Start by checking whether you qualify for a Medicare Savings Program (MSP) through your state. Depending on the program, an MSP can pay your Part B premium and, for some people, other Medicare costs. Then check Extra Help for prescription drug costs. A Social Security check—whatever its amount—does not by itself establish eligibility: income, resources, state rules, and individual circumstances matter.

The figures below are 2026 federal screening amounts, not an eligibility decision. States may use higher allowances or different rules, so apply or ask your state Medicaid office even if you appear to be over a listed limit.

Start with the assistance that matches the cost

Medicare expenses fall into different categories, and the programs that can help are not interchangeable. Medicare Savings Programs address certain Part A and Part B costs. Extra Help addresses prescription drug costs under Part D. You may qualify for one, both, or neither; a state or Social Security determination is needed to confirm.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Part A and Part B: Ask your state Medical Assistance (Medicaid) office about an MSP. The Qualified Medicare Beneficiary (QMB) program can pay Part A premiums when applicable, Part B premiums, and Medicare-covered deductibles, coinsurance, and copayments. Specified Low-Income Medicare Beneficiary (SLMB) and Qualifying Individual (QI) help pay the Part B premium.
  • Part D prescriptions: Check whether you qualify automatically for Extra Help or apply through Social Security. Extra Help can reduce prescription drug expenses, but your plan and pharmacy still matter.
  • Other possible help: Ask about your state’s State Pharmaceutical Assistance Program, if one is available, and any assistance program for the specific medicine you take. Eligibility and coverage vary.

These are separate checks. Getting help with a Part B premium does not, by itself, answer what you will pay for prescriptions or whether a particular plan fits your doctors and medicines.

Think of the programs as separate levers rather than one all-purpose Medicare discount. If your largest expense is the Part B premium deducted from your Social Security check, an MSP is a logical first question for the state. If pharmacy bills are the strain, check Extra Help and compare Part D coverage. If both types of costs are difficult to manage, pursue both checks; receiving one benefit does not mean you should skip the other.

Check the 2026 Medicare Savings Program screening limits

The Centers for Medicare & Medicaid Services (CMS) lists the following 2026 monthly income and resource limits. They are useful for deciding whether to contact the state, but they are not a substitute for the state’s rules or a decision on your application.

Program Individual monthly income Individual resources Married couple monthly income Married couple resources What it can help pay
QMB $1,350 $9,950 $1,824 $14,910 Part A premiums when applicable, Part B premiums, and Medicare-covered deductibles, coinsurance, and copayments
SLMB $1,616 $9,950 $2,184 $14,910 Part B premiums
QI $1,816 $9,950 $2,455 $14,910 Part B premiums
QDWI $5,405 $4,000 $7,299 $6,000 Part A premiums for certain working people with disabilities

Limits shown are 2026 amounts from CMS. QDWI is for certain people with disabilities who are working and lost premium-free Part A after returning to work; it is not the general route for someone seeking help with Part B or prescription costs. The table’s married-couple figures are for a couple, not an individual living with other relatives.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The numbers describe different program screens, not a single cutoff for all Medicare help. For example, QMB has a lower income screen than SLMB or QI, but QMB can help with a broader set of Medicare-covered costs. SLMB and QI help pay the Part B premium. QDWI is a narrower program tied to disability, work, and the loss of premium-free Part A; its higher income screen should not be mistaken for general eligibility for Medicare assistance.

Why the federal figures are only a screen

States administer MSP applications and may permit higher income or resource amounts or exclude certain income or resources when deciding eligibility. The figures alone cannot tell you how your Social Security benefit will be treated in your state or whether your household qualifies. CMS advises people to apply even if they think they are over the limits. Ask the state office how it applies its rules to your situation rather than ruling yourself out based on the table.

A screening amount is a reason to ask questions, not a promise that you qualify or a reason to give up. State rules can affect how income and resources are counted, and the federal table does not show every state allowance or exception. If a simple comparison of your monthly Social Security amount with a federal figure appears to put you over, ask the state how it evaluates your full circumstances. The Medicare.gov guidance from CMS says: “Even if you don’t think you qualify, you should still apply.”

