When a state changes a workers’ compensation form or filing process, check the agency responsible for the claim before using a saved form. Confirm the current version, effective and required-use dates, whether older forms are still accepted, and where and how the filing must be sent. These rules vary by state and by form; there is no single nationwide workers’ comp form or transition schedule.
Start with the state and claim involved
Identify the state process that governs the particular claim and who is responsible for filing or receiving the document. Do not assume the employer’s headquarters alone determines the applicable process. Employers, insurers, claims administrators, and workers may have different roles and filing obligations.

Use the official workers’ compensation agency for that claim. State agencies maintain their own form catalogs: for example, Texas organizes workers’ compensation forms by category, while New York lists forms with information about form owners and filing destinations. The relevant state agency—not a form copied from another state or an old internal file—is the place to verify what applies.

Check the current form and the dates separately
Search the agency’s official forms page by form number and title. Review the form itself, its instructions, revision date, and any linked bulletin or announcement. Then record each applicable date: when the revision takes effect, when its use becomes mandatory, the end of any transition period, and the last date an older version may be accepted.
#1 Best Overall
Those dates can differ. Maine’s official forms index says forms revised for effectiveness on June 1, 2026, are required as of September 1, 2026. That schedule is specific to Maine; it is not a general transition rule for other states. See the Maine Workers’ Compensation Board forms index.
Do not assume an old version remains acceptable just because it is still available in a shared folder or on an unofficial site. New York says the Board may announce that it will not accept older versions of updated forms. Georgia’s forms page said outdated forms would be returned beginning October 1, 2025. Check the policy for the particular form and jurisdiction: New York forms and Georgia forms list.
Verify the filing channel, recipient, and companion steps
A form update may change more than the document. Confirm the required submission method, recipient copies, portal, and whether a separate electronic transaction is needed. An upload may not complete every step in the claim system.

A New York payer update dated April 9, 2026, says use of the RFA-2 eForm became mandatory effective April 8, 2026. Paper forms postmarked after April 7 would not be accepted or acted on. For this transition, use the newer payer update rather than relying on older planning information on the eForms page. Check the New York RFA-2 payer update for the applicable instructions.
Pennsylvania: LIBC-764 and WCAIS
Pennsylvania’s LIBC-764 instructions describe mailing the notice to the employee and uploading it to WCAIS or mailing it to the Bureau. The instructions also say an EDI transaction is needed to update claim status in WCAIS, so sending or uploading the document alone may not update the status. The PDF carries an April 2023 revision date; verify that the form and instructions remain current before using them. See the Pennsylvania workers’ compensation forms page and the LIBC-764 instructions.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Use official sources, and distinguish guidance from binding requirements
Official agency forms and instructions are a starting point, but check the legal status of any accompanying guidance. Connecticut’s Workers’ Compensation Commission tells parties to use its official forms to ensure documents are up to date, accurate, and meet statutory requirements. Its forms page provides PDF forms, most of which are fillable online.
Idaho cautions that its policy statements and guidance do not have the force and effect of law. That distinction matters when a guidance page explains a process but the controlling requirement may come from a statute, rule, or official form instruction. Consult the relevant authority or ask the agency, carrier, or claims administrator when the directions are unclear. See the Idaho Industrial Commission forms and resources.
Put the update into practice
- Identify the controlling claim process. Confirm the state, the form’s purpose, and which participant must file or receive it.
- Open the state agency’s current form page. Search by form number and title; inspect the revision date, instructions, and linked notices.
- Write down the transition dates. Separate the effective date from the required-use date and any cutoff for older versions.
- Confirm how to submit. Check the portal or mailing address, recipient copies, and any companion EDI or status transaction.
- Replace saved copies and filing aids. Update internal templates and checklists, notify staff or vendors who prepare filings, and clearly retire superseded forms to reduce the chance someone selects one accidentally.
- Keep the official notice with the process record. If requirements remain unclear, ask the responsible agency or the carrier or claims administrator rather than relying on an unofficial reproduction.
Updating templates and retaining notices are practical controls, not universal agency mandates. The binding filing requirements come from the applicable jurisdiction’s current law, rules, forms, and instructions.
Compare the details before choosing a version or route
When the agency offers a transition or more than one submission route, check these points before filing:
Quick Recap
- Which version is current for this form and claim?
- When does the update take effect, and when is use mandatory?
- Is the previous version accepted during a transition, and what is its last acceptable date?
- Must the document be submitted electronically, on paper, or through a specified portal?
- Who must receive a copy?
- Is an EDI transaction or other companion step required in addition to sending the form?
- Does the form differ by filer, such as employer, carrier, employee, or self-insured entity?
