How Social Security Benefits Affect Taxes and Medicare Costs

Social Security may be partly taxable, and its taxable portion can affect Medicare IRMAA income—but the tax and premium calculations differ. See how filing status, the two-year lookback, and 2026 thresholds fit together.

Social Security benefits can be taxable, and the taxable amount can affect the income figure Medicare uses to set certain premiums—but neither outcome is automatic. For federal taxes, the IRS applies a formula based on filing status and other income. For Medicare’s income-related monthly adjustment amount (IRMAA), the Social Security Administration (SSA) generally uses modified adjusted gross income from a tax return filed two years earlier. The rules are connected through income, but they use different calculations and timelines.

When are Social Security benefits taxable?

For federal income tax, some recipients may have to include part of their Social Security benefits in taxable income. The result depends on filing status and income from other sources; receiving benefits alone does not determine taxability.

The IRS calculation uses half of your benefits plus other income to determine whether none, up to 50%, or up to 85% of your benefits is included in taxable income. The 85% figure is the maximum share of benefits that can be taxable—not an 85% tax rate. Your ordinary tax rates apply to the amount included in taxable income.

Filing situation Base amount Important qualification
Most single filers $25,000 Used in the IRS benefit-taxability calculation; other income and benefits affect the result.
Married filing jointly $32,000 Used for a joint return; the calculation considers the couple’s income and benefits.
Married filing separately, lived with spouse at any time during the year $0 This filing situation has a distinct rule; do not substitute the single-filer base amount.

These are base amounts for the IRS worksheet, not deductions or income limits that guarantee a particular tax bill. Use the worksheet in IRS Publication 915 for the relevant tax year and your circumstances. The IRS says that if the only income received during 2025 was Social Security or the specified Social Security Equivalent Benefit portion of tier 1 railroad retirement benefits, benefits generally are not taxable and a return probably is not required. “Generally” matters: other filing requirements can still apply.

To estimate your own result, start with the total benefits and filing situation for the tax year, then follow the publication’s worksheet using the other income it asks you to include. The worksheet determines how much of the benefits, if any, is taxable. Do not treat a base amount as a point at which every dollar of benefits suddenly becomes taxable: it is part of a calculation that considers benefits and other income together. Also, “up to 50%” or “up to 85%” describes the portion of benefits included in taxable income, not the percentage of your total income you owe in tax.

Filing status deserves particular care. The $25,000 amount applies to most single filers, while the joint-return base is $32,000. Married taxpayers filing separately who lived with a spouse at any time during the year have a $0 base amount, a different situation from simply using the single-filer figure. Check the IRS publication for the exact year and filing circumstances rather than carrying a figure forward from another year.

How taxable benefits feed into Medicare’s IRMAA calculation

IRMAA is the income-related monthly adjustment amount added to certain Medicare premiums. The taxable portion of Social Security benefits is included in federal adjusted gross income (AGI) when reported on a tax return. SSA defines IRMAA modified adjusted gross income (MAGI) as AGI plus tax-exempt interest income. As a result, taxable benefits can raise the income measure SSA considers.

That connection does not mean that having taxable Social Security automatically triggers an IRMAA surcharge. SSA compares the applicable MAGI figure with thresholds for the premium year and filing status. A person can owe federal tax on some benefits and remain below the relevant IRMAA threshold; conversely, other income can push MAGI over a threshold even if Social Security is not the main source of income.

The two calculations answer different questions:

  • IRS: How much, if any, of your benefits is included in federal taxable income? The IRS worksheet considers half the benefits, other income, and filing status.
  • SSA: Does your tax-return AGI plus tax-exempt interest fall into an IRMAA tier for Medicare? The answer depends on premium year, filing status, and the return SSA uses.

See SSA’s MAGI definition for the Medicare income measure.

The link between the calculations is the tax return: the taxable-benefit amount that the IRS worksheet produces is part of AGI, and AGI is the starting point for IRMAA MAGI. SSA then adds tax-exempt interest. That means the portion of benefits that is not taxable is not added to AGI as taxable benefits through this calculation; the portion that is taxable can contribute to AGI. It still takes the full MAGI calculation and the relevant threshold to determine whether an IRMAA amount applies.

