What Lilly announced in August 2024
On August 27, 2024, Lilly said patients with an on-label prescription could buy four-week supplies of Zepbound single-dose vials for $399 at 2.5 mg or $549 at 5 mg, without insurance. These were self-pay prices for the two lower strengths, not a general reduction in the price of all Zepbound doses. Lilly’s announcement confirmed the vial prices but did not state the previous or new list price.
How much did Lilly raise Zepbound’s list price?
The exact increase and the price Lilly used as its comparison baseline cannot be verified from the available announcement and report material. The 2024 Ars Technica headline reported a list-price increase, but its article page could not be retrieved; Lilly’s release gives the vial launch prices, not the size of the increase. What Zepbound costs under Lilly’s current self-pay terms

The current figures are not directly interchangeable with the 2024 launch figures: the dates differ, and the 2024 announcement specified four-week supplies of single-dose vials while the current terms cover a 28-day supply of either vial or KwikPen. The $449 amount is an offer, not the regular price, and is not guaranteed to every patient.
Where the later prices and formats fit
Lilly’s pricing and distribution changed after the 2024 launch. On December 1, 2025, the company announced lower prices for single-dose vials through LillyDirect. On March 16, 2026, it announced self-pay KwikPen availability through LillyDirect and major pharmacies nationwide, starting at $299 a month for 2.5 mg. The 2025 vial announcement and the 2026 KwikPen announcement describe those later access changes.

When comparing a quoted price, check the prescribed dose, whether it is for 28 days or another period, whether the product is a vial or KwikPen, where it is dispensed, and whether an offer has refill or eligibility conditions. A lower advertised price at one strength does not establish the price for another.

The 2.5 mg dose is intended to start treatment; Lilly says it is not a maintenance dose. A low introductory price for that strength should not be treated as a forecast of ongoing monthly costs if a prescriber later changes the dose. Any dose change should be determined with a qualified prescriber, not made to match a price.
