A microprocessor knee alone is estimated at $25,000–$75,000 or more; it is a component, not a complete leg. These are provider estimates, not guaranteed prices or a prediction of what an insured patient will owe.
What does “bionic leg” mean?
The term can refer to a complete fitted prosthesis, an advanced knee, or a foot-and-ankle system. Those are different things to price. A complete prosthesis may include a custom socket, liner and suspension, a foot, a knee when needed, and clinical services such as fitting and alignment.
For example, Ottobock describes its Genium as a microprocessor knee with features including stumble recovery and stair and obstacle functionality. Its product page does not state a price, and the knee is not a complete fitted-leg package.
U.S. price estimates by system
Quantum Prosthetics & Orthotics’ 2026 guide gives these broad estimates for device and fitting. The figures are clinic estimates, not a nationally representative price survey, guaranteed list prices, insurer payment amounts, or patient out-of-pocket amounts.

An above-knee setup with a microprocessor knee, custom socket and other components may cost more than the basic above-knee range. A NIST-hosted report from 2011 described computer-assisted devices starting around $20,000–$30,000; that is a historical figure, not a current price estimate.
Why quotes vary
- Amputation level: An above-knee prosthesis needs a knee joint, making it more complex than a below-knee system.
- Components and technology: Mechanical or microprocessor knee choice, foot type, socket and suspension design, and specialized features change the prescription and cost.
- Fit and services: Evaluation, custom fabrication, alignment, fitting visits, gait training, adjustments and maintenance may be included or billed separately. Ask the clinic to specify what is bundled.
- Mobility goals and functional ability: A system intended for household walking may differ from one for variable-speed community mobility or higher activity. Component recommendations and eligibility can depend on documented functional need.
- Insurance terms: Deductibles, coinsurance, network status, prior authorization and plan criteria affect the patient’s share. A device estimate is not the same as an approved amount or final bill.
- Future care: Socket changes, repairs and replacement components can add lifetime expense. Current sources do not establish a reliable national dollar estimate for these later costs, so request a separate estimate and service schedule.
How Medicare and private insurance affect the cost
Medicare’s Lower Limb Prostheses coverage policy, revised effective April 1, 2025, covers fluid, pneumatic or electronic/microprocessor knee components for beneficiaries classified at functional level 3 or above. Some beneficiaries at level 2 may qualify when clinical documentation supports functional benefit, lower-level options have been ruled out and additional criteria are met. For a microprocessor knee, those criteria include stumble-recovery capability, the ability to charge daily, and the ability to understand and respond to device alerts. Coverage policy does not guarantee approval for an individual claim.

Private policies differ. Anthem’s policy, published October 1, 2026 and reviewed August 13, 2026, lists microprocessor knee and foot-and-ankle systems and specifies medical-necessity criteria. Anthem notes that a code’s inclusion or exclusion does not by itself determine a member’s coverage; the member’s contract must be checked. Confirm the current policy and benefits directly with the insurer.
No single national out-of-pocket amount is established by the cited sources. To estimate your share, get the clinic’s proposed components and billing estimate, then ask the insurer to verify network status, authorization, deductible, coinsurance, benefit limits and replacement rules.

What to request in a written estimate
- Ask whether the estimate is for a complete prosthesis, a knee component, or a specific service bundle.
- Request an itemized list of whether evaluation, custom socket, liner and suspension, foot, knee, fitting visits, alignment, gait training and follow-up are included.
- Ask which components are recommended for your functional level and goals, and why.
- Ask what alternatives could meet those goals at different price points.
- Have the clinic and insurer clarify the approved amount, estimated patient share and assumptions used to calculate it.
- Ask which repairs, socket changes, liners and replacement components may be billed separately, and request a future service schedule.
Ottobock Care’s cost guidance also recommends discussing component needs, insurance, included services, replacements and financial assistance with a clinic. If considering a replacement liner or other accessory, confirm the exact type and compatibility with your prosthetist.
