Bill Gates’s May 2025 charge that Elon Musk was helping “kill the world’s poorest children” was a forceful accusation about the consequences of U.S. foreign-aid cuts—not a verified finding that Musk personally caused a particular child’s infection or death. Gates pointed to USAID-supported HIV-prevention services in Mozambique; Musk denied the claim. Later reporting documented deaths and severe deprivation associated with aid cuts, but the total impact and individual responsibility remain difficult to establish.

What did Bill Gates say about Elon Musk and USAID?

In interviews reported on May 9, 2025, Gates criticized reductions to U.S. foreign assistance and linked Musk’s role in the Department of Government Efficiency (DOGE) to cuts at the U.S. Agency for International Development (USAID). The Washington Post reported Gates’s remarks from an interview with the Financial Times, including: “The picture of the world’s richest man killing the world’s poorest children is not a pretty one.”

Gates also said, “I’d love for him to go in and meet the children that have now been infected with HIV because he cut that money.” The wording was Gates’s characterization of the stakes and responsibility; it was not a judicial or scientific determination of personal causation.

What was the Mozambique HIV-prevention example?

Gates cited USAID grants supporting a hospital in Mozambique’s Gaza Province and services intended to prevent mothers from transmitting HIV to their babies. The example concerned health services and the systems around them, not a single vaccine shipment.

Fortune reported that USAID contracts worth $33 million for a charity operating in Gaza, Mozambique, were cut. Elizabeth Glaser Pediatric AIDS Foundation executive Trish Karlin described supported work that included health-worker support, counseling, testing, blood samples used to monitor patients, psychosocial-support training and logistics. Fortune also reported concerns that medicines could reach a country without a functioning system to distribute them to pharmacies. These reports describe threatened or disrupted services; they do not establish that a specific child became infected or died because of a named person’s decision.

How did Musk respond?

Fortune reported that Musk called Gates a liar and challenged him to provide evidence of children harmed. Musk also said USAID programs he considered useful had been transferred to the State Department rather than deleted.

That was Musk’s account, not independent confirmation that every useful service continued without interruption. The competing claims concern both the status of particular programs and the consequences of withdrawing or transferring their funding; those questions cannot be settled by either side’s characterization alone.

Did USAID cuts affect children?

In a 2026 retrospective, the Associated Press reported deaths and serious deprivation associated with foreign-aid cuts in several countries. It described children in Myanmar, Nigeria and Somalia dying after food-ration or nutrition programs were cut, as well as deaths among pregnant women and babies in Nepal after nutrition and prenatal-care programs were affected. These are reported cases linked to aid reductions; they do not establish that Musk personally caused any particular death or provide a precise toll attributable to USAID decisions alone.

The AP said experts expect the full effects to take years to measure and that the overall impact may never be known. One reason is that cuts eliminated jobs held by front-line workers who collected mortality data. The U.S. State Department told the AP: “The rest of the world needs to contribute more and share the burden.” That statement explains the administration’s position on sharing aid costs; it does not resolve the reported consequences.

What do the child-mortality figures show—and what don’t they show?

The Gates Foundation’s 2025 Goalkeepers Report projected that the number of children dying before age five would rise by just over 200,000 in 2025, reaching an estimated 4.8 million. This was a global projection amid several pressures, not a final observed count and not an estimate of deaths caused solely by USAID cuts. The report also discussed scenarios involving lower health funding.

In a June 2025 Gates Notes post about his testimony to Congress, Gates argued for the importance of health aid. That is a first-person advocacy account of his case, rather than an independent evaluation of the effects of any specific program or cut.

What can be concluded about responsibility?

The evidence supports a careful distinction between an accusation, reported service disruptions, documented cases associated with aid cuts and proof of direct individual causation. Gates made the accusation and identified a concrete HIV-prevention example; Musk denied it and disputed the account of what happened to programs. Later AP reporting documented grave consequences associated with cuts in multiple settings, while also describing major limits on measurement.

Those findings show why the dispute matters without establishing a precise number of deaths caused by USAID cuts, much less by Musk personally. Association with a funding change, a plausible pathway to harm and proof that a particular decision caused a particular death are different standards of evidence.
