Request your CLUE insurance information through the LexisNexis Risk Solutions Consumer Disclosure portal. You can submit the request online or print and mail a request form. The CFPB says eligible consumers can get one free report every 12 months.

Go to the LexisNexis Consumer Disclosure request page.

Select “Request Your Consumer Disclosure Report.” Depending on applicable law, the portal says an eligible request may provide a Fair Credit Reporting Act consumer disclosure and a state privacy act report.

Enter the identity and contact information requested by the form. For relevant requests, LexisNexis requires at least one of a Social Security number or a driver’s license number and issuing state. The company may be unable to fulfill a request if it cannot verify your identity or match your details to its records.

Submit the request and watch for a message by U.S. Mail. The portal advises contacting the Consumer Center if no message arrives within 10 days; this is a follow-up suggestion, not a guaranteed report-delivery deadline.
Request by mail or ask for help
If you prefer not to use the online form, the portal provides printable request forms and mailing instructions. Follow the current instructions and include the information needed to verify your identity.

The LexisNexis portal currently lists 1-888-497-0011 for its Consumer Center. The CFPB listing gives 866-897-8126 for LexisNexis Risk Solutions consumer reports. Contact information can change, so check the current LexisNexis portal or the CFPB listing and explain that you need a CLUE-related consumer disclosure.
What a CLUE report contains
CLUE stands for Comprehensive Loss Underwriting Exchange. The CFPB describes it as a claims information exchange used to inform insurance pricing and underwriting. According to the CFPB, the report can include up to seven years of auto claims and up to seven years of home and personal property claims. The CFPB’s live listing does not state a publication year for this guidance.

Claims information is supplied by insurers. An insurer may request a CLUE report when you apply for coverage or ask for a quote, according to the Tennessee Department of Commerce and Insurance. When you receive your report, compare the listed names, dates, loss types, and claim details with your own records.
Dispute incorrect or incomplete information
If you find an error or missing information, dispute it with both LexisNexis, the consumer reporting company, and the insurer or other company that supplied the information. The CFPB says the Fair Credit Reporting Act requires the reporting company to conduct a reasonable investigation free of charge. The information supplier must correct an error and notify consumer reporting companies to which it provided the inaccurate information.

Keep a copy of the report and identify each disputed entry clearly.

Send supporting records that help explain the error, such as documents showing the correct date or claim details.

Follow the current dispute instructions from LexisNexis and the information supplier. Texas Department of Insurance guidance also says consumers can dispute incorrect information with LexisNexis or add an explanation.
If an insurer denied coverage, raised your rate, limited coverage, or canceled a policy, LexisNexis says consumers who received an adverse action letter can contact its Consumer Center for information related to that action. Follow the contact instructions in the notice and verify the current route through the LexisNexis consumer site.

The CFPB lists one free report every 12 months for eligible consumers. The Texas Department of Insurance similarly tells consumers, “You can get a free copy of your report each year by contacting LexisNexis,” and directs them to select the “Request a Consumer Disclosure Report” button. The Texas statement is state regulator guidance; the CFPB describes broader federal consumer reporting rights.