Apply for a Medicare Savings Program through your state

The state Medical Assistance (Medicaid) office is the place to start an MSP application. You do not need to assume that receiving Social Security makes you ineligible, or that a benefit amount alone determines the result. Ask the state to evaluate your circumstances under its current rules.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Contact the state office. Ask for an application or the state’s process to apply for a Medicare Savings Program. If you are unsure which office handles it, ask your state Medicaid agency how to reach its MSP application staff.
  2. Ask about state-specific allowances. Specifically ask whether the state uses higher income or resource allowances than the federal screening figures and whether it excludes any income or resources when evaluating applicants.
  3. Submit the application for a state decision. Do not treat a federal screening limit as a final denial. The state—not a comparison of your check against a table—determines eligibility under its rules.
  4. Ask which program is being considered. QMB, SLMB, QI, and QDWI do not all pay the same costs. Confirm whether the decision concerns Part A premiums, Part B premiums, or the broader Medicare-covered cost-sharing QMB can help pay.

If you receive a denial or need clarification, contact the state office and ask which income, resources, household details, or state rules affected the decision. The available federal limits do not establish what documentation a particular state requires, so follow that office’s instructions.

If you are married, use the married-couple column as the initial comparison rather than applying the individual number to the household. If you are not sure how your household should be treated, ask the state office; the listed married-couple figures are not a substitute for its determination. Likewise, do not guess which income or resources count. The state can tell you what information it needs and how it applies its rules.

QI has a timing detail worth noting: applications must be renewed each year, and states approve applications on a first-come, first-served basis, with priority for people who received QI the prior year. If QI appears relevant, ask the state about its current application and renewal process rather than waiting to find out later whether a slot is available. This annual renewal rule is specific to QI; follow the state’s instructions for any MSP application or continued eligibility review.

Check Extra Help for prescription costs

Extra Help is the federal assistance route to check for Part D prescription expenses. It is distinct from an MSP: Extra Help concerns drug costs, while MSP benefits concern specified Part A and Part B expenses.

Who may qualify automatically

According to CMS, people with Medicare who have Medicaid, receive state help with Medicare costs through an MSP, or receive Supplemental Security Income (SSI) qualify automatically for Extra Help. If one of these applies, confirm your status and how it affects your drug coverage rather than assuming that you must start a separate application.

When to apply through Social Security

If you do not qualify automatically, you can apply for Extra Help through Social Security. CMS’s 2026 handbook gives these income screens: under $23,940 a year and $18,090 in resources for a single person; under $32,460 a year and $36,100 in resources for a married person living with a spouse and no other dependents. These are screening thresholds, not a guarantee of eligibility. The handbook notes that higher-income exceptions may apply in some circumstances.

Keep the Extra Help figures distinct from the MSP table. The handbook lists Extra Help screening amounts on an annual income basis, while the MSP figures above are monthly income limits. The household description for the married Extra Help screen is also specific: it applies to a married person living with a spouse and no other dependents. If your circumstances differ, or the figures do not clearly describe your household, ask Social Security how to apply rather than extrapolating from the table.

An Extra Help application can also start an MSP application process through information sharing with the state. If you do not want Social Security to send your information to the state for that purpose, the 2026 handbook says you can ask Social Security not to share it. If you are considering opting out, understand that doing so affects this route for starting the state application; you can still contact the state directly about an MSP.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

This information-sharing route does not make the two programs identical or guarantee that the state will approve an MSP. It can help initiate a state application process, but the state still evaluates MSP eligibility under its rules. If you want to pursue both kinds of help, ask Social Security and the state what steps remain for each program.

Use 2026 costs to see where help could matter

CMS lists the standard Part B premium as $202.90 per month in 2026 and the Part B annual deductible as $283. People with higher incomes may pay a higher Part B premium, so the standard amount is not everyone’s bill. The 2026 Part D annual out-of-pocket limit is $2,100. Plan premiums and pharmacy costs vary, and that limit is not a statement of what any particular person will spend.

These figures help identify which expense to investigate, but they do not calculate your total Medicare costs. An MSP may reduce the Part B premium and, under QMB, certain Medicare-covered cost sharing. Extra Help addresses Part D expenses. A plan’s premium, covered drugs, cost sharing, pharmacies, and your expected care all affect what you actually pay.

