A check stub—also called a pay stub or pay statement—shows how an employer’s earnings calculation becomes the amount paid to you. A useful template makes the pay period, payment date, earnings, deductions, gross pay, net pay, and (often) year-to-date totals easy to identify. The fields can vary by payroll system and state law, so treat a free template as a layout example, not a universal legal form.
What a check stub tells you
A check stub connects the work period and pay calculation to the payment you receive. Review the earnings categories, deductions, and totals together: a net-pay figure alone does not show how the amount was calculated or establish that tax withholding is correct.
The IRS’s payroll-record example illustrates common entries, including hours, rate, gross pay, Social Security, Medicare, federal and state withholding, insurance, other deductions, and net pay. Actual statements may use different labels or omit optional sections.
What each field and section means
Pay period begin and end
These dates mark the span of work covered by the payment. They are not necessarily the same as the payday.

Payment date or check date
This is the date the payment is issued. A statement can therefore show work performed during an earlier period and a later payment date.
Employee and employer information
Names and identifying details associate the statement with the worker and employer. The exact fields depend on the payroll system and applicable requirements. A sample template may include addresses and a masked Social Security number; avoid putting a full SSN in a public or shared example.

Tax status and W-4 information
Some statements display filing status or withholding elections. Federal income-tax withholding depends on the employee’s current Form W-4 information and the employer’s calculation method. The IRS publishes year-specific methods in Publication 15-T. An old “allowances” field should not be assumed to describe the current W-4 process, and a template is not a withholding calculator.
Hours, rates, and earnings
Hourly statements may break out hours and rates for regular work, overtime, bonuses, or other pay. Separate entries help you see which work and rate produced each amount. For example, Maryland says that where multiple rates apply, its pay statement must report all rates and hours at each rate, including straight-time and overtime.

Gross pay
Gross pay is earnings before the deductions shown on the statement. Check the included earnings categories and totals rather than assuming every payment receives identical treatment for every tax purpose.
Deductions and withholdings
Common entries include federal income-tax withholding, Social Security, Medicare, state withholding, insurance, and other deductions. Which items appear depends on the worker’s situation, employer benefits, and payroll setup. Compare each listed amount with its label; tax withholding and benefit deductions are not interchangeable categories.

Net pay
Net pay is the payment amount remaining after the deductions reflected on the statement. A basic check is whether the displayed gross amount, deductions, and net amount reconcile according to the statement’s categories. That arithmetic check does not by itself confirm that the withholding calculation is correct.
Current period and year to date
Current-period figures describe this pay period. Year-to-date (YTD) figures accumulate across the year. The IRS explains that paired period and YTD columns help employees reconcile withholding during the year; use the current column to review this payment and the YTD column to track the running total.

Leave balances and messages
Templates may include leave balances or a messages area, but these are not universal pay-stub fields. Maryland’s pay-statement FAQ says its pay-stub law does not require leave balances; a separate state sick-and-safe-leave rule may require a written balance statement.

- Choose a layout that fits the pay calculation. For hourly pay, look for space to show hours and rates by earnings type, including overtime when applicable. For salaried or mixed compensation, make sure the earnings categories can be labeled clearly.
- Keep the covered period distinct from payday. Fill in the period begin and end dates separately from the payment date so the work being paid for is unambiguous.
- Enter earnings before deductions. Show gross pay and its components, then list each withholding or deduction under its own label. Do not treat the template’s example tax fields as advice about what an individual owes.
- Separate this-period figures from YTD totals. If the template has both columns, enter each amount in the correct column and keep the YTD totals cumulative for the year.
- Reconcile the result and protect personal data. Check that the listed categories and totals produce the displayed net payment, and use masked identifiers in examples or files shared outside the payroll process.
- Check the rules for the relevant jurisdiction. Employers should confirm state and local pay-statement disclosure rules rather than assume a generic template meets them.
Smartsheet’s free pay-stub PDF is one example of a downloadable layout with employee information, payments, deductions, YTD columns, leave balances, totals, and a messages area. It is an example, not a form established as valid everywhere. A spreadsheet can also track the categories if a payroll system does not provide a suitable view.
What federal rules do—and do not—require
For FLSA-covered employers and covered nonexempt workers, the U.S. Department of Labor says: “There is no required form for the records, but the records must include accurate information about the employee and data about the hours worked and the wages earned.” Its recordkeeping list includes identifying information, hours worked, wage basis and rate, straight-time and overtime earnings, additions or deductions, total pay per period, payment date, and period covered. These are federal employer recordkeeping rules—not a universal federal pay-stub template mandate. Tax and W-2 matters fall under IRS authority.
State pay-statement laws may impose additional or different disclosure requirements. Maryland, for example, says its template is voluntary and that properly completing it satisfies the state’s stated pay-statement requirements. That state-specific example does not establish what another state requires. Employers should check the applicable state labor agency and any relevant local rules.
