Start with the rejection notice: identify the state, exact form, rejecting organization and the fields or attachments it says are missing or incorrect. Fix those items using the current instructions for that form, then resubmit through a route the responsible board accepts and keep proof of submission. Workers’ compensation forms and filing rules vary by jurisdiction, so no single amendment form, deadline or delivery method applies nationwide.
First, identify what happened to the form
A portal error, a board rejection, an insurer’s objection and a document that was received but not matched to a claim are different problems. Read the message or letter closely and save it, including any receipt or reference number. A rejection notice may identify an incomplete field; a missing case identifier, for example, can prevent a document from being matched to the correct file even when the form itself was delivered.

Use the notice and the form’s official instructions to establish:
- The state and the exact form number and revision.
- Who filed it and who rejected or received it: a board, insurer, employer or submission portal.
- Whether the issue is missing or incorrect data, a missing attachment, an unreadable or altered document, an outdated form, or an unacceptable filing route.
- What the notice says to do next, including whether it calls for correction and resubmission or a different process.
Do not assume a correction procedure for one state or form applies to another. The right remedy can depend on who filed the form and whether the agency rejected it before acceptance or the information needs to be changed after filing.

Correct the form without creating a new problem
- Use the current form and its instructions. Check the issuing board’s page for the exact form. Some boards stop accepting older revisions; Georgia lists outdated forms among its rejection reasons, and New York identifies forms for which only the current version is accepted. See the New York claimant forms page and Georgia’s rejection list.
- Correct each item the notice identifies. Recheck claim or case number, injury date, claimant and employer details, insurer or claims-office details, required sections, signatures, dates, legibility and required pages or attachments. Do not guess at information that is unknown or conflicts with another record; ask the responsible board or claim administrator how to handle it.
- Make identifiers visible where required. New York’s March 30, 2026 bulletin says to put the WCB Case ID and accident date in the upper-right area of every page. If no WCB Case ID is available, use the claimant’s name and accident date. The Board warns that missing identifiers can leave a submission unmatched to the case file. Follow the exact instructions for your own jurisdiction and form. Read the New York Board bulletin.
- Check the saved document, not just the on-screen form. Open the file you plan to send and confirm typed entries appear, every required page and attachment is present, and the text is readable. New York warns that form data may not be transmitted if the completed document is not printed to PDF; its claimant and employer pages give form-specific instructions, including opening downloaded PDFs in Adobe Reader if needed. Do not alter a form in a way the issuing board disallows.
- Resubmit by an accepted route and keep evidence. Use the route specified for this form and filer. Save a full copy and any portal confirmation, tracking information or delivery record. Check whether the form also requires copies for the claimant, employer, carrier or representative.
What state examples show—and what they do not
Official instructions illustrate why a generic correction recipe can mislead. These examples apply to the named states and forms only; consult your own board’s current directions.
Georgia: field and document defects
Georgia’s State Board lists a missing Board Claim Number; incomplete Sections B, C or D on Form WC-1; a missing or incorrect five-digit SBWC ID; missing insurer, self-insurer or claims-office names and addresses; illegibility; altered or outdated forms; multiple injury dates on one form; and missing claimant or employer addresses as reasons forms are rejected. It says WC-1 and WC-14 must be typed or printed in black ink. These are Georgia-specific requirements, not a nationwide checklist. The Board’s rejection list is useful for identifying the defect, but the appropriate fix depends on the form and filing stage.

Georgia Form WC-14a is titled “Request to Change Information on a Previously Filed Form WC-14.” Its title does not establish that it amends other forms or is the right response to a rejected initial submission. Review the WC-14a itself and the Board’s directions before using it.

New York: identifiers, PDF content and submission confirmation
For New York web submissions, the Board says validation errors prompt the filer to return to the form and correct them. A successful submission returns the completed form as a PDF; if no confirmation PDF appears, the Board says it did not receive the form and the filer needs to resubmit. Save that returned PDF. Some forms also require copies to other parties. These instructions concern the Board’s web-submission process, not every filing method. See New York Web Submission of Claim Forms.

New York has also changed accepted routes: effective April 1, 2026, it no longer accepts faxed claim-related forms and documents, with exceptions for certain non-claims units. The Board lists mail, email, electronic upload, web filing and XML as routes, subject to the relevant form’s instructions. Indiana, by contrast, says forms marked with an asterisk must be submitted electronically through an approved EDI 3.1 process and that paper forms sent before electronic submission will be rejected. Check the current instructions for the exact document rather than relying on an old fax number or a route that works for another form. New York’s fax update; Indiana forms.
California: an injured worker’s claim form
California DWC instructs an injured worker to complete only the employee section of the claim form, sign and date it, send it to the employer promptly and keep a copy. If mailing the form, DWC recommends certified mail with return receipt requested to document mailing and receipt. If the employer does not provide a form, the DWC page says to download one or contact the Information and Assistance Unit. This is California guidance for an injured worker, not a general rule for other states or filers. California DWC: How to File a Claim.

Massachusetts: Form 110
Massachusetts’ Form 110 instructions say to send the employee claim and supporting documents to the employer’s workers’ compensation carrier by certified mail. The state says a rejection letter is attached to the form and identifies the missing or incorrect information. Follow those directions only if filing under the Massachusetts process. Mass.gov: File a Workers’ Compensation Claim.
When the rejection is unclear or keeps recurring
Contact the issuing board’s form or filing support unit, or the claim administrator responsible for the form, if the notice does not explain the defect or the corrected submission fails again. Give them the form name and number, the exact rejection wording, and any receipt or reference number. Ask which field, attachment, form revision or route needs attention, and how to handle information that is unknown or inconsistent.
