How Initial Access Brokers Can Expose Mid-Sized Businesses to Ransomware

Initial access brokers can sell network footholds that ransomware operators may use later. The evidence does not show that they target mid-sized businesses at a higher rate, but practical identity, remote-access, patching, and recovery controls can reduce risk.

Initial access brokers (IABs) obtain or sell a foothold in an organization’s network; a ransomware operator or affiliate may then use it to carry out a later attack. Official advisories document this criminal handoff, but the available evidence does not show that IABs target mid-sized businesses more often than other organizations.

What an initial access broker does

An IAB specializes in getting into a network and making that access available to another criminal actor. That access can become a commodity in the ransomware ecosystem: the broker’s role may end before the buyer carries out the rest of the intrusion.

CISA and its partners documented one example in their June 2025 update to a joint advisory about Play ransomware. The advisory said multiple ransomware groups, including IABs with ties to Play operators, exploited CVE-2024-57727 in the SimpleHelp remote monitoring and management tool after the vulnerability was disclosed on January 16, 2025. This is evidence of one exploited route, not proof that IABs generally rely on remote-management vulnerabilities or that the affected organizations were mid-sized.

Why the mid-sized-business claim needs qualification

The available official reporting connects IAB activity with ransomware, but does not establish a higher IAB targeting rate for mid-sized firms. A statistic from Sophos gives useful context about incident response, not targeting: Sophos reported that ransomware accounted for over 90% of its Incident Response cases involving organizations with 500–5,000 employees in 2024. That is the share of Sophos’s cases in that segment, not the percentage of all mid-sized businesses attacked.

Separately, a joint CISA/FBI/ASD advisory reported that, as of May 2025, the FBI was aware of approximately 900 entities allegedly exploited by Play ransomware actors. That is a group-specific reported count; it is neither an estimate of IAB victims nor a mid-sized-business total.

How attackers may get a foothold

Credentials and exposed or poorly secured remote services are among the routes described in CISA guidance. The SimpleHelp case also illustrates why organizations should review remote-management software for known vulnerabilities. These are examples, not a ranking of entry routes for mid-sized businesses.

  • Compromised credentials: A stolen or otherwise compromised account can give an attacker access to email, a VPN, or systems used for administration.
  • Remote services exposed to the internet: CISA warns that attackers often use exposed, poorly secured remote services for initial access. The FBI advises disabling direct internet-facing Remote Desktop and using brokered access instead.
  • Unpatched software: Vulnerabilities in internet-facing, edge, or management tools can provide another route in. The SimpleHelp advisory is a reason to include remote-management products in vulnerability reviews, not a claim that one product or flaw explains ransomware broadly.
  • Third-party access: Managed service providers and other vendors may have legitimate access to business systems. Excessive permissions or weak boundaries can increase the consequences if that access is compromised.

What can happen after access is sold

Encryption may be only one part of a ransomware incident. CISA notes that actors may deploy ransomware late in an intrusion to obscure earlier activity, and may steal data and threaten to publish it before or alongside encryption. A response plan therefore needs to account for both disrupted systems and possible data theft.

Practical controls for a mid-sized business

Strengthen identity controls

Require phishing-resistant multifactor authentication (MFA) for email, VPN, and accounts that reach critical systems. Choose an approach compatible with the organization’s identity provider, and make sure privileged accounts and account-recovery procedures are covered. A FIDO2 security key can be one way to implement phishing-resistant MFA where the relevant services support it; a key alone does not protect accounts or workflows that are not configured to use it.

Reduce and constrain remote access

Inventory internet-facing services and remove those without a business need. For remote access that must remain, enforce identity checks, grant only the permissions needed for each role, log use, and set clear boundaries for vendor access. Review third-party security practices and formalize expectations with providers, including who can access which systems.

Patch exposed and known-exploited systems

Keep applications, firmware, and software updated, and prioritize vulnerabilities known to be exploited. Include edge devices and remote-management tools in the review rather than treating them as ordinary back-office software.

Limit lateral movement and prepare restoration

Segment networks where appropriate so an attacker who reaches one system has fewer paths to others. Keep offline backups, protect them from deletion or tampering, document a recovery plan, and exercise restoration. An external drive may hold an offline copy, but the value of that backup depends on isolation, rotation, access controls, and successful restore tests—not the drive by itself.

Choosing controls and services

Decision What to assess
MFA Phishing resistance, compatibility with the identity provider, privileged-account coverage, and account recovery.
Remote access Whether services are exposed to the internet, identity enforcement, least privilege, logging, and vendor-access boundaries.
Backup and recovery Offline isolation, safeguards against deletion or tampering, restore testing, and whether recovery can meet the organization’s needs.
Managed services Coverage hours, escalation authority, incident-response scope, evidence retention, and limits on provider access.

CISA recommends offline backups and a recovery plan, but does not prescribe one drive or product as a complete solution. If internal staff cannot provide the needed monitoring or response capacity, assess a managed security provider against the same access, escalation, and evidence-handling requirements.

What to do if ransomware is suspected

  • Use the organization’s incident-response plan and involve qualified incident responders as appropriate.
  • Investigate suspicious logins and newly created or escalated accounts as part of determining what was accessed.
  • Preserve relevant evidence and coordinate reporting through official channels. CISA and the FBI encourage prompt reporting whether or not the organization chooses to pay.

The FBI says payment does not guarantee restored access. A decision about payment should not replace containment, evidence preservation, recovery planning, or reporting.