For a useful comparison, keep four items together: the return’s tax year, the Medicare premium year, your filing status, and the income measure used in each system. A tax estimate for one year cannot be compared directly with a premium notice for another year without accounting for SSA’s lookback. Likewise, comparing AGI alone with an IRMAA threshold can miss tax-exempt interest, which SSA adds when determining MAGI.

What income thresholds and premiums apply in 2026?

For premium year 2026, SSA lists initial IRMAA MAGI thresholds of $109,000 for an individual filer and $218,000 for a married couple filing jointly. Married people filing separately have separate rules and tiers, so the single-filer figure should not be applied to every non-joint return.

The standard monthly Medicare Part B premium for 2026 is $202.90. People whose MAGI falls into an IRMAA tier pay a higher Part B premium. Part D IRMAA is a separate income-related amount added to the premium charged by the person’s Part D plan; it is not a replacement for the plan premium. Exact tier amounts and filing-status rules are listed in SSA’s IRMAA sliding-scale tables and its Medicare premiums information.

Think of the initial threshold as the entry point to the applicable income-related tiers, not as the amount of a surcharge. SSA’s tables show the sliding-scale amounts by income range and filing status. For example, a person should check the table for the specific premium year and filing status rather than assume that crossing the initial threshold means every enrollee pays the same higher amount. Part B and Part D are also distinct: an IRMAA adjustment affects the Part B premium, while the Part D adjustment is added on top of the plan’s own charge.

Thresholds and premiums are specific to a premium year and can change. The 2026 figures above should not be used to estimate a later year without checking SSA’s current tables.

Why can a 2026 Medicare premium reflect income from 2024?

SSA generally uses the latest federal tax-return information available, which is usually from two years before the Medicare premium year. For 2026, that generally means SSA uses 2024 tax-return information. The tax year on the return and the premium year on the notice are therefore different.

This timing can make a premium feel out of step with current finances. A high-income year—perhaps followed by retirement or another income reduction—may still be the year SSA has available when it initially determines a later premium. Check the determination notice to see which tax year and filing status SSA used, then compare the income figure with the return for that year.

The lookback is a data-timing rule, not a statement that your current income is necessarily the same as it was two years ago. The notice identifies the determination and provides a starting point for checking it. First verify the tax year and filing status shown; then compare the relevant return information with the notice. If you are comparing the notice with a newer return or a current income estimate, remember that those figures may not be the information SSA used for its initial decision.

How to check whether taxable benefits affected your premium

  1. Identify the two years. Note the premium year on the Medicare or SSA notice and the tax year of the return SSA says it used. For a 2026 determination, the usual lookback return is 2024.
  2. Recalculate benefit taxability for the return year. Use the IRS Publication 915 worksheet for that tax year and your filing status. Do not use the 2025 tax-year base amounts for a different year without checking that year’s publication.
  3. Find AGI on the return. The taxable benefits calculated for the return are part of federal AGI. Use the return SSA relied on, rather than a current-year estimate, when checking the original determination.
  4. Add tax-exempt interest for IRMAA MAGI. SSA’s measure is AGI plus tax-exempt interest. Compare that figure with the correct premium-year threshold for your filing status.
  5. Check the premium components separately. Part B IRMAA changes the Part B amount; Part D IRMAA is added to your plan’s own Part D premium. Confirm which charge changed before drawing conclusions.

This sequence can identify whether taxable benefits contributed to the income SSA used, but a personal tax or premium result depends on the full return, filing details, and applicable SSA tier.

A practical way to keep the comparison straight is to write down the calculation chain rather than compare the benefit amount directly with a Medicare charge: benefits and other income go through the IRS worksheet; the taxable portion is reported in AGI; tax-exempt interest is added to AGI for IRMAA MAGI; then that MAGI is matched to the SSA table for the correct premium year and filing status. Each step answers a different question, and skipping one can lead to the mistaken conclusion that Social Security alone caused a premium increase.