For someone living on a fixed Social Security check, a monthly premium can be especially visible because it reduces what remains for other bills. Still, looking only at the premium can miss costs that arise when you receive care or fill a prescription. Compare both predictable monthly amounts and likely cost sharing. The standard Part B amount is a useful 2026 reference, not a guarantee that it matches your own deduction; higher Part B premiums may apply based on income.

Compare drug coverage using your own medicines

Do not choose a Part D plan by premium alone. Compare the plans available where you live using your actual medication list and pharmacies. A plan that looks inexpensive each month may not be the least expensive choice for your prescriptions or preferred pharmacy.

  1. Gather your current medication list. Use the exact medicines you take, including their strengths and dosage forms, so the comparison reflects your treatment rather than a generic example.
  2. Check each plan’s formulary. Confirm that each medicine is covered and review the plan’s cost-sharing information for it. Do not assume that coverage or price is identical across plans.
  3. Compare pharmacies. Check the pharmacies you can realistically use, including whether the plan identifies a preferred pharmacy and whether the cost differs by pharmacy.
  4. Compare total expected costs. Look beyond the monthly premium to deductibles and likely prescription copayments or coinsurance. Consider the 2026 Part D out-of-pocket limit as a limit, not a prediction of your spending.
  5. Check additional assistance. Ask your state about a State Pharmaceutical Assistance Program and check whether a program exists for a medicine you use. Each program has its own eligibility and availability rules.

Use Medicare’s plan comparison resources for options available in your area, and verify details with the plan before enrolling. The relevant choices depend on your location, medication list, pharmacy access, and eligibility for assistance.

When checking medicines, use the list you actually take—not just the drug name you remember. Strength and dosage form can matter to an accurate comparison. Then look at each plan’s formulary and cost-sharing information and check the pharmacies you can reasonably reach. A plan’s preferred pharmacy arrangement may affect costs, so a comparison based on one pharmacy may not answer what you would pay at another.

State Pharmaceutical Assistance Programs and manufacturer assistance programs for particular medicines are additional possibilities, not universal benefits. Their availability and eligibility vary by state, drug, and program rules. Confirm details through official program information rather than assuming that a medicine is covered or that a program will accept you. These options supplement, rather than replace, checking Extra Help and comparing the Part D plans in your area.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Compare Medicare Advantage and Medigap against expected care

For broader coverage decisions, compare the costs and access rules of the options available to you rather than treating a low premium as proof of savings. Medicare Advantage and Medigap costs vary; Medigap premiums depend on the policy and location. No single option is established as the cheapest for every person.

  • Monthly premium: Include the plan premium alongside the Part B premium you owe. Assistance eligibility can change the effective cost.
  • Deductible and cost sharing: Review deductibles, copayments, and coinsurance for the care you expect to use. A lower premium may come with different costs when you receive services.
  • Doctors, hospitals, and facilities: Check provider and facility access for your actual care needs. Confirm that the doctors and hospitals you want to use are available under the option you are considering.
  • Prescription coverage: Compare drug coverage and pharmacy arrangements using your real medication list; do not assume that a broader medical coverage choice will automatically be the best fit for your prescriptions.
  • Yearly exposure: Consider likely annual costs and out-of-pocket exposure, not just what comes out of your Social Security check each month.

Medicare’s comparison resources can help you review options in your area. Before choosing, check whether MSP or Extra Help eligibility changes the costs you would face under each option. A plan comparison is personal: location, providers, medicines, expected services, and the final benefit determination all matter.

Make the comparison using a realistic picture of care rather than an abstract “best plan.” Consider the services you expect to use, the doctors and hospitals you want access to, and your prescription needs. Compare premiums with deductibles and likely copayments or coinsurance, and pay attention to yearly out-of-pocket exposure. A lower premium can be offset by different costs when you use care; the comparison has to reflect your own expected use and location.

Medigap premiums depend on policy and location, and Medicare Advantage costs also vary. Check provider and facility access for the specific options available in your area. Do not assume that a plan will include a particular doctor or hospital, or that a change in coverage automatically lowers total annual spending. If you may qualify for an MSP or Extra Help, include the effect of that assistance in your comparison once your eligibility is established.

