Spain did top InterNations’ separate 2026 Quality of Life Index, which measures expats’ reported experiences rather than retirement suitability. The rankings answer different questions, so neither makes Spain the best choice for every American.
What the 2026 rankings say about Spain
International Living’s 2026 Annual Global Retirement Index ranks Spain eighth, with an average score of 83.7. Greece ranks first at 90.1. Spain’s category scores are Housing 83, Cost of Living 75, Visa/Benefits 83, Affinity Rating 90, Climate 79, Development 82 and Healthcare 94. These are scores in International Living’s comparative editorial index, not measurements of the outcome an individual retiree should expect. See the 2026 index infographic.
InterNations’ 2026 Expat Insider Quality of Life Index ranks Spain first among 31 countries. That index draws on expat ratings of Travel & Transit, Safety & Security, Leisure Options, Healthcare, and Environment & Climate; at least 50 respondents were required for a location to be included. Spain ranked fourth for Healthcare and eleventh for Safety & Security. The survey ran from February 1 to March 31, 2026. Its result reflects respondents’ reported quality of life, not whether an American can qualify for residency, afford a particular location, or will personally find Spain the best place to retire. The apparent contradiction is a matter of scope: one ranking compares retirement destinations across several categories, while the other asks expats about quality of life. Quartz’s September 2, 2026 coverage of International Living’s results quoted executive editor Jennifer Stevens: “For years, Portugal and Spain led the way, but recent visa changes and rising costs have retirees looking elsewhere.” That is Stevens’ explanation for the ranking shift, not a quantified finding. What expats reported about life in Spain
InterNations’ 2026 survey captures respondents’ satisfaction, not clinical measures or administrative guarantees. Among expats surveyed in Spain, 80% said they were happy with healthcare affordability, compared with 61% globally; 83% were happy with healthcare availability, compared with 71% globally; and 83% were happy with healthcare quality, compared with 71% globally. In the same survey, 89% were pleased with Spain’s climate and weather (58% globally), and 87% praised culinary variety and dining options (75% globally). These responses can help describe reported experiences, but they do not establish what healthcare will cost or be available to a particular retiree.
How an American should assess Spain as a retirement option
Start with residency feasibility
A Visa/Benefits score of 83 in International Living’s index is not an eligibility decision or a current application checklist. Determine which residence route could apply to your circumstances and verify requirements with the responsible Spanish consulate and current Spanish government instructions before making plans. Do not assume that a particular income, insurance arrangement or document set qualifies you; requirements depend on the route and can change.

Build a local housing and cost estimate
International Living gives Spain scores of 83 for Housing and 75 for Cost of Living, but those figures are comparative index scores, not a household budget or a guarantee that Spain will be cheaper than your current home. Estimate expenses for the specific places you would consider, including housing and everyday costs, and test them against your own income and budget. The index does not establish representative local rents or a cost comparison for a U.S. retiree.
Verify healthcare access for your situation
Survey satisfaction and an index score cannot establish your eligibility for Spanish healthcare, the coverage you would receive, where you could obtain care, or what it would cost. Check the applicable rules and coverage arrangements for your residence status, location and healthcare needs rather than treating Spain’s high index score as a personal access guarantee.

Account for U.S. tax obligations
Moving abroad does not by itself remove U.S. tax responsibilities: the IRS explains that U.S. citizens and resident aliens abroad remain subject to U.S. tax rules. That general guidance does not resolve Spanish tax residence, treaty treatment or how a particular pension or investment would be taxed. Test everyday fit, not just rank
A destination can score well overall and still be a poor match for an individual. Consider climate and environment, daily mobility and transport, language and social connection, leisure, safety, distance from family in the United States, and your budget and healthcare needs. These are personal factors; neither ranking can decide how much they matter to you.

How to read a “best country” claim
- Look for the publisher, index name and edition; “best” is not an official, universal designation.
- Check what the index measures: retirement-related categories and expat-reported quality of life are not interchangeable.
- Treat survey percentages as respondents’ views, not as a promise about your experience or an administrative measure of services.
- Use broad scores to narrow your shortlist, then verify residency, local costs, healthcare and tax implications for your own circumstances.
