
  • “A large one-time payment proves my future monthly amount changed.” A retroactive payment may cover earlier months. Confirm the ongoing monthly benefit and deduction separately.
  • “If the standard premium is $202.90, any different deduction must be an error.” The 2026 standard Part B amount is before an applicable IRMAA or other premium amount, and direct billing can mean a charge is not withheld from the Social Security payment. Check the full premium explanation.
  • “A refund and a COLA are the same kind of increase.” A COLA changes the benefit calculation; a refund returns money associated with qualifying past premiums. They can affect the same deposit, but they are not interchangeable labels.

What to do if you still cannot identify the change

Start with the benefit notice and the Medicare premium or adjustment notice that covers the same time period. If you cannot locate a notice, check the relevant mailed correspondence and the my Social Security Message Center for the 2026 COLA notice. Do not treat the gross benefit and net deposit as interchangeable when asking for help.

When contacting SSA or the agency identified on the notice, ask for a month-by-month explanation of the gross benefit, deductions, and any refund or credit. Specifically clarify whether the adjustment is one-time or ongoing, which months it applies to, and whether the premium is withheld from benefits or billed separately. Keep the notice and payment records together so you can compare the explanation with the actual amounts. The available general rules cannot establish that a particular person qualifies for a refund or explain an individual deposit without that person’s notices and benefit details.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The Social Security Administration’s stated purpose for the COLA is “to ensure that the purchasing power of Social Security and Supplemental Security Income (SSI) benefits is not eroded by inflation.” That explains why the annual adjustment exists, but it does not identify the cause of a particular payment change. For an individual case, the notice’s gross amount, premium line items, effective dates, and refund explanation are the practical evidence to check.

Frequently Asked Questions

How can I tell whether my Social Security benefit increased or I received a Medicare refund?

Compare the gross Social Security benefit before deductions on the current and prior notices. If that amount changed, look for a COLA or other benefit recalculation. Then compare the Medicare premium deductions and look for a separate refund or credit with covered months. A higher net deposit by itself does not establish that the gross benefit increased.

Does the 2026 Social Security COLA mean my deposit will rise by 2.8%?

No. SSA announced a 2.8% COLA for Social Security benefits payable in January 2026, but the adjustment is applied through the benefit calculation, and the net deposit also reflects deductions. SSA says the increase in the new monthly benefit amount over the prior amount may differ somewhat from the COLA. Check your individual notice or my Social Security Message Center for your amount.

Why did my Medicare Part B deduction stop, and could I get earlier premiums back?

SSA’s operating manual describes State buy-in as one reason an eligible Part B deduction may stop for covered months. If premiums were already paid or deducted for months covered by buy-in, a refund may be due for those months. Confirm the reason and covered period on your notice; not every premium refund has the same cause.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is the $202.90 Part B premium the amount every beneficiary pays in 2026?

No. SSA’s Office of the Chief Actuary lists $202.90 as the 2026 standard monthly Part B premium before any applicable income-related adjustment or other premium amount. The 2026 summary says individuals with modified adjusted gross income above $109,000 and joint filers above $218,000 pay the standard premium plus an income-related adjustment. Premiums may also be collected by direct billing.

Does a Medicare premium refund count as income?

In its State buy-in operating-manual example, SSA says an SMI premium refund is not counted as income for assistance eligibility. That statement concerns the documented buy-in context; it should not be generalized to every kind of refund or every program without checking the applicable notice and rules.

Who should I contact if the payment notice does not explain the change?

Contact SSA or the agency named on the notice. Ask for a month-by-month breakdown of the gross benefit, each premium deduction, any refund or credit, and the months covered. For the 2026 COLA amount, SSA directed beneficiaries to the mailed December notice or the Message Center in a my Social Security account.

The Bottom Line

A higher deposit can come from a higher gross Social Security benefit, a reduced Medicare deduction, a refund for eligible past premiums, or a combination. Compare the gross benefit, premium lines, refund amount, and effective months on notices for the same periods. For 2026, SSA announced a 2.8% COLA for benefits payable in January, while the standard Part B premium is $202.90 before applicable adjustments; neither figure alone explains an individual net deposit.
