You can ask a medical provider to correct an inaccurate bill, reduce a valid balance, or agree to affordable payments—but first check the charges and how insurance processed them. The right next step depends on whether the bill is wrong, unaffordable, or potentially covered by a federal protection. This guide is for U.S. patients and caregivers; eligibility, deadlines, and provider policies vary.
1. Gather your bill and check what you owe
Before negotiating, assemble the provider’s bill, your insurer’s explanation of benefits (EOB) if you have insurance, any good faith estimate you received, and records of payments already made. The EOB explains how the insurer processed the claim; it is not itself a bill.
Compare the service and amount billed with the insurer’s allowed amount, adjustments, payments, and stated patient responsibility. If an item or amount is unclear, ask the provider for an itemized list. The CFPB advises: “If something doesn’t look right, ask for an itemized list of charges.” See the CFPB’s guidance on medical bills you can’t pay.
- Check for duplicate charges, services you do not recognize, or payments that have not been credited.
- If insured, compare the provider’s balance with the EOB’s patient-responsibility amount. If the claim appears to have been denied or processed incorrectly, contact your insurer about its decision and appeal process.
- Keep copies of bills, EOBs, estimates, payment confirmations, and correspondence.
2. Identify the problem before choosing a solution
A large balance alone does not establish that a bill is wrong or that a law has been violated. Work out which situation best fits before deciding whom to contact.

- Possible billing error: A charge, payment, or adjustment may be incorrect. Ask the provider to investigate and correct it if warranted.
- Insurance or claim issue: The insurer may have denied or limited payment. Ask the insurer why, and follow its appeal instructions if you disagree.
- Correct but unaffordable bill: Ask the provider about a lower balance, financial assistance, or payments you can manage.
- Possible protected surprise bill: Certain out-of-network charges may be subject to the No Surprises Act. Check whether the type of care and circumstances qualify; do not assume every high out-of-network charge is covered.
- Uninsured or self-pay estimate difference: A separate federal dispute process may apply if the bill meets its criteria.
3. Contact the provider’s billing department
Once you know what you are asking about, call the billing department or use its written contact channel. Have your account number, bill, EOB or estimate, and payment records ready. CMS notes that “Providers (or their billing departments) can sometimes lower the price.” A provider may consider a reduction or payment arrangement, but is not required to agree simply because you ask. - State the issue clearly. Identify the account and the specific charge or balance. Explain whether you are disputing an error, asking about insurance processing, or seeking help with an otherwise correct bill.
- Ask for the relevant action. Request an explanation or correction if something appears wrong. If the balance is accurate but difficult to pay, ask whether the provider can reduce it or offer an affordable payment plan. The CFPB also suggests asking whether an interest-free repayment plan is available.
- Ask what happens next. Find out who will review the request, whether you need to submit documents, and whether there is a deadline or collection activity while it is being considered.
- Keep a record. Note the date, representative’s name, what was discussed, and the next step. Ask for any agreed discount, payment schedule, and other terms in writing before relying on them.
4. Ask about financial assistance early
Ask the hospital for its financial assistance policy (FAP), plain-language summary, and application. Tax-exempt hospital organizations must have a written policy explaining eligibility, whether assistance provides free or discounted care, how to apply, and relevant billing and collection policies. - Ask how to get the policy and application, and confirm the application method and any deadline.
- Check the eligibility rules and gather the documents the hospital requests.
- Submit the application, keep a copy and proof of submission, and follow up on its status.
- Ask what happens to the bill and any collection activity while the application is under review.
If the bill is from a doctor’s office or another non-hospital provider, ask whether that provider has its own assistance program; hospital FAP requirements do not establish that every provider offers assistance. The CFPB’s overview of help with medical bills explains ways to look for assistance.
5. If you are uninsured or self-pay, check the estimate dispute process
A federal patient-provider dispute process may be available when a provider’s bill is at least $400 above a good faith estimate. The threshold is not a general right to dispute any medical bill: confirm that your care and bill qualify, and check CMS’s current submission rules and instructions. 6. Check whether the No Surprises Act applies
The No Surprises Act protects patients in specified circumstances, including many emergency services and certain out-of-network services at in-network facilities. Whether it applies depends on the facts of the care and bill. An unmet deductible by itself is not a violation: CMS states, “If you got a bill and haven’t met your deductible, that’s not a violation of the No Surprises Act.” Review CMS guidance on disputing a medical bill for covered situations and current help options, including its No Surprises Help Desk.
7. Compare payment options before using medical credit
Before paying with a credit card or medical credit product, compare the total cost, interest, fees, and payment schedule with any provider plan offered. The CFPB warns that credit cards or medical credit cards may carry high interest and can reduce your ability to negotiate. Ask about the provider’s terms first, and get them in writing. An interest-free plan may be worth asking about, but a provider is not required to offer one.

8. Escalate to the resource that fits the problem
If the provider or insurer has not resolved the issue, choose an escalation route based on who can act on it. CMS identifies state Consumer Assistance Programs and patient advocates as potential resources. Its No Surprises Help Desk can address questions about that law and assist with complaints. Find relevant options through CMS’s medical bill payment action plan.
If the account has gone to collections, the rules and options depend on the circumstances. The CFPB describes possible protections under the Fair Debt Collection Practices Act (FDCPA) and Fair Credit Reporting Act (FCRA), along with complaint options, in its guide to medical debt collection and credit reporting. For a lawsuit or a question about state-specific rights, consider qualified legal help; general consumer guidance is not a substitute for advice on your case.
