What TrueMed does
TrueMed is a health payments platform for merchants selling health-related goods and services. At participating brands’ own online stores, it can process HSA/FSA card payments through a clinical eligibility workflow. It also supports a separate route in which a customer pays personally and later seeks reimbursement from their plan administrator.
TrueMed’s marketplace directs shoppers to participating merchants; purchases take place on those merchants’ sites. The integration is not an Amazon payment option: TrueMed says it does not appear at Amazon or other third-party retail checkouts, even if an item may qualify when bought directly from a participating brand. What makes an expense eligible?
The tax rule is medical purpose, not whether a product is described as healthy or appears in a wellness catalog. IRS Publication 502 (2025) defines medical expenses as costs of “diagnosis, cure, mitigation, treatment, or prevention of disease” that affect a body part or function. It says expenses must primarily alleviate or prevent physical or mental illness or disability; items merely beneficial to general health, such as vitamins or a vacation, do not qualify on that basis alone. TrueMed says it reviews products during merchant onboarding and assigns reviewed items to one of three statuses. A partner brand’s participation does not mean every product it sells is eligible.
- Pre-approved: TrueMed identifies the item as eligible for direct HSA/FSA payment without an LMN.
- Eligible with an LMN: The item requires a clinical intake and, where supported by the review, a Letter of Medical Necessity for a specific medical purpose.
- Ineligible or not yet reviewed: The item cannot be paid through TrueMed at that point.
Fitness equipment, gym memberships, sleep technology, supplements and nutrition, recovery and mobility tools, and health-monitoring devices are categories TrueMed says it commonly reviews—not blanket eligibility categories. Products marketed for general wellness may qualify only when the applicable medical-purpose requirements are met.

What an LMN means
A Letter of Medical Necessity (LMN) is a clinical document supporting the medical purpose of a purchase. TrueMed describes an independent licensed provider reviewing the customer’s intake; it is not simply a note obtained by completing a survey, and an in-person visit is not stated as a universal requirement. The customer’s health information and the clinician’s judgment matter for products requiring an LMN. An LMN supports a claim but does not compel an administrator to approve it.

For integrated checkout, the merchant displays an eligibility widget. The customer chooses Truemed as the payment method, completes the intake, and enters HSA/FSA card details. If an LMN is required and issued, the payment is captured. TrueMed’s merchant documentation says an unqualified payment integration order is canceled without charge; the shopper can still buy with a personal card. This is a workflow, not a guarantee of qualification.

For reimbursement, the customer retains the merchant receipt and sends it with any LMN to the plan administrator. Documentation rules, review decisions, and payment timelines vary by administrator.
What happens if a product is not eligible at checkout?
A missing eligibility option can mean the product is ineligible, has not yet been reviewed, or is not supported for the particular checkout. Check the item’s status and the merchant’s instructions. If it is not eligible through TrueMed, the customer may still purchase with a personal card; that does not establish that the expense will be reimbursable. A customer considering reimbursement should confirm the plan administrator’s requirements before relying on an LMN or filing a claim.

What merchants need to offer TrueMed
TrueMed’s current merchant guide says a business must sell to U.S. customers, offer products that can be medically substantiated, complete Stripe onboarding, and have an administrator available to make setup changes. The business need not be U.S.-headquartered, although international merchants face additional Stripe onboarding requirements.
The documentation describes Shopify, WooCommerce, BigCommerce, Stripe, and API-based integration options. It says Shopify installation is arranged through a direct link during onboarding rather than the Shopify App Store. TrueMed estimates that most implementations take 30 minutes or less; this is the company’s estimate, not a deployment guarantee. How much can customers save?
TrueMed’s consumer materials currently advertise “average 30%” savings, describing this as an estimate of tax savings from paying with pre-tax dollars—not a discount. The company says actual savings depend on the customer’s tax bracket and state.