A practical order of operations

If your monthly budget is tight, handle the public-benefit checks before making a plan change solely to chase a lower premium. That order reduces the risk of overlooking help that is based on eligibility rather than a plan choice.

  1. Write down your current costs. Separate Part B premiums and medical cost sharing from Part D premiums and prescription expenses. This makes it easier to ask about the right program.
  2. Contact the state about an MSP. Ask about QMB, SLMB, QI, or—if the specific QDWI circumstances apply—QDWI, as well as state income and resource rules.
  3. Check Extra Help. Confirm automatic qualification if you have Medicaid, an MSP, or SSI; otherwise, use Social Security’s application route and ask about the state information-sharing option.
  4. Compare prescriptions and plans. Use your medicines, pharmacies, doctors, hospitals, and expected care when comparing Part D, Medicare Advantage, or Medigap choices.
  5. Verify state or medicine-specific assistance. Ask the state whether it has a pharmaceutical assistance program and check any relevant program for your specific medicine.

Do not assume that changing plans will save money, that receiving Social Security settles eligibility, or that being above a federal screening amount means no assistance is possible. Seek an actual eligibility decision and compare the costs that apply to your own coverage and care.

For a practical first call, describe the expense that is causing the most pressure and ask which program addresses it. You can ask the state about MSP eligibility and state allowances, then contact Social Security about Extra Help if you are not automatically eligible. If a state application is started through Social Security information-sharing, remember that the state still makes its own MSP decision. Keep the focus on confirmed program eligibility and the plans, medicines, and providers available where you live.

Frequently Asked Questions

Does receiving Social Security automatically qualify me for help with Medicare costs?

No. A Social Security benefit amount alone does not establish eligibility for an MSP or Extra Help. Income, resources, program rules, individual circumstances, and state rules can matter. Ask your state about MSP eligibility and check Extra Help through Social Security if you do not qualify automatically.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Which program can help pay my Medicare Part B premium?

SLMB and QI help pay the Part B premium, and QMB can pay the Part B premium as well as certain Medicare-covered deductibles, coinsurance, and copayments. The state determines whether you qualify under its rules.

Can a Medicare Savings Program help with prescription costs?

MSPs address specified Part A and Part B costs. Extra Help is the program to check for Part D prescription expenses. Some people may qualify for both, but each program has its own eligibility and benefit rules.

What if my income is over the 2026 federal MSP screening limit?

Apply or contact your state Medical Assistance (Medicaid) office anyway. States may use higher income or resource allowances or exclude certain income or resources, so the federal figures alone do not decide eligibility.

How do I qualify for Extra Help automatically?

CMS says people with Medicare qualify automatically for Extra Help if they have Medicaid, receive state help with Medicare costs through an MSP, or receive SSI. If none of those applies, you can apply through Social Security.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Will applying for Extra Help also apply for a Medicare Savings Program?

An Extra Help application can start an MSP application process through information sharing with the state unless you ask Social Security not to send your information for that purpose. The state still makes the MSP eligibility decision, and you can also contact the state directly.

What should I compare before choosing a Part D plan?

Use your actual medication list and compare formulary coverage, drug cost sharing, premiums, deductibles, and pharmacies, including any preferred pharmacy arrangement. Check options available where you live and verify plan details before enrolling.

Is the lowest-premium Medicare Advantage or Medigap option always cheapest overall?

No. Costs and access vary by plan and location. Compare premiums, deductibles, likely copayments or coinsurance, provider and facility access, drug coverage, and yearly out-of-pocket exposure against the care you expect to use.

Does QI require an annual renewal?

Yes. QI applications must be renewed each year. States approve applications on a first-come, first-served basis and give priority to people who received QI the prior year, so ask your state about its current process.

The Bottom Line

Start with your state’s MSP application process for Part A and Part B costs, then check Extra Help for Part D prescriptions. Treat 2026 federal limits as screening figures, not a final eligibility decision: state rules may allow higher limits or exclude some income or resources. Before changing coverage, compare the premiums, cost sharing, medicines, pharmacies, providers, and yearly exposure that apply to your own situation.
