The notice reportedly alleged “significant lapses” in planning and oversight and sought an explanation over possible non-compliance with pilot duty and rest rules. A later government inquiry documented thousands of cancellations and delays, identified operational and management shortcomings, and reported penalties and reform requirements.
What did the DGCA notice say?
Contemporaneous reporting said DGCA issued the notice to Pieter Elbers on December 6, 2025, asking him to explain the disruption and what it described as significant lapses in planning and oversight, including prima facie non-compliance related to pilot duty and rest requirements. The notice itself was not available in the sources reviewed, so its date and wording are based on secondary reporting.
Scroll reproduced this sentence from The Hindu’s account of the notice: “Whereas as the CEO you are responsible for ensuring effective management of the airlines, but you have failed in your duty to ensure timely arrangements for the conduct of reliable operations and the availability of requisite facilities to the passenger,” the DGCA said. How severe was the disruption?
The Ministry of Civil Aviation’s inquiry account describes the disruption as occurring from December 3–5, 2025. Its January 17, 2026 release reported 2,507 cancelled flights and 1,852 delayed flights, inconveniencing more than 300,000 passengers. These are the official figures for that event, not a measure of any later disruption. See the Ministry’s inquiry summary.

What did the later inquiry find caused the cancellations?
The Ministry said the inquiry committee identified several connected failures, rather than attributing the event to a single cause:
- Over-optimisation of operations.
- Inadequate regulatory preparedness.
- Deficiencies in system software support.
- Shortcomings in management structure and operational control.
The Ministry’s published summary stated that these were the primary causes found by the committee. During the response process, IndiGo had reportedly said that several factors had compounded and that it could not immediately identify the precise root causes. That preliminary airline account and the committee’s later findings are distinct statements. The Ministry’s summary records the inquiry’s conclusions; Moneycontrol reported IndiGo’s response.

What action did DGCA and the government take?
Warnings and accountability measures
The January 2026 government account reported that the CEO received a caution for inadequate oversight of flight operations and crisis management. It said the COO was warned for failing to assess the effects of the winter schedule and revised flight-duty-time limitations (FDTL) requirements. The senior vice-president responsible for the operations control centre was warned, directed to leave current operational responsibilities, and barred from assignment to an accountable position. The release also reported warnings for other flight-operations, crew-planning, and roster-management officials. IndiGo was directed to act on any other personnel identified by its internal inquiry and submit a compliance report. The Ministry’s release describes these measures.
Penalty and reform-linked guarantee
The Ministry reported a total penalty of ₹22.2 crore and a ₹50 crore bank guarantee. Release of the guarantee was to depend on independent DGCA verification of reforms in leadership and governance; manpower planning, rostering, and fatigue-risk management; digital systems and operational resilience; and board-level oversight with sustained compliance. The same government account said specified revised FDTL provisions remained subject to continued non-compliance for 68 days, from December 5, 2025, through February 10, 2026. These are findings and enforcement measures reported in January 2026; the available account does not establish that the guarantee has since been released or that every reform is complete. Passenger support during the crisis
In a December 5, 2025 statement, the government directed automatic full refunds for cancelled flights, accommodation for passengers stranded by prolonged delays, enhanced flight information, and assistance for passengers needing priority support. These were directions issued for that crisis; they should not be read as guaranteed remedies for a separate or later disruption. Read the government’s December 5 statement.
Was Pieter Elbers still IndiGo’s CEO in 2026?
No. Elbers was the CEO identified in reporting on the December 2025 notice. IndiGo’s investor-relations material says Willie Walsh assumed the CEO role on August 3, 2026, so Elbers should be described as the airline’s then-CEO, not its current CEO as of October 8, 2026. The notice was an early accountability step; the later inquiry and enforcement account addressed both individual responsibility and system-level changes. For passengers, the event’s disruption figures and temporary support directions provide context for what happened in December 2025, but do not establish current passenger entitlements for another cancellation. Check the airline and government guidance that applies to the specific flight and date.
