Cashless and reimbursement claims are two ways to pay a hospital bill—not two levels of insurance coverage. With cashless treatment, your insurer or its third-party administrator (TPA) pays an approved amount directly to a hospital in its current network after preauthorization. With reimbursement, you or your family pay the bill first and then ask the insurer to assess the claim. In either case, payment depends on your policy’s terms and the expenses the insurer finds admissible.
What is a cashless claim in health insurance?
A cashless claim is a direct-payment arrangement between the insurer or TPA and a network hospital. The hospital requests preauthorization, and the insurer approves payment up to the authorized amount, subject to the policy. “Cashless” does not mean every charge is covered or that you will have nothing to pay.
IRDAI says a cashless facility is offered only at network providers that have an arrangement with the insurer. Check the insurer’s current hospital list and confirm the process before a planned admission; network status can change. See IRDAI’s health-insurance FAQ and health department guidance.
How does reimbursement work?
For reimbursement, you or your family generally pay the hospital and submit a claim to the insurer afterward. IRDAI says reimbursement may be allowed at any hospital or medical establishment, subject to the policy and applicable licensing or registration rules. That does not make every treatment or expense payable: the insurer still assesses the claim under the contract.

Reimbursement is also a possible route when cashless settlement is unavailable or a cashless request is declined, but follow your policy’s notice and claim procedures. Ask the insurer how to proceed rather than assuming a cashless decline automatically converts to an approved reimbursement claim.

What the policy may still leave you to pay
Neither route changes what the policy covers. The payable amount can depend on the policy’s limits, exclusions, co-payment, deductible, room-rent terms, sub-limits, and the particular treatment and expenses claimed. The general IRDAI guidance cannot determine these details for an individual policy. Check your policy wording and ask the insurer about the claim in question; do not treat a cashless authorization as a promise to pay the entire final bill.

How long does an insurance claim take in India?
IRDAI’s health department guidance, referencing its Master Circular dated 29 May 2024, states that insurers should decide a cashless preauthorization request immediately and within one hour. It also states that final cashless authorization should be issued within three hours after the hospital’s discharge authorization request, and that claims other than cashless should be settled within 15 days. These are stated service timelines, not guarantees that a claim will be accepted or that the full amount claimed is admissible. Before admission: what to check
- Confirm the policy is active. Review your policy details and contact the insurer if you are unsure about its status.
- Check the hospital’s current network status. Use the insurer’s current list, then confirm directly with the insurer or hospital that cashless service is available for your admission.
- Ask how preauthorization works. Confirm who submits the request, what information is needed, and how you will be told the decision.
- Ask about the policy’s claim procedure. Check notice requirements, forms, supporting records, deadlines, and how to claim if cashless is not available or covers only part of the bill.
- Keep records and copies. Ask which documents your insurer requires. Depending on the claim, useful records to retain may include itemized bills, receipts, discharge papers, prescriptions, diagnostic reports, and claim communications. These are examples to check against your policy, not a universal regulatory checklist. A simple folder can help keep originals and copies organized.
IRDAI’s health-insurance FAQ says insurers should not ordinarily treat documents absent from the policy terms as necessary, except where fraud is suspected, and that required documents should be requested together rather than piecemeal. Follow your insurer’s current published procedure and keep copies of what you submit.

What if your cashless claim is denied or the amount is disputed?
Ask the insurer for the decision in writing, including the reason and the policy condition it relies on. IRDAI’s FAQ says a denial or repudiation communication should give reasons, refer to policy conditions, and include grievance-redressal details. Check whether the issue is the hospital’s network status, missing information, a policy condition, or a difference between the bill and admissible expenses before deciding what to challenge.
- Write to the insurer’s grievance channel. Explain the disputed decision, refer to the claim and policy, and attach the relevant records and communications. Keep a copy and note the response.
- Escalate through IRDAI’s Bima Bharosa complaint channel if unresolved. The online form asks for a policy or claim identifier for claim complaints and accepts PDF, JPG, JPEG, and PNG attachments. Use the official Bima Bharosa portal.