Keep a copy of the SSA determination notice and the tax return year it references while checking the figures. If the notice reflects a different filing status or return year than expected, or if you cannot reconcile the income measure, use the notice’s instructions to contact SSA. Recheck SSA’s tables each year: both threshold amounts and premiums can change, so a prior-year comparison is not a reliable substitute for the current premium-year table.

What to do if your income has fallen

If your current income is lower than the income on the tax return SSA used because of a qualifying life-changing event, you may ask SSA to review the IRMAA determination using more current information. SSA says beneficiaries can contact the agency and may use Form SSA-44 to request a lower IRMAA determination. The form and SSA’s instructions explain the qualifying events and information needed; a lower income by itself does not establish that a request will qualify.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use the income and event details SSA requests, and review the resulting determination for the premium year and income data it reflects. For other changes, or if you think SSA used the wrong return or filing status, contact SSA using the instructions on your notice.

Do not assume that every income decline qualifies for an adjustment or that submitting a request guarantees a lower premium. The request is for SSA to review the determination based on a qualifying event and the information required by its process. Before contacting the agency, identify the tax return cited in the notice, the premium year, and the event and income information you can document; then follow SSA’s directions, including any directions on Form SSA-44.

Keep tax and Medicare years straight

The IRS figures cited here—the $25,000, $32,000, and $0 base amounts—are for tax year 2025. The $109,000 and $218,000 initial thresholds and $202.90 standard Part B premium are for Medicare premium year 2026. They are not interchangeable year labels: one describes the tax return’s year, the other the year Medicare charges a premium.

SSA also announced a 2.8% Social Security cost-of-living adjustment for 2026. That adjustment is general benefit context, not an IRS taxability base amount or an IRMAA threshold. A COLA does not, by itself, determine whether benefits are taxable or whether an IRMAA tier applies.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

This explanation concerns federal income tax and Medicare IRMAA. It does not determine state tax treatment or cover every Medicare premium or assistance program. Because the relevant amounts and thresholds can change, consult the IRS publication for the tax year at issue and SSA’s premium information for the premium year at issue.

Frequently Asked Questions

Are Social Security benefits taxable?

They can be. For federal tax, the IRS worksheet considers filing status, half of your benefits, and other income. Depending on the calculation, none, up to 50%, or up to 85% of benefits may be included in taxable income. The 85% is not a tax rate. Use Publication 915 for the specific tax year.

Does taxable Social Security count toward IRMAA?

The taxable portion of Social Security is reported as part of federal AGI. SSA generally calculates IRMAA MAGI by adding tax-exempt interest to AGI, so taxable benefits can contribute to the amount SSA considers. Taxable benefits alone do not automatically mean you owe an IRMAA surcharge; the result depends on total MAGI, filing status, the premium-year threshold, and the return SSA uses.

Why is my Medicare premium based on income from two years ago?

SSA generally uses the latest federal tax-return information available, which is usually from two years before the premium year. For 2026, that generally means a 2024 return. Check your notice to see which return year and filing status SSA used.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can I ask SSA to lower IRMAA if my income has dropped?

If your income fell because of a qualifying life-changing event, you may contact SSA to request a review using more current information. SSA says beneficiaries may use Form SSA-44. A lower income by itself does not establish eligibility; review SSA’s instructions and provide the event and income information requested.

Are Part B and Part D IRMAA the same charge?

No. Part B IRMAA raises the Part B premium. Part D IRMAA is a separate income-related amount added to the premium charged by your Part D plan. Check the SSA premium information and your plan premium separately.

The Bottom Line

Social Security benefits can affect Medicare costs through the taxable portion that enters AGI, but taxability and IRMAA are separate calculations. Check the IRS worksheet for the right tax year, add tax-exempt interest when determining IRMAA MAGI, and compare it with SSA’s premium-year tier for your filing status. For a 2026 premium, SSA generally looks to 2024 tax information; if income has fallen after a qualifying life-changing event, ask SSA about review options.
